This place never sends the entire correct meal and they dont care, lost 14 lbs since being here and again they dont care. Place has been in trouble with state before over the last 3 years and many people in the unit i was in contacted the state over many issues. They need new leadership badly, leaders do nothing they say they are going to do.

My father was a patient in the Grove unit for two months in 2019. The Grove Unit, if I understand correctly, is mainly for patients receiving rehab and then going back to wherever they came from. It was recently remodeled, but rooms are still pretty basic (mostly 2/room). This was the only facility with an open bed for someone with his needs (recovering from traumatic brain injury and with a trach & feeding tube). It’s not an attractive place, for sure, but we found staff to be caring and kind with few exceptions. PT, OT and Speech therapy were amazing! It was there goal to prepare him for entry into an acute rehab program, and they did just that! They set the bar high from Day 1 and helped him make great strides. (My Mom actually requested going there late March to get rehab after injuries sustained in a fall, but they had just closed to admissions due to Covid-19.) The aides he had were wonderful. They were caring and compassionate, and treated him with respect. The nursing team was severely stretched by their workload, so even though they cared, they weren’t able to give any extra time and attention. Three things to consider: 1) Biggest issue we had was that the facility told our hospital social worker that Dad would have 24/7 respiratory care, but that’s only true if the patient is on the respiratory floor in the other building — which my Dad wasn’t. Therefore, getting care for my Dad’s trach was a constant battle the entire two months. Resp staff said the nurses should do it, and nurses said the resp staff should do it. The physician who is ultimately responsible for respiratory care at Brighton saw my Dad once in 2 months, and was very slow to address or approve any concerns or recommendations made by the resp. therapists. Therefore, his respiratory care was dismal. 2) Be there as often as you possibly can to advocate for your loved one. The squeaky (but kind) wheel gets the grease. My Dad wasn’t able to ask for help much of his time there, so we were there each day. 3) We were given absolutely no information about the facility when my Dad arrived. We had to ask questions and learn the ropes on our own (ie. - when are meals, when are baths, where are public bathrooms, what are visiting hours, who is who and who does what, etc., etc.).

I did complain to management on the phone at the time about the lack of hand washing I witnessed. To be very clear: I was in this facility several times over six weeks of his rehab and NEVER ONCE SAW ANY WORKER WASH OR SANITIZE THEIR HANDS BETWEEN PATIENTS OR AT ALL......NOT ONCE!!!! Rooms are pretty dismal..it is a very tired facility. Other than the TV basically no amenities and few if any activities' I would never recommend this as a rehab facility

Parent was a patient for 2 weeks before we removed. Under no circumstances would we recommend facility for a respiratory problem. Everything about our experience was awful. Reported to DHS but investigation was superficial despite our physical evidence. Parent was on the Grove Unit.

Check the Medicare website: they consistently FAIL annual inspections. But, for some unknown reason, Beaver County offices (aging, protective services, etc) protect the reputation of this place. I can only assume it is because it is so big that it is the only place in the county that consistently has an open Medicaid bed. The "lockdown" wing (East wing) is truly awful...overcrowded,and understaffed, and the medical director misdiagnosed my loved one several times, making most patients on the floor suffer from scabies for three months! Fresh vegetables are non-existent, with (cooked) frozen carrots with just about every meal. Dirty floors and gnat infestations in rooms that squeeze four beds in with just a foot or two between them. There is good reason they are at the bottom of the list as one of the 16 worst facilities in Pennsylvania.

This place is a real filthy place. I would not recommend leaving your dog there. I went to visit a patient and it was just awful. The matress was ripped and the 3 prices of thin foam were exposed this rip was about 10 inches long. The faucet was turquoise color and white from corrosion. The air conditioning has been blowing hot air. After 5 complaints nothing. The curtain that hangs around the bed was filthy. The elevator had trash in it. Went outside to get fresh air thought I was Gona be sick and it was just as filthy out there. Cigarette buds everywhere. Discustingly dirty place. Wish I could share the pics I took.

They treat you like a little kid if you tAlk back to the aides who get smart with you first, then they write you up. Third floor on the Grove is the worst would not recommend no one to stay there. Few aides are good the other are not worth a dam only there for the money. Management is bad also, some nursing directors are bad also don't go there spend 10;000a month some place else. By Kim T

I don't know what some people ate at this facility but it was terrible! Most of the staff were wonderful and PT great and caring. Management you can have them. Never came to me when I wanted questions answered. This went on for quite a while and were unpleasant when they did come. Some bathrooms were dirty, looked like toilets wern't cleaned in weeks. Shower room with wet washcloths on the floor still from the day before. Used toilet paper on the floor. If you care for someone don't send them here.

