Food is horrible. My sister got two meatballs and potatoes. No veggies or fruit . For lunch she got the pull pork sandwich and that was all. I think this needs turn in for this. They get money to feed them healthy meals.

You will have to closely monitor your loved ones medications and be prepared to do all of the paperwork for discharge your self. My mom needs a wheelchair to get around they discharged without getting her one, this place gave her 17 doses of a med she was allergic to . The nursing care is Dangerous so if your looking for a place to dump someone this would be it. The entire experience was surreal . Extremely unprofessional I can’t even recommend this place for rats. The food was horrible we had to bring food from the outside. My mom did not receive a shower while there . The smell was extremely overwhelming I had to put an air freshener.. There was no welcome package and all though we are not having covid issues they never did a care plan meeting , and she was there for rehab if I had not made a fuss and speak with nurses she would have died in there. The room was dirty I had to bring home supplies to clean it . No one in that building knows what they are doing.

There's nothing I like about the Park Place Nursing & Rehabilitation Center at all, that's why I got my uncle out of there. It's not clean, it's nasty, and it smells. I was trying to get the paperwork done to move him over but nobody would help, we got the run-around from the staff, and it took us over two weeks. I ended up taking him to a doctor to get the paperwork I needed because we couldn't get the doctor there to do it or the staff to help us get what we needed. So we just had an overall horrible experience. I would never put any of my family there. To me, their staff needs to be checked up on, educated, or disciplined, and someone should pay attention if they're doing their job. His room was gross and disgusting, it wasn't getting cleaned and they weren't giving him baths. The food was cold and gross. The day I was there they chopped up a weenie and threw it in some mixed vegetables, it looked so disgusting. The facility is stinky, dingy, and not maintained, and the elevator was just nasty. They need some cleaning. In terms of value for the money, it's overpriced even though the cleanliness was lacking, the food was lacking, and the staff was not helpful.

BEWARE!!
This place is NOT the place you want your loved one to be. They are under staffed. Everyone is. I get that. They throw cold food from the refrigerator on the plate and never warm it up. Also, beware…they don’t use trays they just carry the plates with their hands. What? This is on the weekends. They have mean, outspoken nurses who are rude. I personally talked to the nurse who was very rude to me let alone my sister. I’m a very kind person but I do have my limits as to how far I let someone overpower me or my loved one. If anyone wants the names I have them. I also don’t mind the element of surprise to catch them in the act. I don’t even tell my sister I’m on my way so they aren’t faking it. I’m my sisters POA and advocate and the nurse would not even talk to me. I’ve had it with this place. I will report this horrible place to the state board or whomever I need to report to. To scream at a patient or their family that is trying to overcome massive trauma is not professional at all. My sister goes in because she had to be quarantined and needs physical therapy. So they send in a speech therapist. He wanted to argue with her that she didn’t need him. Finally I had to interject. She can talk she just can’t walk. (Car wreck). Their Covid quarantine is a joke. They go in one room to another and never gown up or glove up. My sister went 24 hours without meds. They rarely answer a call button. BEWARE!

The admissions director Alberto will not return calls. Left multiple messages and voice mails. No calls were ever returned

This is a beautiful facility where my friend was "incarcerated" for 3 weeks in April of 2019. BUT I witnessed staff becoming angry and verbally abusive toward my friend when she was too ill to raise her head from her pillow. Staff had arguments about who was to provide care for her loud enough for her to overhear. Wound care was pronounced "inadequate" by the ER Doctor who diagnosed a UTI. Even then it took several days for wound care to be revised after my friend furnished more expensive dressing materials. Food was often cold and tasteless. When she was too nauseated to eat for 3 days, I witnessed a simple request for Jello that went unmet for 3 hours because they were "out of Jello" so I went around the corner to a grocery and brought some. What nursing home does not have Jello - poor planning! Luckily she had private sitters around the clock to attend to most of her needs. Patient needs are not a priority here; I know of no amenities or enjoyable activities.

The staff at Park Place Nursing & Rehabilitation Center does not respond immediately when my dad presses his call button. When they do, they tend to his needs but not adequately. It has a sad environment because a lot of people are out in the hallways yelling to help them, but the staff said not to mind them because they don’t know what they need.

My Mother was here for 2 months for rehab. The food was terrible. She marked Manu selections every but often did not get what she requested. Most of staff was friendly, but occasionally would not help at all. She did not think the rehab she did was appropriate and did.not help her much.

Park Place Nursing and Rehabilitation Center is a medium sized nursing home in Tyler, Texas. I have visited this facility approximately fifteen times over the past three years and have known several of the residents. It is a nice looking nursing home. In my opinio, it does not have a homelike feel to it. It appears more like a motel which isn't such a bad thing if you plan on being their for a short while. I don't think a two-story nursing home is a good idea. If one has to be rushed to the hospital you must take a slow elevator. The several people that I know who lived there gave mixed reviews of the care received. some were pleased and others were not. however, this is typical of most nursing homes. I know three of the staff members who work there and all three are highly competant and friendly. Overall, I would rate Park Place an average nursing home. I would probably trust that my loved would one receive quality care at this facility.
