Welcome to Hillsville Rehabilitation and Healthcare Center. We are a 60-bed skilled nursing facility that is conveniently located approximately 12 miles from Twin County Hospital in Galax. Our center provides a wide range of skilled nursing and rehabilitation services for both short stay recovery, as well as ongoing supportive services for individuals requiring long-term care. Our focus is on improving the quality of life for those we serve through our orthopedic, neurological, cardiac, pulmonary, wound care services as well as interventions for other medically complex diagnoses, admitting 24 hours a day, 7 days a week. At Hillsville Rehabilitation and Healthcare Center, it is our goal to make your transition from hospital to home a pleasant and productive experience. The rooms need to be cleaned painted and updated.
The staff as for the CNA's some are very helpful and good. Residents are not keep as clean as they should be. Nor are they changed into clean clothing. The lead team seems to not be on board with each other. If your love one is in extra care they seem to fight against each other in the care for them. Communication between staff and family is nonexistent. Call's are not answered information is not shared with family.

The rooms are outdated, furniture old in bad need of repairs, paint chipped, and doors are missing wood. Color scheme is the burgundy, navy, dark brown from 20 years ago. I will note that there administration area is newly remodeled, contains cherry furniture and appears bad new and expensive. Speaks volumes of their priorities.
We only stayed a few days before recognizing this was a horrible place. Staff are in constant chaos, on their phones, and seem overworked or unconcerned. The equipment bedside commode, call bells, etc. are in disrepair or scream cheap "purchased in bulk from Amazon". Therapy equipment was nowhere to be found. Food often served cold and without any "prepared with love" feeling. Our elderly and disabled populations deserve better. There's a facility 100% better in Laural Fork. I would only live here if you were chosing between homelessness and here.

My mother stayed at Trinity Mission for ten weeks. It was not as clean; there were a lot of bad smells. Most of the staff members were kind and professional, but they were understaffed quite a bit. My mother was in a shared room, and the bathroom was not kept clean. The food was sometimes good and sometimes not good. I understand they have two different cooks. My mother wasn't interested in any activity, but they seemed to have a lot of activities that residents attended. They need to improve on staffing and cleanliness.
