We had a HORRIBLE EXPERIENCE at this facility. My mother-in-law spent 29 days at the Healthcare and Rehab Center. During this time she spent 4 nights in the hospital for a severe UTI. The facility/staff were negligent on recognizing her symptoms of disorientation and confusion. Upon her return it was 2 days before the bathed her, got her dressed and out of the hospital gown, and only at my persistence. This could be because of a high turnover rate and overcrowding of residents. The nurses and doctor are terrible communicators. The nurses were not helpful and one has a terrible bed side manner. We had several ignored emails and phone calls. Still waiting on results from urinalysis ordered on 4/18, it’s now 11 days later. The doctor is a complete disappointment. Our family has only spoken to him ONCE during her whole stay at Legacy, again 29 days. We have requested him to call us and answer questions, he never reached out to anyone. We never even met with him, the only contact was an initial phone call for a medical history. The assigned case worker quit during this time, so there was extra added confusion when we tried to move my mother-in- law to a new facility. Again communication at this facility is horrible! The best part of this facility are the aides, the two who I have befriended in my many visits are friendly and caring, but I do not recommend this facility!

I went to The Legacy At Willow Bend before the pandemic hit. The facilities are beautiful. The staff there was very warm and welcoming when I went there, and then the coordinator, Lisa, was very good with follow-up. I saw some of the units, the units were very nice and just overall the facility was well maintained and it seemed like it was very active and there were a lot of activities that were going on there. They had like a fitness center, they had music. They had religious services as well for the Jewish and Christian faith. So they had different religious organizations. It looked like they had a lot of stuff going on on the calendar, book clubs, game nights, and a lot of social activities. My mom is in the skilled nursing section for rehab, which is separate from assisted living.

This facility has an A+ rehabilitation facility and qualified individuals that aid your loved one to increase strength and mobility. Saying that, everything else goes down hill from there. Typically first shift aids and nurses are available for the patient and respond when support button is pressed for in-room support needs however you can expect to wait for this help to show up. Personally witnessed multiple aids and nurses at nurse station standing around with buzzers going off from patient rooms just chatting vs. responding. The second and third shift support declines even more. Longer delays in response, get in and out as quick as possible and not take time to listed to what patient is asking for. Plus, leaving a family member in soiled clothing longer that needed resulting in a rash that now has to be delta with. I recommend this facitily for rehabilitation but would not leave your loved one in for longer stay with the type of support they are going to receive. Extremely dissatisfied with that level of service. Note: There were a few aids that were outstanding and did their job extremely well the problem is this was not the norm.

I was referred by Baylor Scott and White hospital for rehab and left on my own after three days. I had a serious wound on foot and this place had NO wound care. They used an ace bandage for lack of medical gauze? They did more harm than good.

Lolanda, one of the floor nurses is amazing, on top of things and really cares. Sammie, one of the CNA’S is amazing and caring. They have great volunteers who donate their time with activities. I felt very blessed at first,but my mom requires extra care with a urostomy and memory issues. My mom fell at least 4 times in the first week and has a major contusion on her head and complained of her ribs hurting. I went with her to UTSW Urologist doctor yesterday and the doctor told me my mom needed a scan and he was shocked that Legacy Willowbend did not order or request one. He said my mom could have bleed out. ( internal bleeding) He ordered a scan today and they found an embolism in her lung. MARCIE AND DARCIE, both in administrative roles could not wait to get my mom out the door. So, if you are an easy patient with minimal medical issues, your loved one will be fine. If not, look out because they put up a good facade. The best to all other concerned family members and clients.

My mother had multiple organ failure after a hip replacement. After touring multiple places I chose The Legacy Willow Bend Health Care Center. Our care there was wonderful. Her physician or his nurse practitioner saw her almost everyday and even called me to talk about her care. The nurses were great and the CNA's who were always busy, took time to come help her with all of her needs and even took time to make her smile or laugh. The therapists were great and really pushed her and she was able to walk before she left. I loved the Food Director. He came by and then made her food that she liked so she could gain back the weight she lost at the hospital. I ate there at least once a day, by choice. The Social Worker made sure that we had everything that we needed when we went home. We felt like part of a family when we were there. We got to know all of the staff including the Administrator who came by to check on us and to answer questions. Mom has to have her other hip done and we will definitely be going back. A big thanks to all the caring people who took care of my mom.

