I was in the rehabilitated hall for care after I was in the hospital due to heart and other problems . the care and rehab I received was great. All the staff is curious . I loved the aides who took great care of me and my needs.I got to chose the food I wanted the kitchen staff is wonderful.I enjoyed my stay.

My mother-in-law, my mother and my sister have all lived and passed on from SEM Haven Nursing facility. I cannot say enough good about it. They have activities, entertainment and the facility is very clean. The staff is very caring and kind. Of course you can always find someone who is not nice but for the most part people there really care. SEM even has outing like going out to eat and even going the the Baseball Hall of Fame. They even have a yearly trip to River Downs. The Gathering Room also houses the cafe where company can eat with the residents at a very reasonable cost. They have a Chaplin who does Bible Studies and sing alongs, etc.

I was there visiting a friend of the family. To start off the whole building smells. I highly doubt they regularly clean. Especially the bathrooms. I've been there numerous times and have over heard the stna staff talking very rude to the residents. For the cost of this facility it is definitely not worth it. The food is a joke. The medical care is unacceptable. When you enter the building the lady that works at the front desk is rude and unprofessional. To make things worse they are now starting to put ankle bracelets on the residents. Which is so inhumane! The nurse must put in a code to deactivate the ankle bracelet for the resident to leave or enter the building. If no code is entered and the resident tries to enter or leave the building the doors will forcibly shut on the resident. Quite quick if I may add! This is a serious safety concern. If the doors shut on the resident it can seriously harm them or knock them over. We were not informed of this by any of the staff. The door shut on our family friend and on us. We ALL could have gotten hurt! The lady at the front desk started screaming at us because we were not informed about the ankle bracelets or the codes. It is a staffs responsibility to inform visitors about these type of things. Don't just assume everyone knows about these insane new devices. Communication is key! The woman at the front desk acts like she hates her job and takes it out on the visitors and the residents. She needs to find employment elsewhere. Don't send your loved ones here to live if you care about them. I would rate the staff and facility zero star's if it was an option.

Very family oriented with lots of personal attention. Staff tries to make everything as much like a home as possible. My grandfather was here for physical rehabilitation as well as nursing care and I couldn't have been happier with his care! And we loved seeing the resident cat!
