Not a good facility. Would give zero stars if that was an option. I removed my mother from their care. My mother is thriving in her new facility in Maryland. Woodbine left my mother in soiled briefs for hours on end up to 12 hours at one point when on floor 1N. My mother had multiple bed sores and was never positioned correctly in bed. She was not fed her meals on many instances. You have to fight to get a nurse to check on your loved one; and on top of that I had to take my mother to the hospital myself, given a later diagnosis of a heart attack on one instance (when I noticed my mother very weak and fatigue) COVID on another, and Urosepsis during another instance during her less then 8 month Woodbine stay when my mother's heart rate was high (Tachacardia) for over 12 hours, she was sweating profusely and had reddened skin. They said she was fine, just anxious. Woodbine refused to call 911. I saved my mother's life more then twice in that facility. Back in May my mother was throwing up blood and when I told the nurse, she mocked me and said "oh no, call 911 she threw up" next day I decided to call 911 and take my mother to hospital for the third time now, and my mother's diagnosis was a positive result for Covid 19 and pneumonia. Woodbine is not a facility for someone who needs total care, or any real assistance.

PERIOD OF CARE: August - October 2020! This is an Absolute Awful place. They almost killed my brother who was in there for physical rehabilitation. That included speech therapy, occupational and physical therapy. They gave him 2 medications that shut his bladder down. Then they put a catheter in and stopped giving him fluids. They were apparently too lazy to empty the bag. I didn’t see him because of the Pandemic and I had no idea how sick he had gotten. I finally took him to a urologist about the catheter and he collapsed in the parking lot. He was rushed to the ER and spent 3 weeks on IV antibiotics. In addition, while he was in the hospital , his electric shaver, Galaxy S9 phone charger and some clothes were stolen! To make matters worst, they still intend to collect a co-pay. I filed charges against Woodbine with The Adult Protective Services for Abuse and Neglect DO NOT TRUST YOUR LOVED ONE TO WOODBINE

This is not a facility you want to send your loved ones. They will do the bear minimum and let your loved one decline until they must be taken to the emergency room. My father was so malnourished and dehydrated that he almost died. They did not bring him to the hospital until the last minute. Although mentally incapacitated, if he said no to something they would leave him alone and so he slowly deteriorated until he almost died. They will spend more time being defensive and arguing/avoiding you than taking care of the patient.

Poor wound care, poor medical supervision. If you have an option, go elsewhere.

They are under staffed. There nursing team need training. You can find the sleeping late at night in patients room. The food is awful. The rehabilitation puts up a good front until the patient is a resident. The therapist live on their evaluation yet they're being paid by Medicaid and Medicare. I had to go to the top to have my sponse re-evaluated. The supervising nurse comes to meeting chewing gum. Very unprofessional. Most of the patients on the ground floor have serious bed sores. This facility needs to be shut down.

My wife and I are regular visitors, and we can't speak highly of the Woodbine Staff. They are incredibly cheerful, creative, engaged, and professional! We love them. They do a fantastic job, especially now during these difficult and challenging coronavirus times.

My 90 year old father (who is mostly confined to a wheelchair) was at Woodbine for 3 weeks following a 10 day hospital stay and we were very pleased with the care they took of him. I visited every morning and the staff was always extremely friendly. the facility was very clean, with no "nursing home" smell to it at all. Most importantly Dad was very happy there, liked the meals and loved everyone who took care of him. I can honestly say that he was in much better shape when he left than he had even been in the previous 6 months. I would highly recommend Woodbine.

I'm a bit disappointed that you would put on your website that PT is available 24/7. My Mom was transferred there on Saturday afternoon and now, Sunday night, has had no interaction whatsoever with a Physical Therapist. She was told that none are there on weekends. How is that 24/7?

Our Mother was recently in Woodbine for rehab. We were very pleased with the facilities and staff during her two week stay.

This place is a mess. The workers do not care much about there patients and the rooms wear the patients stay smells so bad. They never clean it. The only time they clean it is when the inspector comes. The workers slack off and care only about themselves.