I strongly discourage anyone following a strict medication regimen from using this facility. I was assured my dad would be given his Parkinson's medications at specific times as directed by his neurologist, however, that hasn't been the case. I feel that medication mismanagement has quite possibly impeded my father's recovery. I'm moving my dad to a different facility in the hope that he's given medication at precise intervals as required for Parkinson's patients. On a positive note, I couldn't have asked for better therapists or nurses but have to give a low rating due to medication management.

Not a good facility for significantly compromised patients. More of a rehab "lite" where patient needs to be able to communicate. As a medical professional, I placed my father here for skilled rehab after a devastating stroke requiring total care, Discharge goal was group home. Was reassured they could manage his care and help try to maintain what little function he had (drinking from a cup and maybe learn to feed self). We had warned them of significant physical and behavioral challenges. Unfortunately, they were unable to live up to the challenge. He was agitated if wet; they were unable to keep dry/change frequently enough so agitation affected the rehab. No effort made to learn the agitation triggers. Nursing staffing was indifferent, and I overheard comments at lunchtime several times about their unhappiness/job seeking. The therapy staff may have been fine but did not see the 2 sides work together to create success. Speech did improve but that was stroke healing. The "medicare" nurse was cruel and unprofessional in her presentation. Unfortunately, I saw my father's limited physical abilities decline in this facility.

My mom was at the The Legacy of Willow Bend Health Care Center and whenever Mom had a need it was taken care of immediately with a positive and assuring attitude. This began the first night we arrived and continued through the morning we left. The nurses were so helpful in monitoring Mom's medications and giving them in a timely manner to stay ahead of the pain. Aides were especially caring. Physical therapists were great. The Department Head got just the right persons (warm and fuzzy) that were able to encourage my 97 year old mom to keep moving in spite of the pain. Mom is almost back to the health and mobility she had before her fall 2 months ago. Thank you. Thank you! Also thanks to the admitting person for the ease and speed of admitting Mom. The staff made me copies of menus and gave me information that made Mom's stay run smoothly. Legacy is the BEST!! I especially liked the professional and caring atmosphere. I can't think of anything area needing improvement.

My family had an awful experience here. They are horribly under staffed. They could not handle my moms level of care. Our parent was sitting in urine for hours. I had to actually transfer my parent more than ten times because no one was there to help. They would not help our parent use the toilet and only allowed them to go in a diaper. Therapy was a complete waste of time. This place is horrible.

My husband was in The Legacy for three weeks for rehab. It was nicely furnished, and the food was very good. He appreciated that very much. They had separate sections for rehab. The staff was very patient, accommodating, and helpful.

Awesome place one of the best rehab, my uncle moved to this rehab after had a bad experice at another rehab. my Aunt and uncle love this place.. fast response from the nurses and attendents. best care... can not ask for more...thank you

My father is in The Legacy. It has four levels of care: memory care, independent living, assisted living, and a nursing home. The food is okay. The staff is attentive and hard-working.

I am the daughter of a 91 year old independent father. Dad moved to LWB Independent Living 2 1/2 years ago. He came kicking and screaming as he did not want to leave his home in Houston. He now aplologizes and thanks me for giving him the opportunity to live his life amongst wonderful friends and staff. As dad says, he is part of a truly caring "community". In January 2015, dad had an episode of congestive heart failure. He was hospitalized for 5 days and then was discharged to LWB rehab., where he lived for 2 months, receiving excellent care by the nursing staff and therapists under the direction of their physician. Dad was discharged back to living independently after 2 months in rehab. He then received The Legacy at Home, Home Health Care in his apartment. Dad's health is back to where he was prior to his congestive heart failure episode. WE can't thank Legacy enough for their excellence in caring! Knowing that dad lives in a CCRC has given both of us peace of mind and dad is truly living life to the fullest. Thank you to The Legacy Senior communities caring staff! With gratitude, Elise Power

My mother just recently left this facility. I cannot say enough good things about the staff and our experience. The staff were very helpful and the therapy was excellent. I do believe that she left doing better than she was before she went in to the hospital who sent her there. We feel very blessed to find the facility. The Social Worker was wonderful and help set up things for her when she went home. She has even chosen to go back to them for her out patient therapy. If you are looking for a great place to go through rehabilitation after therapy, check them out.

