About This Community

At Brook Gardens Place, we are a smaller senior living community where everyone knows our residents' and their family members' names. We develop individual care plans to meet the needs of each resident, and a full-time registered nurse is available 24 hours a day to provide clinical oversight and coordination of care. Our community welcomes pets and offers activities to help residents thrive in mind and body, such as exercise classes, arts and crafts, bingo, happy hour, volleyball, king's corners, movie and popcorn nights, sing-alongs, and church services. Our residents also enjoy trips to the outlet mall in Johnson's Creek, going to farmers markets, and participating in community events. We strive to offer high quality assisted living in Lake Mills to provide our residents with the care and attention they deserve.

Brook Gardens Place on the inside was very nice, the nursing and the staff seemed nice personally. The outside didn't have that much, I couldn't see the back side because they had construction going on. We like a place with a view. They were surrounded by homes, so there wasn't much of a view. That was the only drawback, there was not much of a view to look out over anything. You're just surrounded by homes and businesses. They were very informational, very helpful, and giving me the straight, honest, straightforward suggestions. They let me know up front. She said, "We'd hate to have him come here, and then if he gets worse he'd have to transfer somewhere else." Their meals smelled awesome. I only saw the model room.

Brook Gardens Place had a very nice facility, but it was just that much farther for my family to have to go to see my mother. We went there six weeks ago. They had like a one-bedroom. They have a large common area with a TV. Everything looks very clean. They did have transportation if she had a doctor's appointment. They had a library. They also had a beauty salon/barbershop where somebody came in twice a week to do hair. I believe that they also did laundry per person per day. So it wasn't like they gathered up everybody's laundry and then you hope you got your clothes back.

Brook Gardens is a great place for loved ones to be truly cared for when the time arrives. In the two + years my mother-in-law was at Brook Gardens I experienced many things that the Staff/Caretakers and all administrators & Sadi, the Manager help to make this place run smoothly. I stopped in many times in the years/months my mother-in-law was there (1-2 times a week plus her son 1-2 times a week) and found a lot of not only follow through of requests or questions I have had but also the whole place is kept clean, tidy and cheery. One can see "Kindness & Love Matters" at Brook Gardens. The custodian is also a huge team player and very personable, helpful and went the "extra mile" to help my mother-in-law with comforts of life she needed. Their personable style spoke excellently over the phone also--all the times that mode of communication was used. I was impressed with all the care Brook Gardens exhibited to people --that were not their own relatives--but someone else's! Truly they were going for the one big happy family! They have lots of activities for the residents to partake in if they chose. Always something going on there from Thanksgiving Dinners, Halloween fun, Christmas gatherings and musicians scheduled! They sent out timely announcements too so we always knew --and tried to come --after being invited! They have great meals and specific meals to accommodate the residents special diets--if need be. The pictures on their computer site are true i.e. airy, nice windows for viewing outside feeling. (They also allowed a dog but we found a home for him instead. Now, that truly tells you they care!) I would highly recommend this place for a loved one...if you need a place. And Sadi, the Manager is awesome. She goes over and above to help with not only the important paperwork but follows through with the smiles and chatting to really get-to-know her residents. I have seen her roaming on weekends also---going the truly extra extra miles for clients there. Thank you Sadi! (One time making a chocolate shake herself for my mother-in-law to help her feel calm and safe. Bless you--what a lady!) May my mother-in-law rest in peace now --as she slipped away in a cozy comfort style. Her whole family knows--Brook Gardens helped make that wish happen. Thank you so much again!

I thought Brook Gardens was really nice, and I was really impressed. When you walk in the door, they have a gorgeous dining room. It seemed like they might have a little better care, and I am just assuming from what the administrator told me. They told me that they have a lot of activities. It smelled nice, and the layout of the place seemed really nice.
