My mom is at Pleasant Valley Healthcare & Rehabilitation Center, and it's been very good. They're very professional, the place is clean, and the people are very caring. She's in skilled nursing and has a clean, big room. They come in and check on her. It's just been a good experience. The food has been good, and she's been eating in her room. The staff is very caring, passionate, and very professional and takes care of her very well.

My grandmother is there now, and we are doing everything we can to get her to a new facility as fast as possible. It’s basically a bed for her to sleep in, and nothing more. Her nurse call light didn’t work, so she’d go who knows how long needing a nurse, and nobody would show up. She wasn’t being worked with properly in order to heal- even the very basic need of building strength and sitting up for meal times. We shared our concerns very early on with nurses and the director, with no changes made. The last straw was this morning when an employee was rough with her, didn’t change her properly or clean her, threw her gown on top of her for her to change herself, and denied her a sponge bath when she asked for one. I’m actually worried about other elderly patients who aren’t able to advocate for themselves, getting the care they need here. Extremely saddened and disappointed with our experience.

My mom is at Pleasant Valley Healthcare & Rehabilitation Center. She already moved in. She's doing fine, she's alright. They let me know everything going on. We chose this facility because of a hospital recommendation and then they take a Medicaid. She seems to be alright over there. I've been over there, everything was nice, but she's on a COVID lockdown right now, cause she got COVID.

My mom moved into Pleasant Valley. Due to COVID, we can't go in, so it makes it hard. We go to the window to see her. A maintenance man put up a TV in her room. It makes things pretty difficult because she's got dementia real bad. So far, everyone's been friendly and professional, so I can't complain. They treat us good. I had to have a COVID test to get in, but they ran out, so I can't get over there till I get the test. I understand everything is pretty tough right now, but other than that we're pretty satisfied.
It's nice there. They're off of Pleasant Valley in Garland. It's close to us, that's why we took it, so we can try to take care of mom. She needs a skilled nursing facility, the doctors ordered that. If you're someone looking for a skilled nursing facility, Pleasant Valley is a nice one. For us, the location is good. We just live about 3-1/2 miles away, and I have about four kids that live in this part of Dallas County. They can go and see grandma every week. We just keep tabs on her. For us, it's been good. If I have questions, they get right back with me. If I need to talk to a doctor or nurse on call, or the staff's head nurse, they seem to get right back with me, so that's a good thing.

I brought my father to this facility for rehab and he really loved it. He was able to reach his therapy goals and was released to go home much sooner than anticipated. The nursing staff was so kind and caring. The doctor always explained the plan of care and staff always called to let me know about his progress. This place is awesome!

I WENT TO VISIT THIS PLACE TWICE TO SEE IF I WANTED TO PUT MY LOVED ONE THERE... NOT AT ALL... THEY DIDNT HAVE ENOUGH STAFF WHEN YOU ASKED QUESTIONS THEY WERE RUDE AND TO TOP IT ALL OFF THE STAFF THAT WAS THERE ALL THEY DID WAS GOSSIP AND TALK ABOUT WHAT WENT ON AMONGST ONE EMPLOYEE AND A RESIDENT AND I KNOW YOU ARE NOT SUPPOSED TO DISCUSS THINGS LIKE THAT... THAN WHAT MADE ME REALLY JUST NOT WANT TO PLACE MY LOVED ONE WAS I SAW AN EMPLOYEE EATING OFF ONE OF THE RESIDENTS LUNCH TRAY!!! I KNOW THEY ARE NOT SUPPOSED TO DO THAG NO MATTER IF THE PATIENT WANTED IT OR NOT.... THAN WHEN YOU COMPLAIN NOTHING GETS DONE ABOUT IT SO IT WAS THE WORSE NURSING HOME I HAVE EVER VISITED... THEY HAD ONE EMPLOYEE I CAN SAY THAT WENT ABOVE AND BEYOND FOR HER PATIENTS I CANT REMEMBER HER NAME BUT IT STARTED WITH AN F BUT AMYWAY I WOULDNT RECOMMEND NO ONE TO TAKE THEIR LOVED ONE THEIR.. I WOULD NOT WANT TO BE PAYING MONEY FOR MY LOVED ONE TO STAY SOMEWHERE AND THEY ARE NOT BEING TAKEN CARE OF PROPERLY AND THEIR FOOD IS BEING EATEN

My loved one is at Pleasant Valley. She had been there before and was happy with it the first time, so we went back. They're average. The rooms are clean, but they've been there a long time. The food is not really good. They feed them the same stuff every day. The worst thing they did was they used her insurance up and milked her insurance company for her rehab. She went to rehab probably once or twice per week. They had her go in every day and next thing they know, they used up the insurance for the whole year of $6,000, so she can no longer have rehab. It's a pretty bad deal there. They need to hire more staff. They're always shorthanded.
So disappointed!

Don't bring your loved ones here. The food is uneatable. It is so sad that these poor people can do nothing about it!! The management and owners should be held responsible for this outrage. Complaining has not produced any results.

My father-in-law was here for just a few days for physical therapy. They never told him what meds they were giving him, the staff never came in to check on him even though he had a braclet saying he was a fall risk. They were suppose to have a meeting to discuss the plan of action for his therapy, my in-laws were the only ones that showed up. My father in laws feet started swelling and they done Nothing, so he was brought home. He gets more therapy at home than there. I give 0 stars!!!

I would have liked more communication from staff when they noticed changes in my mother's condition. The only time staff ever made an effort to contact us is when she was being rushed to the hospital for emergency treatment. I think what makes me most upset is that no one ever told us what medicines she was being given while she was a patient there.
I feel that there were not enough staff to give my mother the care she needed. When I was there to see her, staff very rarely came to check on her or turn her over. She was expected to dress herself for therapy, even though she is not even able to stand without help.
While she was there, she was sent to the hospital twice. During her second stay in the hospital, staff took her wheelchair from her room. The wheelchair had her name on it. When it was brought back, one of the footrests was missing. Later, it was replaced with a footrest that obviously wasn't hers.
The doctor who admits that facility's patients to the hospital also was very unhelpful in that he never communicated with us or explained to us what was going on with her. He never even told us that he was planning to discharge her. We had to find out on the same day from a palliative-care professional who just happened to stop by. Any credit for the care my mother received in the hospital goes to her nephrologist and the nursing team. The nephrologist is the one who kept us informed about what was going on with my mother and ordered tests for her.
My mother was discharged from the hospital Saturday. We went to pick up her items from Pleasant Valley Sunday, and left the inventory form with a nurse. Mother is now in hospice care here at home. As of today, Pleasant Valley has made no effort to contact us with any medical information or questions about what they could have done to help.
