We recently had a chance to visit our relative that had been placed in the home a few days prior to our visit. The facility had very little notice that my relative was coming to their facility. I appreciate the fact they were willing to accept him every other home said he was too young or they did not have the staff to handle his special medical needs. The staff were so very nice and did the best they could to make him comfortable.

My husband was recently a patient in their sub-acute rehab unit and I could not be more satisfied with the level of care and caring. The rooms are single and kept very clean. My husband was very well-cared for and loved the food and the staff. Staff were very courteous and responsive to his needs, and he was always kept neat and clean. He participated in PT/OT and the therapists kept me abreast of his progress and further needs every step of the way. The physician caring for him was excellent and got him on the right meds at the right doses, and he continues to do well at home. I can think of nothing that needed improvement. I would highly recommend this rehab facility as a first choice to anyone who needed a combination of very effective physical rehabilitation and excellent nursing care.

I placed my father in this long term care facility and it turned out to be a nightmare. I visited every day and their work staff of nurses are very unprofessional and very lax. There are no standards they either day or night shift follows. I pulling my dad out immediately. Meds aren’t given on time if at all. I sat with dad for three hours waiting for him to get PT and no one showed up. Don’t put your love one in these peoples care.

I was in Jefferson Health Care for a little while and I liked it. I know some people don’t like cafeteria food, but I liked it. The people there were very friendly to me, and I thought it was pretty good. The staff was really good, and they never really said anything bad to me. I had physical therapy, they helped me after my heart attack, the rooms for rehab were private, but the rooms for the nursing section were shared. Every now and then they would have something going on for everybody. The facility was getting older, but they were meticulous in keeping it clean. I thought it was pricey, but I was told by several people that it was a normal price.

This place is so out dated the furniture etc . are old miss matched and depressing. The walls are dark and dirty. Windows are filth, haven't been washed in years. The food is gross. Looks like they think they are feed children. Sandwiches (dried up bread) small, small servings ( my husband lost weight while he was there). Complaints were not addressed. Social worker didn't think complaints were valid and said so. There were rude and uncaring employees. Unbeliable actions toward my husband. My husband was here only a month. And when I said I was changing him. The service got even worse. On the day he was dismissed they took an hour and a half to get papers ready that should have been ready at the planned dismissal time. Then I got no help carring out his belongings. 80 yr old women had to carry tv to auto a long distanance ( I could believe even after I ask for help they told me they had no one available. It took me four trips because I could not care the heavy weight. ( however with the determination and anger I managed everthing. Even the TV. Rating 0.. I don't understand how they have any business. But they do. If you are reading this for reference. Please check out other places. This is a dump and uncaring employees B

On two occasions he was not sent to the hospital in a timely fashion. He was not sent till I arrived . Both times either situation could have taken his life. One occasion allowed to go septic, second a dangerous blood clot (deep vein thrombosis ) My brother was paralyzed. A leg amputee , with severe emphysema weighing about 100 lbs.. My brother had a photo of an elderly lady falling one crawling , Jefferson HC called the Lee's Summit Police on my brother because they said it was criminal . Although against HIPPA ., or their policies it is NOT criminal . They had wasted the police officers time however, succeeded in scaring my brother to delete the photos. On December 1, 2015 the staff (Administrator/D.O.N/ Social Worker/Dr on staff) carefully had come to the conclusion that my brother needed to be evicted /discharged/ dumped at my residence or City Union Mission(homeless shelter) on December 30 ,2015. Neither place was safe or adequate for his needs. A hearing was scheduled for December 29 2015 , with the state to see if that was going to even be possible as my brother needed daily oxygen and medication for blood clot . How sick was my brother ? He died before the hearing date December 22. This facility lacks Medical Professionalism., Moral Integrity , and the ability to adequately Communicate properly..

I recently went through Jefferson's rehab. Their rehab department is excellent. Carmine and her girls in therapy were wonderful and got me moving better than I had in years. Private rooms were great and comfortable. Of course it wasn't without some hickups, however I was overall very impressed. Last year I rehabbed at a nearby facility that was much larger, however their service wasn't as attentive and I was actually injured due to incompetence, and this was nt the case at Jefferson. Care here was very attentive and individualized.

Top notch service. Very good staff, especially Physical Therapy staff. Room is nice and clean. No complaints whatsoever. Would not hesitate to come back. - Dale

I recently Placed a loved one at Jefferson Health Care. The facility is very clean and smells clean also. The staff are friendly and answered all my questions without making feel rushed.

Touring Jefferson Health Care today is different than it was in the past. Things have greatly improved over the years, especially in the last year under new management. It is clean, smells like cookies or popcorn depending on the day, staff is very nice and residents are content and happy. Did great on their state inspections and have added great additions to their staff.

Our mother was treated poorly by staff members and her medications were either not given or recorded improperly. No one on the staff knew which. Administration and staff supervisors lie on a consistent basis. The food is fair on it's best days and rarely warm. The facility employs a handful of good nurses, therapists and aides. However, it is very under-staffed and the rest of the staff is unkind and uncaring. They should not be allowed to call them self a memory center or be allowed any patients with any kind of brain injury. People who can not defend themselves or tell what happened should not be there.

My father-in-law was a resident at this facility. He is an alzheimers patient. In every situation, I try to always look for the positives. However, at this facility, we found very few. From the very beginning, we had our doubts about this facility. When we moved my father-in-law in, they could not find any admission paperwork for my wife to sign. The staff seemed very "hands-off" as far as their patient care. We have been lied to on several occasions regarding the care he was being given, his activities, and even what room he was in and what he was doing. For example: we were told over the phone that he was sitting in his chair watching his tv. When we arrived unexpected, he was in a room without any of his possessions, while another patient was sitting in his chair and watching his tv~ When asked what was going on, the staff was very rude and very uncaring about the situation. I am hopeful that they have gotten better regarding their patient care. We praise God for the day that we were able to move my father-in-law out of that facility. And I wouldn't recommend it to anyone that cares about their loved ones.