Avoid this place like the plague. Staff is unresponsive and borderline incompetent. Medications are not dispensed on schedule without constant badgering of the staff. Meals are low quality. Little to no food available outside of scheduled hours. Facility other than the rehab center is in DIRE need of maintenance and upkeep. Amenities close to nonexistent. DO NOT put anyone you care about in this place.

Rooms are disgusting. The staff, even the Administrator and Assistant Administrator don't care. My friend is constantly in a stench filled room with various incontinent roommates. My interactions with the staff are I complain and nothing happens. There are no amenities no activities unless you want to go on trips with mentally challenged people. I am working on getting my friend out of Woodbine. It's the worst place ever.

RUN!!! PLEASE do not be fooled. At first look this place looks spacious and more hospital like than other options in the area... far from it! The care of the nurses is awful, and my mother in law was there most of the time with my father in law who was a resident there... I can only imagine the poor treatment to those that didn't have caring family members fighting for them. My father arrived there with all kinds of issues, but stable, not being able to eat much nor drink much... the first thing we got was a CNA fighting with us because they wouldn't give him not even a spoon of water to drink, followed 15 min later by the food cart with the days meal for him.. PIZZA! these people should be out of business. In 2.5 weeks that he was there my father went from 135lbs to 105lbs... from being stable and ambulatory to not eating or drinking and in pain all the time. I wouldn't have this people watch my dog much less ever again care for a love one. Shame of them.

Low quality service provided by therapists. Sometimes trainees working alone with elderly needing serious therapy support. Not clean rooms with roaches on 3rd floor and parts of other floors Less attention paid to the most elderly with no family visitors No attention paid by so called medical specialist to complaints of chest pain or difficulty in breathing by elderly on long term stay.

I was here for 6 days to recover from a traumatic accident. If you are very ill or beaten up and need care not available at home, a brief stay is perhaps ok. The PT and OT people are excellent: positive, knowledgeable, take initiative, and are honest. The gym is equipped quite well for what is needed. That said, the procedures for completing what is needed and being discharged are draconian and complex, seemingly designed to maximize time there, overseen by nice but evasive social workers. I literally had to stage a form of politely insistent jailbreak after a doctor failed to appear to clear me two days in a row despite assurances they would do so. The system of discharge seems designed to maximize your stay, hence $. Doctors are basically ghosts whose comings and goings are closely guarded secrets or misleading in their "schedules.." In six days I saw an NP once and a doctor I ran into in the hall who agreed to prescribe something I needed for pain med constipation--which I had to advocate for. The nurses are nice enough but competence is spotty. The food is abysmal, unhealthy, unappetizing, and disheartening. If you have a loved one there, get meals from outside and avoid lunch and dinner. Breakfast is OK but not healthy for hearts. Outside this looks appealing, inside only the lobby has any beauty. The hallways are dark and crowded, sometimes with beds of profoundly compromised people. The rooms are small and confining. It smells of urine and since it is under construction, renovation it is dirty and messy and noisy. I would do anything possible never to return except to say thank you to a few people who helped me understand my view was not crazy or cranky.

I do not know about the past but yesterday it is a nice place to rehabilitate and recuperate. my father was there ten years ago-he walked out after a month-fully recovered. my mother went in yesterday. the place is undergoing renovation-looks nice when they get done. the food has much improved with mom getting crab cakes for dinner tonite. the staff is alert and attentive to mom's needs and the questions have been quickly and completely answered. the staff is alert and aware of needs-you hit the call button and one of the ladies shows up-what more can you ask? I recommend them, and I have relatively high service standards as I am in the service business as well. maybe the low ratings are from people with an "attitude" rather than one of respect and dignity for us who serve others.

Woodbine has 24-hour care, and they are approved for Medicaid. The staff was very nice, attentive, and understanding to my mother’s needs.

