Do not evaluate this facility based on their website or reviews of Independent and Assisted Living floors. I see they say nothing about the nursing home floor, and no photos of that floor or people would see how bad it is. The independent and assisted living floors are much better than the nursing home floor, #3. High staff turnover since new management took over, call bells not answered for up to 30 minutes, staff seems bored, no one answers the phone and the dining room is a disgrace, with broken chairs, staff shouting, a tv blasting and more. Wish I could move our relative somewhere else.

Came to The Watermark for post-surgery rehab because I thought a residential rehab facility would be best for my short term rehab. I arrived at 12:30PM from Pennsylvania Hospital via emt escort. There was no indicatin that they wee expecting me or what my room number was and only the emt people found out the room I was assigned and dropped me off. I received no greeting, no visit from any admission person or any sort of visit from any healthcare professional for seven hours. I was left in the bed and the only contact I had was from a person who set up the TV and installed a new house phone. I was refused a walker and a urine bottle and persistently throughout these seven hours was told I had to be evaluated first before anything could be assigned t me. No one would bring me a walker or give me a urine bottle or even help me to the toilet. When a nursing assistant got involved after I threatened to call 911 to get me back to the hospital , she said the evaluation would not happen until Saturday at best. Several staff people who I flagged down by yelling for them as they passed my door and the "ambassador" who said he was there to ensure I got all I needed all refused to get me a walker or a urine bottle. They said they would check and either told me I could not leave the bed or just never returned to my room. I threatened to call 911 and a nursing assistant told me not to and that my evaluation would not take place until the following day at best. I had to use the rolling meal tray to wheel myself to the bathroom a good twenty feet away. I almost fell. Finally at 7:45PM a nurse came to take my vitals and she was shocked that I had had no one visit me before her shift started. She procured a walker and a urine bottle for me and took my discharge papers with the medication list, promising to return it to me. She never returned. She did manage to set up some of my meds such a heparin and senna but nothing else. Luckily I had my own gabapentin and lorazepam. I was distraught and felt abandoned. I slept but woke at 5:30 am waiting for someone to come in and at least acknowledge that I was there. A woman who told me she was my "ambassador" for the day and an agency worker to ensure I got my breakfast and all I needed. She never came back. I waited until 9:45AM and as no breakfast was delivered, I used my walker to go to the door and tried to get kitchen staff to get me breakfast. I might as well have been speaking French, The day nurse did respond and procured some scrambled eggs and a tiny muffin which she microwaved and a banana and another kitchen worker was kind enough to get me a coffee from another floor. Still no word about the evaluation or any sort of indication that anyone knew anything about my care, other than my meds, I did not feel that I even existed. I realized that it was less than 24 hours and just wanted out of there as I felt unsafe and abandoned and did not want to be there at all. A friend came to rescue me and take me to his house amid some drama and a sudden outbreak of attention from people who I had never met who all apologized and claimed they did not know that I was so unattended to. I left and will never go back there. I am safe now and will undergo in-home nursing and pt care at my friends' home. No one should eve go there.

I have spoken to an administrator. Called liaison for Jefferson. Poor recommendation to vulnerable folks. Needs to be seen by doctor. Will begin looking to transfer to a more capable place to handle our needs.

I am 93, and I am normally not a complainer, but I was pushed to the edge at Watermark! I was a patient at Watermark for over a month. I was brought to the rehab/nursing facility at Watermark by ambulance from Magee Rehabilitation Hospital on Saturday, April 25, 2015. I was immobile due to a hip fracture, and I was also suffering from a bedsore. I was put into a bed by the ambulance attendants and just left there, completely unattended for hours. When someone finally came in, I said that I had a bed sore and needed to be turned every two hours, but I was told that no one there could touch me until I was evaluated. I was not evaluated until the next day, and was not taken care of at all for any of my medical needs! Despite the fact that I had severe edema and my legs were terribly swollen (approximately 24 lb. of excess water), and the fact that I am diabetic, it was difficult to get the appropriate diet. Even though this is supposed to be a skilled nursing facility, the dietary staff seems unable to grasp the concept that they are dealing with patients who have different medical dietary requirements. There was also a problem with the dietary staff either not filling out menus or not getting the information at all on menu requests. A major problem at Watermark is that they don't have enough employees to take care of patients. Even though I was not supposed to get out of bed myself, after waiting over 20 minutes for someone to help me get to the bathroom, I risked falling and hobbled there myself. I had such a hard time using the toilet that I ended up messing up the bathroom. Another time, when I had difficulty using a urinal in bed, an aide refused to help me telling me that I had to learn to do it myself. When I said I would report her, she said, "Go ahead, report me!" For three days, I was not given my diabetic medication. I was told that the pharmacy hadn't delivered it yet. I was given the same reason when I did not get my glaucoma eye drop. Obviously, the nursing staff doesn't keep up with pharmacy prescriptions in a timely way. When my diabetic medication was there, the nursing staff did not administer it at the proper time in relation to meals, something that's very important for a diabetic. At one point, when I was about to get insulin, I had to have a quick bathroom trip, but the nurse just left. I had to track her down to have her give me my insulin. I was still non-weightbearing when I had an appointment with a doctor outside of Watermark. I was in a wheelchair, and Watermark was supposed to send me to the doctor in a van with an aide. They said they couldn't spare anyone and wanted to send me alone in the van. Fortunately, they were able to send an aide with me, maybe because they would have been legally responsible if I had been injured on the trip to the doctor. I had so many problems with the care I was receiving that I requested a meeting with the head of Watermark, the head nurse, the nutritionist, and the social worker. Only the head nurse, the nutritionist, and the social worker came to the meeting, and after 20 minutes, the head nurse said they had to leave and I should put my complaints in writing. When I did thank the head nurse for sending me to the doctor with an aide, she knew nothing about it. She pointed her finger at me and said, "You didn't tell me that an aide went with you to the doctor!" I answered her, "Am I your patient or your employee?" She backed off. On another occasion, I was sent to a doctor's appointment, without a aide. Needless to say, the problems and concerns I raised at the meeting were ignored. When I was supposed to be returned to Magee Rehabilition Hospital after being approved by my doctor for weight bearing on May 13th, the Watermark social worker acted as though he had never requested a hospital transfer before and endlessly delayed my transfer back to Magee--a scheduled transfer that never took place. Since the Watermark facility is only approved for 30 days, this delay cost me a lot of money, which I would not have had to pay if I had been properly transferred to Magee. A Watermark administrator actually came to my room with a bill when I was two days over the 30-day limit! That wasn't the end, though, and another bill arrived when I got home. The only good thing about Watermark was the physical therapy department. When I entered Watermark initially as a rehab patient, I was actually interested in possibly moving to a Watermark apartment when I was back on my feet. My experience there was so horrible that I would never recommend Watermark to anyone. Even the people who are healthy and enjoy the independent lifestyle at Watermark must fear the possibility of needing the nursing care on the second floor.

My mother was at The Watermark a couple of years ago. It was good. They are assisted living, but they also have a rehab. Our rehab experience with them was excellent. They had incredible physical therapists. They really worked my mother well. They prepared her to go home. My only complaint about The Watermark was the food. My mother was very unhappy with the food. She didn't like it. She said the food was very bland.