Not nearly enough staff to handle what they claim they can do. Request for the simplest things, including information, will be completely ignored. It eas noticed that those requiring bedpans will not be tended to and left to swim in their own bodily functions. Would never recommend for a loved one or worst enemy. If you prefer eatable food, you will surely starve here!! RUN RUN AWAY

My brother, who has cerebral palsy, and cannot walk or rollover, or move for that matter, was there for about a year and half. The rooms he was in on the 3rd floor were overcrowded, he was in a 4 man room. Half the time you couldn't find someone when you needed them and the OVERNIGHT crew was the worst. Got a phone call on Valentines day about 2am. stating he "FELL" out of bed. He ended up with 8 staples in the top of his head. I know he had to have been flipped out of bed, he doesn't move. They covered up saying no one was in the room at the time, but we know better. If you care about your loved ones, do not send them there.

Understaffed, the only good thing about this place is PT and OT. They're very attending and professional. Very noisy place, exspecally on 11-7. The whole experience was awful!! The Supervisor Sara seemed unconcerned!!

The rooms are cramped, some have 4 to a room. Staff was hard to find and several times,. My Aunt was there, and her treatment was poor, several times we walked in to find her in a urine soaked depends, and the urine was dripping from the bed to the floor. I am not impressed with this facility what-so-ever.

I watched nurses disconnect wheelchair alarms, aids intentionally not respond to a patient known as a "button pusher". Associates are severely overworked. They simply cannot get to all of the needs of all f the patients in a timely manner.

The Nurses, Nurse Aides, and the Therapy team are all very friendly, compassionate, and helpful. They are always there to help in any way that they can be of assistance. They all truly want you to get better than ever. I have been to other homes before, and this has been far and away the best place. I got a feeling of being in a hotel. It was glamourous, and had a luxury feel to it. The food is tasty there as well. Well done Brighton!

Therapy evaluates each resident upon addmision, and then works with them according to what their limits are. I have built up an incredible amount of strength from the wonderful therapy department. The nursing care that is provided is excellent. They distribute medicine, meals (which tastes great), and snacks etc. They are always there to help with anything that comes their way. I recommend this facility to anyone that needs some care.

The Physical Therapy really cares about each and every resident that they treat. They really want you to do well and get better. The staff that I worked with was Stephanie, Kelly, and Matt. They were fantastic. The overall care there was amazaing.

My mom has spent some time in a few different nursing homes over the years. Visiting her over the years, I got to see how everything was run, and how everything gets taken care of in a nursing facility. Based off of what I had seen, it would be a waste of time to even compare those places to Brighton Rehab and Wellness. The care that I have recieved at Brighton Rehab and Wellness has always been there when you need it, the nurses are great there, and everything about my stay has been excellent. I live about 30 minutes away from Brighton Rehab and Wellness, and I can even see a different nursing home from inside my house. I have absolutely no regrets that I chose Brighton Rehab and Wellness for my care.

The care at Brighton Rehab and Wellness Center has been excellent every where I turn. The Nurses, Nurse Aides, and Therapy have all been wonderful. The food is good there. With all of the renovations that they have done there, the grounds are looking spectacular. There is a beautiful fountain and a very pretty backdrop of all the colorful trees right outside my window. The exuberant therapy staff does a wonderful job in the brand new gym.

When I am feeling weak and not able to complete some tasks on my own, the nursing staff has always been there to help me with things as simple as opening up my milk for me. The care I have recieved here is top notch. The therapy department is exceptional as well. Their gorgeous brand new gym is equiped with brand new state of the art equipment enabling the therapy department to work with each and every resident to get the quality care they deserve.

The care given to me here has been very kind and proffesional. The staff is very attentative to detail, making sure each and every resident is getting the proper care to make them comfortable and safe.

The care is great the staff are amazing and they have a concierge named Steve who will help you with anything you need during your stay

I can not say enough about the individual care that I have recieved at Brighton Rehab and Wellness. From the Nurses, to the Nurse Aides, to the Physical Therapists, my treatment was spot on. With the type of injury that I had, affecting my spinal cord and my legs, just geting out of bed was my main goal. Now that I have had the oppurtunity to recover at Brighton Rehab and Wellness, I can now boast that I am capable of walking on a treadmill for FOUR MILES straight, a mere two months after my surgery. I honestly do not know where I would be without Brighton Rehab and Wellness.

