My mother was a resident of the rehabilitation center during most of the month of December, 2019. The staff was great / took the time to answer our questions and answered the call light quickly and cheerfully. Kelli, the admissions coordinator, greeted us right when we arrived and was very helpful in getting us settled in. Mom's room was very nice and was kept clean. The food was fantastic, and she always received what she ordered at the breakfast and lunch meals - the accuracy of the supper trays was not as good - often items were missing from her tray. The therapy staff provided appropriate physical and occupational therapy activites during her stay. I would have liked to have seen Mom participate in more of the activities that were available - not sure if she wasn't asked or if she told the staff that she didn't want to do them. I would highly recommend Heartland Health Care Center - Fostrian Rehabiliatation Center to any family looking for this type of service.

Food is inedible Average call light response is 30+minutes Wrong medications given, staff not familiar with correct dosages. Pharmacy issued meds listed on allergy alert. Third shift staff is a horror. Rude, angry, uncaring, unkind. Do not go here if you expect good care.

If I could give less than 1, I would. They are very unprofessional, they steal from patients, the staff is more concerned about themselves than patients care. They serve cold foodleave patients unattended for hours and are annoyed when called upon.

My husband was at Heartland Health Care Center-Fostrian. When he first came in, the girl said, "If you have to pee he can use the urinal, and if you've got to poop I'll take you to the bathroom. But I'm not going to hurt my back and I'm not gonna hurt you." Well, he got the bed wet, and she came back with a clean sheet and she was going to put it on the wet bed without cleaning it off. The rehab was fantastic. We were there a year ago and it was fine, but some other company took over and it was awful; they wouldn't come to you room when they were supposed to and they didn't take care of him like they're supposed to. The food is awful, like everybody that came in brought food for their relative. The therapy was fantastic, but the nurses and aides were terrible. He had a roommate but the room was OK and could have been a little bigger.

One watchful caregiver

I visited this facility and was not impressed at all. The Nursing home section looks old and no updates for those patients who live there day to day. No decorations to look at. Could smell odor of urine when walking through the halls.. The patients were lined up by the Nurses desk like they had nothing to do. Just sitting around.. Don't they provide activities for these folks? It looked sad. There was a male patient with a tube connected to him just sitting there and you could see writing on equipment. Nothing covered over. The Rehab Unit looks new and many staff were working in that area. I would not recommend this facility. All of the patients should have similar accommodations and appear well cared for. Our loved ones deserve the best accomodations regardless of how long they will require others to help with their care.

This nursing home I wouldn't recommend to my worst enemy. Aides will walk by your. Call light no care. The nurse that really helped me would only be there she was part-time employee. Wish she was full time. Most of the nurses are lazy and late with meds. The doctors sick too. Don't send your loved ones there

My Mother was there for rehab. They had on her chart having heart failure and kidney failure. She had neither one!! I confronted them and the floor nurse could of cared less. They said they would correct it. Is this a medicare scam? Were they medicating her for this. Two weeks later, It was still on her chart!! They didn't change her sheets in a week. Couldn't get an aid to do it, so I did it myself. They couldn't get her meal right and the food was horrible. They never brought her water. The list goes on and on. The next day, I pulled her out and took her home. It needs to be shut down.

I clicked one star because it wouldn't let me choose no stars. We pulled our loved one out of this place after being there one week. We did it against their medical advice but on our own doctors advise. They were very angry and belligerent with us. There is a total lack of communication between staff members like doctor & nurses & aides. My family member should have been on a soft diet but they were bringing her things like hamburgers. They do not help with daily hygiene and if you need help and put your light on you better hope it's one of the male aides that respond because they don't have an attitude that you are bothering them. My family member had diarrhea a lot but she was left to clean up herself even though she was weak and elderly. + they gave her several glasses of juice a day...something you can't have when you have diarrhea!!!! We could never get the doctor to address the diarrhea (again, lack of communications between nurse and doctor) And, good luck trying to get in touch with the staff doctor. Our staff doctor was NEVER available to us until we decided to walk out with our loved one.

Staff are very nice & friendly although my father has dementia & was there for rehab & received no help with his day-to-day hygiene. Clean wash cloths & towels were piling up in his room but no one checked to see why they were not used. The Food Is bad. Usually all a beige color. Not even sure what the meat was most of the time. And my father could not be discharged until the doctor called his guardian. We waited a whole week and the guardian never received a call from the nursing home doctor. We are still waiting. Would never put a loved one here by choice.

My mom was at the Heartland Health Care Center-Fostrian for some care after being in a hospital. They had assisted living in a separate building, and we looked at that, but we thought it was a smaller operation and she would not do as well there. She didn't live there at all, but I liked the people at Fostrian themselves. She was a very belligerent kind of a patient in the beginning, and they did really well with her. I was surprised. She had a lot of exercising in there, and she did really well with that. The exercise program for her rehab was excellent.
