About This Community
We are a long term care and rehab facility. We have a 32 bed specialty program for those suffering from Alzheimer's and related dementia. This unit is secure, has a secure outside courtyard, activity based program through out the waking hours 7 days a week. This is the Solana Program. This unit has a seperate group of staff who are didicated and educated in strategies to work with this population and a seperate program director who with nursing oversight oversees and directs the program, admissions and transfers for residents in middle stages to late stage whom may benefit from the activity programming.

Recently had to place my grandmother here. She was terrified to come into this type of a setting. This facility took the time to find out what mattered most. They ensured that she was met upon admission and even had a few of her favorite things set up in her room. They took the time to make sure she felt safe an comfortable. I have seen great improvment since her rehab has started, and we are looking to discharge to home soon. A few weeks ago I thought for sure she would never be able to go home. The therapy is excellent she has more energy, and gets around so much better. I cannot thank them enough for giving her back her independence. She loves the food, and has a safe spacious room to stay in, I was really impressed with the variety of activities they offered. Highly reccomend

Do not leave your loved ones in this facility!! I wish my family had been warned. My grandmother was here for less than a week. The staff was incompetent, she was not cared for properly. My family's experience with Swanton Valley was truly a nightmare. If I can prevent anyone else from being hurt by these people, I hope this will help. They were very convincing that they had a great "5 star" facility, but we stayed with her as much as possible as we could see as soon as she was in there, it was not what they promised - Not even close! Unfortunately, we couldn't be there every minute. They gave her the wrong prescriptions, or didn't even have what she was prescribed to give her. The staff disregarded all of our concerns and questions, and treated us like we were overreacting when we were concerned about things that were or were not being done. If you care about your loved one's well being, find another facility, or better yet, look into home health care. Do NOT leave them at Swanton Valley!

Swanton Valley Center was an older nursing home. They had mostly semi-private rooms that were just separated by a curtain. If your loved one was in very advanced stages of Alzheimer’s and basically didn’t know where they were, they have a very secure locked-down unit, but it was a very tiered nursing home. The lady who took me around was very nice, and I met very few other staff. I didn’t see evidence of a lot of activities -- limited activities and limited amenities.