Following my surgery to free my spine of three vertebrae which had been crushing it for the past ten months, I had to find a facility which provided nursing care as well as rehab care in the Brighton Township area. Choosing Brighton Rehab and Wellness turned out to be a true blessing. The attention to detail that was given by not only the nursing staff, but also the therpy staff was personal, and caring. I would like to express my appreciation especially to Aimee and Christin, two therapists, who took personal interest in acheiving my goals of standing and walking. I will personally recomend this facility to others needing care.

My mother is in Friendship Ridge. They seem to have caring and very attentive staff. When she moves, the buzzer goes off. If she gets off of the pad, the buzzer goes off. They're supposed to come see if she fell out of bed, and they respond very quickly to alarms. The building is being renovated, and the floor was very nice. She says the food is good. They have ice cream socials, daily crafts, and movie nights.

Dad was at Friendship Ridge for rehab, and the care he received was OK. I felt they should have made him stay longer at the rehab because we brought him back to the emergency room two days after he was sent home. I wish there had been more therapy involved. The food was good. The building was new, so it was very clean and well maintained. The rooms were pretty nice.

I remember my Mom taking us to visit my grandmother here. She seemed so happy, and now that I'm older I visit without my mother there and my grandmother is still there and happy to be. The clientelle there always greets me with a smile and recognize me enough to know exactly who I'm there to see. I do miss going to my grandparents' house for holidays, but everytime I walk into Friendship Ridge it's like coming home for the holidays.

My mom was in Friendship Ridge. It was nice, but it was old-looking. It needed updating. The staff was very helpful and friendly. The food was just adequate, and I did not care for it. My mom was in a shared room with 3 other people. It was too small for four beds, and there was no room for visitors. I asked for her to get a private room, but they did not give her that because they knew she was dying with stage 4 cancer. I would recommend this facility to others. The staff was very nice.

My mother was a resident of this nightmare for only a few months. Since i lived in a different state and i worked i would fly in every weekend. The staff knew my routine. The room was so hot and the whole floor did smell like urine. There were 4 to a room. My mother was afraid. Then i decided to make a surprise visit during the middle of tbe week. When i arrived in the afternoon my mother wasn't dressed and her diaper w a s soaked. Fortunately i took her out the next weekend and moved her with me. I am so glad i did. The time we spent together before she passed away is priceless to me. I would have never forgiven myself if she had died at this facility. To all of you that read this, please visit your loved ones often and at different times. My mother cared for me growing up. So i did the same for her in the end.

A relative was a resident at Friendship Ridge and recently transferred to another facility. During the months after the change of ownership, during my visits, I heard caregivers in the elevator, hallways, and even when attending to my relative complaining. When packing her things on moving day, I found some of her clothing thrown on the floor in her locker with dust balls over them. As far as cleanliness in the room, there was a layer of dust at least 1/2" thick on the furniture/shelf next to her bed and on the shelves to the TV stand. I just wish I had transferred her sooner.

My Father has been in this home for a long while now. The nurses and Drs are very good with my Dad and take very good care of him.(I couldn't even take care of him and he is my Dad) .He has 8 children living all over the USA, they never get mad when any of us call to check the status of Dad,(Some in Colorado, Florida, other side of PA, Ohio, & Maine). He likes the food and also some of the nurses REAL WELL!!!I I thank Frienship Ridge for everthing they do for my family

I had the opportunity to visit Friendship Ridge on many occaisions over the past twenty years. The facility has changed its once hospital feel and environment that it used to have. It has evolved over the years to look and feel more like a "residence" for their community, with updated decor throughout. The facility is a large skilled nursing facility which proivides excellent care for the residents. Each nursing unit is fully staffed and they are more than willing to assist the residents on their floor with the best possible care, even if it is only to keep someone company with friendly conversion and hold a hand. Friendship Ridge is committed to the care of their residents and updated the facility to include several amnenities such as an onsite beauty salon, snack bar and gift shop and even several activity rooms. Residents are able to attend church services, play games, make crafts, visit the library or even enjoy the outdoor courtyards. Many residents can enjoy visits with family members outside at the pavillion. Friendship Ridge also has a special unit dedicated to those residents in need of extra care due to dementia. I have always had a positive experience when visiting Friendship Ridge and would definitely recommend it to anyone looking for a skilled nursing facility in the area.

There were four people to a room and one central bathroom. My gram hated it it. When we would visit her the hallway always wreaked of urine. The one nurse on her floor spoke poor broken English and I caould not understand her let alone my gram. Most of the aides were wonderful and most of the nurses were nice. I could never get passed the odor.
