Admission to Mount Carmel Home Care requires a physician's order and is based on a patient's medical needs, homebound status and the type of skilled nursing and/or therapy services he or she requires. Each patient's unique plan of care is designed to ensure continuous care and improvement while he or she transitions home from acute or sub-acute care.
