Home Health Care Partners is a licensed, not-for-profit home care agency serving nine counties in upstate NY. We have a long-standing, reputable history of providing excellent medical and companion services in the home and community. Home Health Care Partners readily responds to help other agencies provide medical care to home care clients. All services are overseen by our Registered Nurses who work closely with you, your family, and any providers involved in your care. Our goal is to provide you with the very best home care possible and to do this with honesty, integrity, compassion, dignity, and safety. We are the first and only licensed agency in our service area with staff certified in end of life care.

Home Health Care Partners was instrumental in preventing an out-of-state placement for my relative. Their staff are reliable, compassionate and talented in caring for someone with dementia and post-stroke complications. Our family is grateful for their continued involvement and ability to keep my family member safe, comfortable and content.