My mother was at Woodbine Rehabilitation & Healthcare, and it was a good facility. It was nice enough, and everybody was caring. It gets pretty good ratings from what I have seen. It is also Medicare and Medicaid certified. I don't think that it was the best for long-term care because mom ended up staying in there longer than she thought she would, and it just wasn't as good; the facilities were not as nice as what I hoped they would be for a nursing home. But I thought the physical therapist and everybody was great because they really worked hard to make them better. So as far as a rehab facility, it was good. But as far as long-term care or nursing home care, it was not so good. I know they had church every Sunday -- a couple of different denominational services -- and they have a couple of open houses during the year where they invite all the families and everybody in; they're nice.

Brought my dad there from hospital. Less than 10 hours later the nurse practioner was telling us that we needed to sign paperwork for last rights. Via phone, told my sister to get my dad out of there fast as possible and back to our hospital he was just transported from in Woodbridge. We were told we would have to wait for the Docter who was in a meeting and then found out later, the docter was only there once a week :(. Then was told they would send him to the Alexandria hospital vice the once that had all his records. Told my sister BS, and demand he be set immediatly back to originating hospital. They made her sign a wavor saying they were no longer responsible for our decision and finally let him go. Two days later he is sitting up in hospital room. While in there when i arrived, started out great, they cleaned him up, changed his bandages, and seemed like it was going to go great. Rooms are really small and since their shared, the tv is far against the wall with no speakers next to the person. So only way to listen for the older folks is to turn the tv up loud which makes the other person their sharing room not to be able to sleep. Additionally my dad was a fall risk person and beds have no railing on sides to prevent them from turning and falling out of bed. I highly recommend never to use this place. Last but least paremedics who took him away confirmed the above information that the front of building looks modern, and the back of it a very dirty and nasty place.

Our mother is presently in Woodbine, and we are NOT happy. The nursing staff does not appear invested or concerned. They dont even have the wherewtihall to speak as individuals pass them in the hass to visit their loved. That simple gesture would make them "appear" professional and concerned. It definitely makes you question what else may be lacking in terms of customer service. We have not felt assisted during this process as the staff seems so desensitized. To their credit, there has been a staff member or nurse that has been helpful, but the breakdown in communication and last minute changes are very frustrating. Policies and procedures are what they are but, as humans, i would think that one person in the whole facility would consider how devastating a family can be and properly warn/prepare for time-sensitive situations and suggest preventive measures. Perhaps a sensitivity training should be arranged....sad, but true. PS. I only chose one star because my review wouldn't be accepted w/o it.

My relative has been at Woodbine in long term care for several years now. This facility was sub-standard then and still is sub-standard. The building is dilapidated, crumbling, in dire need of renovation. The owners have stated they are going to fix it up, so far a small amount of repair work has been done on exterior. The environment inside is horrible. The smell of urine and feces is enough to choke anyone. It is dark and dreary, no wonder the residents seem so unhappy.

We moved my mom to Woodbine Rehabilitation and Healthcare Center in September of last year. From the hospital that she was in, they suggested that we take her to a nursing home and out of three of the facilities we looked at, this is the one we picked. We chose them for the price. The facility is okay. It is not as nice as I would like, but I would say for the money it's a pretty good deal. The staff, on a scale of 1 to 10, I would give them about a 7. My mom needs round-the-clock care, but she is showing no improvement. She's not doing any better.

The amenities were horrible! I felt as though this was a place that people went to halfway between being alive and being dead. It had a putrid smell that roamed just about every hallway. My cousin's room (the patient) was below ground in the basement. The room itself seemed very old and cramped - sometimes he had to share, others he was luckier. The staff was okay, I found a few to be extraordinarily helpful, but most of them were lazy and uninterested, and seemed as though they just couldn't be bothered. There were moments when he needed breathing assistance, and it took nurses minutes before they arrived. In all honesty, I could not wait for him to get out of there and every visit was something that I just didn't look forward to.

Wonderful nursing care and rehab facility! Fantastic nursing and rehab staff, nice accomodations, and great meals.

