My family member there has had bed sores a lot and is kept in a wheelchair for too long, blood in urine for weeks before they did anything. There food is poor in size and always cold, some CNAs and Nurses care but most are rude and talk to patients bad. I heard a nurse said when a patient dropped their food " Now you will have nothing to eat" Sad, Call light will be on for too long before someone comes and all the money they used to remodel the building makes not sense when wheelchairs are broken and beds don't work and they are under staffed CNAs working doubles for a week straight, this places need prayer ..
I wouldn't board my cat here.
Triad has now become Genesis Meridian. My mom was in the center for over 10 years. My mom received good care until Genesis purchased it. They spent money renovating the facility vs. on the CNA/RN staff. The were severely under staffed.
One day, I received a call that it was time to call in Hospice because my mom's Dimenia. They also casually mentioned had bedsores. One turned out to be stage 4 and this was the first I heard of it. During that day, they didn't turn her in bed for or change her diaper for 8 hours.
During the Hospice evaluation, the nurse learned that she has a serious infection and sent her to the hospital. She was Septis. As soon as she was treated, she was alert again. If we hadn't found this when we did, she would have died that night.
The hospital also discovered that her Albumin was 2.0 and the normal range was 3.5-5

my sister broke both hips and needed a place for rehab close to her home. Triad was the place.m the administrative staff did more than we could ask for transporting her for her radiation. They heard our concerns and acted on them. They went far beyond the extra mile to make sure she was comfortable.

SunBridge is actually a multi-tier system that works well from what I can see. I did not get a tour that I would have loved but I did get a quick walk through because there was something going on within the building.
Outside was lovely. The grounds were kept up and there were a few elderly folk chatting it up and having a great time on a few of the benches placed outside.
In fact, SunBridge is just one of many facilities across America owned by the same corporation so you know they must be doing well for themselves and once you step inside the building you will see what I am talking about.
The staff was very helpful in answering questions that I had and the guest did seem to have enjoyed themselves while I was there. I would definitely give this place a try and see where it leads.
