My friend has been a patient with this facility since 2018. Poor patient care and lacks properly trained staffing. When a patient and/or their friend needs assistance, they act like they don't hear. Aides and nurses violate patients bill of rights. Either they overmedicate or undermedicate. Patients given medication for other than the real cause.

My initial impression at Liberty Nursing was that it looked nice. This is where I had a lot of interaction with the staff. The staff needs a lot of help. The general appearances are fine, but the staffing cannot handle the patients there. They are too overloaded, too overworked, and incompetent. They are trying, but they are too overworked, and they cannot properly care for their patients. They are not good, and they cannot properly do their job. The food there is bad. I would not eat there. I have seen it just about every day. It is for my friend, and he is only in rehab. As soon as he finishes his rehab, we are going to get him out of there. The place is nice, but they cancel their activities. We've asked them to bring our person to the activities, but they never do. They don't follow through, and they don't keep a record of anything. It is horrible, and I would never put anybody there unless they get their acts together. They need to improve their staffing.

My brother and I visited the Liberty Nursing Center one day, and we spoke with a lady who gave us a tour. It was a very nice facility -- expensive -- but very nice. She showed us two rooms, and we went to where the residents met for entertainment. There were people in there, and they seemed upbeat and happy. It was new, and it was not really crowded. It had some shared rooms and some private. The way they were set up made you feel it was really private.They offered some high level care, which I would have needed for my mother. The only concern that I had was that she has to have an air mattress, and then I was a little concerned because there were no rails or anything. I know they can restrain her, but I was concerned about her maybe falling out of bed, and their beds were not a hospital bed. The person who gave us the tour was extremely upbeat, she knew the patients by name, and they knew her.

My mother has been in Liberty since July 11th. I have been to see my mother and have observed that there is not enough staff. The aids have dropped my mother twice my mother is missing clothes. My mother can not walk or stand on her own I have been on he phone with my mother for over an hour in which time she had requested an aid. I was on the phone with my mom and mom asked the aid to move her and the aid told her she couldn't move her then I heard the aid say I could move you but I'm not going to move you. I could go on about what I have observed. I speak to my mother as much as I can and she is so upset about the way the aids treat her. My mother is very clear and sane. An aid did tell my sister that they are already short staff and on the weekend they are only half staff. I know it is hard work to care for older adults but after talking to other family members and speaking to my mother regarding her care I don't feel that they are giving her the care she requires.

My brother is in Liberty Nursing Center of Beavercreek. He had a stroke. It is a brand new facility. It is also just around the corner from where I live. Their rooms are amazing. They have excellent and very friendly staff. He doesn't like the food only because healthy food doesn't appeal to him, he would rather have hamburger or hotdogs. I'm not impressed with their activities. I don't think they have enough yet because they are new. Also my brother has only been there since April. What they could improve on is their activities and their food as far as giving residents choices. However, when you put everything into perspective, the cleanliness of the facility and having friendly and caring staff, those come over everything else in my book as a caregiver or a nursing home facility.