We had a horrible experience at that facility. Such a pretty place but horrible care. I have never witnessed such cold and mean spirited staff. We were afraid to leave my mom alone here. Please look for someplace that has kind, engaged staff. So sad that it looks good which lures people in.

Don't judge it by the beauty of the building. My mother lives there now. So many things have happened there I won't have enough space to list them all. The most important thing is that the medication person made a mistake and no staff member knew about it until a resident questioned it. Then we got a call from the nurse with the news that our mother had been affected by it. Really, no checks in place for medications? Mom is still there so and we feel she is not able to move at this time, so I won't say more for fear of repercussion against her. By the way, they allow some caregivers to work 16 hour shifts back to back. All I can say is don't put a loved one in that place.

The facility is very clean. I think for the needs of our loved one it is very nice . Some of the staff always have a warm greeting where they need to have others learn from Peggy. Net, Tara . They are super CNA 'S .

I had a step-father in this facility Spring/Summer 2010. Since that date, I have had much difficulty getting necessary forms signed for the VA. First time took several months and the second time took 5 months. Because it took so long the second time, my step-father has been denied VA benefits. I asked Legacy if they would give me a letter stating the length of time it took to get my form signed was out of my control so I can appeal the VA denial and Legacy refuses to help me. I would avoid this place if you are going to need signed documents.

While in the area, we happened upon The Legacy at Willow Bend. We had been visiting some other facilities around the region as options for my brother's friend. We were glad and impressed that we didn't need an appointment to take a tour of the facility. The staff was welcoming and seemed to really like their jobs. That's really important to the family. The outside grounds were nicely kept, with plenty of seating areas. Residents are encouraged to walk around regularly as they desire and supervision is always available. The indoors were impressive, with a nice library area and a large room for other activities, like board games and computers. There are other areas where residents may read, sit with friends or just relax. The film collection looked interesting - and they have their own movie theater - sometimes venturing into town to see foreign and independent films, experience theater, dance and other cultural events. We liked the exercise room where they had yoga mats and machines and a pool. They also have scheduled classes for those who are interested and personal trainers. The private living spaces were really nice - fresh, clean, airy and nicely decorated. The food in the dining room was extremely well prepared - the smells were terrific and it was clear that the residents were really enjoying their meals. This is a wonderful community that is designed in an upscale fashion, where every resident is very well cared for.

My mother went there for rehabilitaion after a mild stroke. They started her on a regimen of strong psychotropic drugs which caused severe behavior issues and physical side effects. She suffered two falls there because the drugs made her dizzy and she was left alone on the toilet. The director of nursing and the staff were more concerned with their own social activities than caring for the patients. Diabetic dietary instructions were ignored and the food was awful. Toilet assistance was sporadic at best. I once found a dirty adult diaper in the drawer with my mother's clothes. She developed a huge bedsore from being left in wet diapers all day. The facility is modern and attractive, but the care is horrible.

The Legacy is a beautiful, gorgeous!, new community in Plano just minutes from the Dallas North Tollway. This elegant, gated community sits on 28 acres. It is a CCRC with independent, assisted living, memory care and health care/rehab. Assisted living apartments can be leased on a monthly basis. The assisted living area has 40 apartments on 2 floors, both 1 and 2 bedroom units. Each has a small kitchenette and large closets. Living here is similar to living in a boutique hotel; a large staff tries to meet the individual needs of each resident. There is a movie theater, pool and gym, and exercise class 7 days a week. An LVN is on duty 24/7. Activities here are age-appropriate and 'sophisticated'. The food is spectacular. There is a large Jewish population, and Jewish, as well as Christian, tradition and holidays are observed. Each resident has a pendant that has an GPC tracking device; they can activate this for assistance wherever they are in the community. There is a dog park and grounds for walking. Every resident has a personal trainer, if desired, included in the fee.
