About This Community
The first thing everyone notices about Split Rock, is the genuine warmth and the friendliness. We are all about creating that feeling of home. Our residents are charged with creating their own environment to ensure they are most comfortable during their stay here.

During your stay, we work closely with family to ensure they are closely involved in your care. We encourage lots visits, invite them to care planning meetings, and schedule lots of phone calls and letter writing, which all helps keep the connection to home.

Our expert care led a multidisciplinary team of nurses, social workers and therapists, work closely together to bring our residents to their highest level of function. Both during their stay here, and after discharge through our Adult Day Care & Home Care programs, our goal is to maximize the patient’s ability and return them to their prior state.

When a patient is ready to be discharged, it can sometimes be frightening for them. But at Split Rock, our goal is to return them to the community in comfort. This includes a regimen of follow-up care, housing and medical assistance, and care counseling, which helps make the transition smooth and pleasant.

Smiles really do help with recovery. Our hand picked staff are not only chosen for their expertise in their professional fields, but also we ensure they are friendly and outgoing. And everyone at Split Rock knows that the administration is always interacting with both staff and residents, making sure everyone has a chance to be heard.

I absolutely love the workers. They care about the patients even though some don't but I understand that they are practically overwhelmed yet they do their jobs perfectly. The rehab was amazing. I loved it.

Courteous and professional staff, a very friendly and welcoming environment. Also very clean with a great ambience, I would definitely recommend Split Rock to provide care for your loved ones, they have excellent nursing staff.

My cousin was a resident at the Split Rock Rehab facility in the Bronx, for close to two years, before we moved him closer to his children out of state (recently). During this entire time, we could not have been more pleased with their care, attention and devotion to him and to looking after his wellbeing. We participated in family meetings and they were always responsive and honored our wishes that we be contacted, pro-actively, even for what other families might consider "small issues/things." We will always be grateful for the time he spent at this facility. It's unfortunate that in some nursing homes, people aren't treated with enough dignity and respect. Every human being is somebody's loved one and all caregivers need to remember that. Thank you split rock healthcare!

FROM BAD TO WORST: After having a quadruple bypass in August 2017, my wife Lydia was sent to Wayne Nursing and from there to Split Rock "WORST MISTAKE EVER". Don't know how to start, everything there is a mess, The place is filthy, unprofessional staff, They don't follow the patient's diet, They take forever to answer the emergency call button, During the weekends and nights there's only one Nurse per floor and nobody at the Nurse Station and if you don't stay alert of the medications that you take; there's a good chance that you might get overmedicated; Oh! and it will be almost impossible to speak to the Doctor; just make sure you ask the person they sent WEARING A WHITE COAT IF IS A DOCTOR. I did try to get my wife out during the first week, but they gave me the runaround and it took me almost two months to get her out. If you love your family; stay away from Split Rock and if you still don't believe me, go to Split Rock "INCOGNITO" and ask the patients.

The staff at split rock rehabilitation were very good to me. The food is great. The care is excellent. My wife was a previous resident and liked it here. Her care was excellent and that's why I chose to come here. I pushed for the hospital to send me here.

The staff and service is very good at split rock rehab. The rehab staff will really bring you back to who you were. I will come back to visit when I leave

Split rock rehab was there for me when I needed short term rehab and they did a fine job. Some aides were better than others (a few were rude and should not be working in healthcare and interacting with patients), but I still give them 4 stars because they were close to home and the rehab was exactly what I needed.

My Mom liked 2 of the CNA's specifically, because they went out of their way to assist. One of the nurses was very sweet. We appreciated the care given

Do NOT send your loved ones here. They are severely short staffed. Furnishings look like they come from Goodwill. You have to bring your own TV. Staff rather put your loved one in Diapers than toilet them. Staff prefers to spend time in Dining area watching TV. It smells on certain floors. Staff is RUDE. There are better places.

my mother received excellent rehabilitation services in the facility. Most staff were very helpful and friendly. They are working on the facility, but there was no noise on my mothers floor.

My father was a patient at split rock for a week. The floor smelled of foul human odor. It smelled like infected blood to me. My poor father layed in vomit for at least an hour when I arrived I dont know for how much before I arrived. Feces seeping out of his diaper twice!!! Was with my father all day on Saturday and Sunday, was not turned or moved or changed the whole day. Called social worker and supervisiors numerous times never returned any of my calls. Asked for the patient relations phones number, and was told to fetch it in the lounge - nowhere to be found to say the least. Patients asking for money?? Its horrible to see what healthcare has become. People dont care about others its just a paycheck. NO ONE SHOULD EVER BE MISTREATED THIS WAY!!
