Visiting Nurse Services

• Establish a plan of care with the patient and family
• Evaluate and assess patients based on their diagnosis, signs/symptoms, and labs
• Complete wound care assessments and dressing changes
• Maintain catheter care and drains
• Manage IV therapy and tube feeding
• Educate on disease processes (Pre- and Post-Operative), medication regimen, diet, and nutrition
• Parkinson's & Alzheimers Disease Treatment

Visting Physical Therapy

Maintain home safety and establish a program for patients with the following:

• Limited mobility
• Gait Disturbances
• Limited ability to transfer
• Loss of Range of Motion, Strength, and Muscle Tone
• Musculoskeletal Pain

*Affinity Home Care visiting Physical Therapists train and educate patients on prosthetics and Durable Medical Equipment

Visiting Speech Therapy

Rehabilitate patients with the following conditions:

• Hearing Loss
• Dysphasia
• Dementia/Cognitive Problems
• Aspiration Pneumonia
• Language disorders
• Non-verbal communication

Visiting Home Health Aides

• Assists with personal care hands-on
• Assists the patient with activities of daily living while the nurse is completing the plan of care

Visitng Occupational Therapist

Establishes a home program for patients with the following conditions:

• Cerebrovascular accidents
• Exacerbation of Chronic Obstructive
Pulmonary Disease (COPD)/ Arthritis
• Upper body weakness or trauma

Affinity Home Care Visiting Occupational Therapists Assist and train patients with transfers, activities of daily living, and fine motor coordination

Visitng Medical Social Worker

• Assess the social/emotional aspects pertinent to the patient diagnosis and environment
• Assist with plans for long-term care and disease management
• Counsel patients on adaptation to the disease limitations
• Educate and provide information on community resources related to the patient's diagnosis
