Held-up as a standard in dementia care, MOSAIC honors the personhood of each individual by understanding that dementia is a disability in only specific areas of the brain, not the whole person. As a result, our strengths-based programming works to utilize the parts of the brain that are still functioning properly leading to engagement and enhanced quality of life. When you visit Azura you will witness our everyday use of MOSAIC programming through creative expression, small group peer interactions, special interest clubs, one-on-one engagements with caregivers, outdoor activities, small group outings, daily themes, church services, supervised baking, music and much more! In addition, you will be visiting a home that was specifically designed to support and engage those with memory loss throughout their disease process.

We offer a wide range of Alzheimer's, dementia and memory care services at Azura Memory Care of Beloit including: 24-hour memory care; on-site respite care; high caregiver to resident ratio; on-site beautician services; electronic charting system; medication management; diabetic management, including sliding scale insulin; registered nursing services; in-home podiatry and dental services and offer on-site physician oversight. In addition, our home works in collaboration with area home health and hospice providers to assist with higher medical needs including wound care, physical therapy, occupational therapy, speech/swallow therapy and end-of-life care. Azura also provides daily behavioral and medical monitoring and is sit-to-stand and Hoyer lift capable.

My tour experience with Azura Memory Care of Beloit was fine. The director was very thorough. The facility was clean and the residents were well cared for. The staff during the tour was very polite and very caring. The rooms were compact. The outdoor facility was very welcoming. They had a great room, a dining room, and the halls were in basically a circle, so the residents could easily find their rooms. They had outdoor patios. There was nobody out though because it was cold. They're already closed up for the season.

Azura Memory Care of Beloit's people were exceedingly pleasant and caring, but the facility was very shopworn. They have two buildings and they seem to have duplicated some things, but the residents have to move from one building to another, which is fine during the summer time, but is a bit of a problem during the winter. They have their own patented memory care activities, which they were quite proud, so there are plenty of activities. The person in charge of the activities was very enthusiastic and she had everything planned. People there have been employed there for a long time so that speaks for the place.

Azura Memory Care of Beloit was a very nice, small place that didn't have lots of amenities, but the staff was nice. It looked like a pretty caring place. The rooms were nice and looked like collage dorm rooms.

THIS FACILITY WAS GREAT FOR MY MOM. THEY TOOK HER IN AT THE LAST MINUTE WHEN I WAS ADMITED IN THE HOSPITAL. THE WERE INFORMATIVE WHILE I WAS THERE. SHE REALLY LIKED THE PLACE AND THEY WERE FABILOUS WITH HER. I WOULD HIGHLY RECOMEND IT TO ANYONE.. I WILL BE USEING THEM AGAIN IN THE NEAR FUTURE..

We liked the look of the Harbor House as we drove up. We had been in the region looking at facilities for our uncle. The exterior appeared very well maintained. The grounds were nicely landscaped, with many tree-lined seating areas. There were ample communal rooms for every activities like reading in the library, card games, physical exercise, crafting and hosted meetings. There was a computer center and they add activities as suggested by residents, family and friends. Excursions included many interesting cultural events in Beloit -- which also hosts music and art gatherings. There was also a large room for interesting group events. All units were well designed, efficient and yet still spacious. The living spaces were spacious and beautifully decorated with lots of fresh air and light. The dining room had plenty of space. The menus all had healthy food options for all kinds of diets. The food we saw looked and smelled delicious. Healthy snacks are offered all day and into the evening, as well as whenever requested. The fitness center included regular classes, and personal trainers to ensure safe usage to assist seniors in every way possible. We were relieved to see that this facility can accommodate a variety of ambulation levels and as a resident's needs change, care can be shifted accordingly This is a well-designed living environment for seniors that includes everything the residents require to ensure a content experience with kind and caring staff.