When I was working as an EMT, a large part of my job included transporting elderly patients from nursing homes to hospitals or vice versa. One particularly memorable day during my first week on the job took me to two seemingly very different nursing homes. The first was Woodbine Rehabilitation & Healthcare in Alexandria. The staff was warm, friendly, and invested in the well-being of their patients. The atmosphere was cozy, almost reminiscent of a home or a large bed and breakfast. Everyone seemed happy and as healthy as could be expected, and my team and I left with a positive feeling, which was rare. Later in the day, we had to drop another patient off at a nursing home (stay with me here, I swear this is relevant to my review of Woodbine). We pulled up along the back entrance and took our patient inside. This facility, unlike the one I had visited earlier in the day, was absolutely squalid. The entire building reeked of human waste. In almost every room there was at least one patient moaning, crying, or calling out for help, and they all went ignored by the staff. I had difficulty transferring the care of my patient, because very few of the staff understood English. As we prepared to leave, I headed back towards the door where we entered, and one of my partners tapped me on the shoulder and said that we'd go out the front. We climbed a staircase and, to my great surprise, emerged into the lobby of the wonderful nursing home we'd visited that morning. I had been riding in the back of the ambulance, focusing on my patient, so I hadn't noticed that we were at the same place. I couldn't believe it. I was shocked that the kind and caring people in the nursing home were only one floor away from the disaffected employees of the rehab center. Take Woodbine - and your tour, and the brochures, and whatever the sales rep tells you - with a grain of salt. While it seems like a nice facility on the surface, this is not a good place for a patient who can't advocate for themselves.

I was admitted for rehab after knee replacement surgery. I was led to believe that I would be on a floor with similar patients, ones who were there short-term for PT; however, I was placed on a floor with patients suffering from severe dementia and worse. The staff was incompetent: one staff member wanted to give me additional pain medication, insisting that it was all right to do so even though I had taken a dose of my regular pain medication an hour earlier. I was lucky I was alert enough to check on medications when they were delivered because every time (!) the medication was incorrect! I woke up one afternoon to find a CNA rummaging through my bedside drawers, with a plastic bag. When I woke and asked what she was doing, she said she was "organizing" my drawers. Not surprising, during my stay I discovered items missing. As for rehab, I had to get myself down to the PT area on the basement floor by myself, was left alone to do my exercises with no supervision. The doctor on call was a joke...all he wanted to do was talk about his ski trip. When I said I was only supposed to be there a week, I was told that was for the staff and doctor to decide...since I was an insurance patient, I am sure they wanted to keep me longer. I stayed the required week for rehab and then got myself out of there as quickly as possible. My room shared a bathroom with the room next door. I was told that the woman in that room was bedridden so I'd have the bathroom to myself. On the first night, I got up in the middle of the night, went into the bathroom and discovered it was being used for storage! In addition, soiled sheets filled the tub and the stench was horrendous. I had to move wheelchairs, porta-potties, carts away from the toilet in order to use it, gagging the whole time. The next morning I complained to the nurse (the CNA on duty during the night just shrugged her shoulders), everyone all of a sudden was very concerned and apologetic. The CNA must have been called to task because she was hostile towards me from then on. The food didn't seem very nutritious: gravy and biscuits for breakfast, sandwiches for dinner (on my first night, I was given a peanut butter and jelly sandwich for dinner!). The main meal at lunch was usually some mystery meat covered in gravy. I eat a low-fat diet with lots of vegetables and fruit. When my husband called the dietician to discuss the meals I was receiving, the dietician came in to see me to ask what was the matter with the food. I think a health care facility should think twice about having a dietician who is grossly obese. Enough said. My stay at this facility was an absolute nightmare. I spoke with people after I left, who had also been patients at one time or another, and they had similar experiences. How this facility can get away with operating the way it does is criminal. PS I would give this facility NO stars, but in order to fill out this review, at least one star had to be highlighted.

In the first day that my mother was there most of her clothes went missing---she was left with one pair of pants, one set of underwear, and one top. It was November, and her coat was stolen. I would walk down halls and hear patients asking not to be hit. We got my mother out of there immediately.
