With nursing low staffed, you need to learn how to help yourself the best you can. Be assertive and alert and tell them you can assist yourself. Let family or friends know if you need anything. Room is OK, but I do not like the beds and sleeping is not easy for me here. Some of the staff try their best, while others may not understand why you are here. Nurse gives your meds and lets you review them before you take. Food is OK, make sure your tray is for you with the paper slip having your name. Pressing your red button for help may take awhile for them to come to you so try yourself to do as much as you can for yourself.
Patient phone is useful as your tool to use for other departments.

My father was discharged from Beaumont Hospital R.O. 2/3/2020 to Evergreen Health & Rehabilitation Center. There was no edema in my father's lower extremities (legs and feet) bilaterally. When I arrived to visit my Dad 2/5/2020 at 12:38 pm, there was noticeable swelling of the lower extremities. My Dad has congestive heart failure and fluid build-up can kill him. I inquired with the nurse practitioner. The NP informed me that there was nothing she could do about the swelling in my father's legs. I asked her if she would contact the medical director (doctor) on staff to obtain an increase in diuretic or compression socks to combat the swelling. She stated that there was nothing she could do for him. I inquired with the staff nurse about elevating my Dad's legs and she stated that the wheelchair my father was in was not equipped to elevate his legs. I then asked if she would contact the medical director (doctor) to obtain an increase in diuretic or compression socks. She stated that she had called the medical director (doctor) but the medical director (doctor) had not returned her call. I then called the medical director (doctor) but there was no answer and the telephone disconnected after I heard the message, "thank you for calling, good bye". Every visit with my father, I noticed that both of his legs, ankles and feet were swelling more by the day. When I arrived at Evergreen 2/8/2020, my father's ankles were pitting +4 and his legs had blisters, open sores and were weeping. I also took pictures of my father's feet and legs. I complained to the staff nurse again and she stated that the medical director (doctor) was not responding to her call. I had to believe her because I called to speak with the medical director (doctor) three days in a row and received the same message "thank you for calling, goodbye" prior to being disconnected. On Saturday 2/8/2020, I placed two pillows underneath my Dad's feet prior to putting him to bed. When I arrived Sunday, 2/9/2020, I noticed that some of the swelling had gone down. I then realized that the staff nurses nor cnas had been placing a pillow underneath my Dad's feet to decrease the swelling. It was then that I knew I was removing my Dad from Evergreen before they caused irreparable harm.

We found out today that my mom does not have to return to Evergreen to finish her rehab. I’M SO THANKFUL!!!!!! They’re going to do it at the hospital. Many but not all of the nursing staff and aides aren’t doing their jobs —- just barely enough to get by. Word to the wise - make sure that you consistently and frequently check on your loved one. Otherwise, they will suffer because staff aren’t doing their jobs.

My mother entered Evergreen Health and Living Center on April 30, 2019 for rehab and was there for over three weeks. My observations are mixed. On the good side, it's a beautiful facility, and most important, her physical therapy and occupational therapy were very good. I live far away and was able to come in to town to see two OT sessions. The therapists gave her exercises that pushed her and the facility mimicked a home situation: bathroom, kitchen, stairs. They also arranged a special session with me there, where I showed them Mom’s bathroom setup. If my calls to staff weren’t answered right away, I did receive a call back (especially from Heather, the head of the therapy department), with detailed answers to my questions and a detailed description of Mom’s progress. I also received helpful information on Mom’s medical condition from the medical records department. Mom said all the staff were very kind, and the ones I met (e.g., nurses, attendants who brought meals) were extremely pleasant and attentive. One problem: even though on Mom’s whiteboard in her room it said “no coffee, only hot water with meals”, the staff kept bringing her coffee. A few problems did arise that related mostly to administration. I called Evergreen on May 1 to see about Mom’s rehab. The person at the nurse’s station was new, and said that Mom wasn’t on a list for rehab, and that they didn’t know anything about her. That disturbed me, and I left a message for the therapy department; they called me back later and told me that with the new staff mistakes were being made, Mom was being evaluated, and that an update would be called to my sister (primary contact) in several days. On my mother’s pre-determined discharge date, one of her caregivers came to pick her up, and no one at Evergreen was prepared for her discharge according to both the caregiver and my mother. No one had her paperwork ready, no one helped her pack, and both women had to wait at least a half hour for an Evergreen employee to pull the paperwork together. This is unacceptable. They should have had the paperwork ready, her belongings packed, and should have had Mom waiting at the door in a wheelchair, ready for her caregiver to pick her up. Her caregiver had to do all this for her. So I’d give three “grades”: a “5 star” for her rehab, a “3 star” for the on-floor staff, and a “1 star” for the administration.

