My grandma went to the Manchester Commons rehab facility after being discharged from the hospital for pneumonia. She was at Manchester Commons for three days. Two days later she died. She was fine when she went to Manchester.
In the three days she was at Manchester Commons there were two incidents. She fell down trying to get to the bathroom because they would not give her her walker (until she was assessed by the PT who wasn’t in until Monday). On the third morning they rushed her to the hospital because her oxygen levels dropped overnight (did anyone check on her?)
