I was at Heartland twice this year. My first stay was in February and I was reluctant to go due to poor experience with another Rehab home. My first stay was excellent. The Rehab therapists were awesome. They worked with me to help me get home as some as possible. The nursing staff right on down to the cleaning staff were wonderful. When I had COVID the last December and was asked were I wanted to go for rehab, without hesitation I said Heartland of Mentor. This stay was as good as the first with the exception of Covid restrictions. The restrictions were not as bad or strict as the hospital, but still there. Again physical therapy was beyond awesome. Again nurses and staff down to housekeeping were great. Everyone was friendly, took an interest in you, did all they could to make sure you were comfortable. I could go on all day singing praises because they deserve them. With that said if I ever need rehab, definitely I will choose Heartland of Mentor!

Just plain DIGUSTED

Think twice about sending anyone you care for to this facility, My sister-in-law was there for about 8 weeks, she felt like a prisoner, not able to leave her room for any reason, told by the staff that no one was allowed to leave their room. My wife who has power of attorney for her sister was never contacted about anything going on, she would call and never get an answer and only two times did she get a call back EVERYTHING THEY TOLD US WAS A LIE When we were attempting to have her moved to another facility they told her that if she left she would have to pay them $20.000.00 she was under Medicare and Medicaid, they told her she would be responsible for the money herself IF THERE IS ANOTHER OPTION PLEASE TAKE IT

Hartland is good for rehab. But for long term nursing care it is very poor.The staff will tell you anything just to get rid of you. There is very little follow through on requests for patient care or patient requests. Almost nothing is explained to the patient. Nursing staff is suppose to provide information to the nurse practitioner who handles the treatment as the physician comes in once a month. Communication with in Hartland is a mess phone calls aren't even returned when your on the hipa list and questions are never really answered. They always seem to be short staffed or have temp working. Would never place a person at Hartland now, years ago it was a great place.

The team here at Heartland of Mentor are very attentive. The therapist have been awesome and very knowledgeable The medical staff is top rate they do even the little things that you wouldn't think about. They actually talk to you not at you. This is the only facility i would highly recommend to others. Thank you Heartland of Mentor!!

If you are in need of rehabilitation of any kind: I reccomment Heartland of mentor. The work hard to get you well enough to get you home. Nursing on down to cleaning staff get's my 5 star rating.

The nursing staff was very professional and willing to go above to help you out. The staff was excellent at assisting patients when they are recovering. I really liked the food especially the breakfast. The therapist are very well trained and will to go out of their way to assist you, Their goal is to get you home here at Heartland of Mentor. I would recommend this facility to anyone!!

Heartland of Mentor has been nothing but the best. The therapist, nursing and management staff has been so wonderful to me and my family. Thanks to Heartland of Mentor I am now at home and in great shape. I would recommend this facility anyone. They are truly the best!

My father was a patient at Heartland of Mentor 2 times. He received excellent care from the nurses, aides and therapy team. Everyone was so caring and compassionate and really seemed to care not only for him but also for me and my family's experience. I credit Dad's ability to go back to living at home on the awesome staff at Heartland of Mentor.

Being my grandmother's caregiver for years, it was hard to see her have to go into someone elses care. I was unable to do the things a 'skilled' professional could do, so I thought. I would visit everyday and ask nurses questions. I've been questioned, pushed aside, disrespected. The administration wasn't any different. I've had many phone calls not returned, was told different things by different administators, and most importantly they don't follow SOP for admin/discharge procedures. The list could go on. If I would have known all this was going to happen I would'vve chosen elsewhere!!!

My mother was in Heartland of Mentor in the rehab section. She was there for about six to eight weeks. We like it very much. They also have a long-term nursing wing but we didn't like that as much. The facility itself was in a wooded area. The living environment was very lovely. They also have gardens and stuff outside. You could take somebody in a wheelchair out there. The staff was good. The care was good. They have a wonderful rehab staff. She had therapy three times a day with different people for physical, occupational, and speech therapy. They were very good at their job. They were prompt and they took her every day. She made some real progress so that she was able to leave and go live on her own again. I would definitely recommend this place for rehab. My mom was in a room with another person. I wouldn't say it was spacious; all you have room for was your bed, your little nightstand, a kind of wardrobe thing for both people, and then a bathroom that was shared. That's adequate when you're there for rehab, but if it came to living there, that wouldn't be adequate for her. You also don't have your own furniture there in their room. We wanted my mom to be able to have some of her belongings. Overall, they were good to her. However, as far as staying there, it wasn't the same kind of atmosphere as the other places. It wasn't homey to live there.

Most staff friendly, generally clean. Some poor experiences with 1-2 of the aides that were providing care (slow to take relative to the bathroom when needed).

Heartland of Mentor does Nursing/Rehab for clients The nursing staff is very caring and helps the client be comfortable the best they can. When my relative was there and was at risk of falling she was kept a close eye on by the nurses and aides. The meals were healthy and tasty, The aides would help feed the clients who couldn't feed themselves. The only area that could use a little attention to is the rooms/floors being cleaner----but the laundry/bedding was kept clean and my relative was always clean while there.