This is my dad's second trip here for Rehab after a serious infection. The only true good thing here is the PT! Everyone tries to pass the buck of blame, they are either short staffed or have hired people that just don't care about the quality of the care they give people - and that is on every level but Physical Therapy.
When you do inquire about an "issue", you are told it will be taken care of - but it never is. Multiple phone calls go unanswered. It took 4 days to change a soiled pad on my dad's wheel chair - 4 DAYS! That is unacceptable.
When my dad was discharged the first time, they wrote that he didn't need assistance with anything - but he NEVER did anything without assistance! Because he "refused" a shower - he was sleeping and they woke him up (they showed up a 9pm at night finally to bathe him when the schedule says 9am-3pm) - they put he didn't need assistance with hygiene on our discharge papers. Yet they had to have 2 people assist him with showering and not once did he ever do this himself. I asked them to correct it and they gave me a hard time about it. In case anyone thinks this isn't an issue? When you try to apply for VA or Medicaid Benefits - they look at what the facility had to say about his ability to care for himself. IT MATTERS!!!
I reached out to the Administration about the issues we faced - it took 3 phone calls and 4 days until someone finally called me back.
This facility is the only one close to our home which is why he is there. But this is NOT by any means a 5 star facility. I am truly disappointed that Beaumont rates this as one of their "top" facilities - makes one wonder if Beaumont ever bothers to look at the multiple reviews of problems this facility has logged??
I find it interesting that , of course, they only show the best, glowing 5 start ratings they received - but dig deeper and you will find that this facility only gets a 2.5 star rating.

Evergreen is a very high quality run facility under incredible administration. I have had several relatives stay there. My grandmother stayed there after receiving a pacemaker surgery when she was 84, which had left her very weak. She went to evergreen for physical therapy to regain strength , and received very high quality care and assistance for all of her medical needs. Upon her discharge from the facility, she felt stronger and healthier than she had in years. This is just one example among many of the high quality care given at evergreen. My great uncle and my best friends grandfather also stayed at the facility, and we have all been extremely satisfied with the care and hospitality that is present at evergreen!

My grandmother spent 2-3 weeks here on three separate occasion, her and our family were very satisfied with everything the facility offerred. The nurses and staff are very attentive. Even helped sterilize toys for gramma's youngest visitors to keep them entertained while providing excellent care. I would stongly recommend giving this place a chance for your loved ones. Leo

I'm so thankful my mom is home and doing well, after being in Evergreen Health and Living Center for 2 1/2 weeks certainly I owe them "no accolades"" People please if you read this review, do not put your loved ones in this facility. The medical care is inadequate, the food is lousy, and most of the CA's that are staffed here are lazy. I dediced to place my mom here after I was told by Botsford/Beaumont in Farmington Hills that she needed to be in a rehab facility to become stronger before returning home, and it would be easier for her medication to be monitored if she stayed in network with a Beaumont facility. I looked at 5 of the 7 facilities Beaumont offered, and this appeard to be the best out of the 5. "WHAT A JOKE", this was one of the most ill-run facilities for elderly patients that I have ever seen. My mother did not even see a doctor, until I took off an entire day from work, called Dr. Winters local Livonia office, and called Nurse Manager Stephanie at Evergreen Health. He then appeared on scene after she had been there about 9 days. My mom is a diabetic, her blood sugar was not monitored, her vitals were not taken reguarly and when they were it would be 4 or 5:00 pm. I eventually brought her glucose machine from home and we took her blood sugar ourselves. Dr. Skoures (my mom's primary doctor) called and gave instructions to Evergreen on how do monitor/and do her insulin and they could not even get that right. The dietcian showed up about a week after my mom was admitted, took her food order for 2 days, and after that she never saw her again. The food that was served was cold and most days not edible, so almost everyday I visited, I would bring her dinner from home, so she could have a decent meal. She had 3 nurse assistants, that were kind, considerate and caring, Stephanie, Nicole, and Angela. The others were noncaring, noncommitted and seemingly just there to get a paycheck. A few nights she put her help light on and waited over 2 hours for assistance to go to the bathroom, and when ALMA arrived, she told my mother to hurry-up, I have other patients to attend to. UNBELIEVEABLE!!!
My mom was truly sent to this place for rehab therapy, so a shout out to AARON for all the help he gave her. He did his job!
The facility is pretty and very up to date, but like the old adage says "NEVER JUDGE A BOOK BY THE COVER", and please by all means avoid this place if you love your loved one. My mom was discharged on October 19, 2017 and is doing fairly well, she is 78 years old. I wonder what happens to the elderly people at Evergreen, that don't have close family and friends to speak for them and don't have a voice???

My mother was in an assisted living facility and needed a higher level of care. Evergreen Health and Living Center's staff and facility have met her needs. Medical attention, care, and food have both met and exceeded expectations. This was a decision I had to make on behalf of my mother, I am reminded on each visit, why I selected it.

This facility seemed to be one of cleaner, warmer, less clinical, more involved place for my grandfather. They do regular social activities, have beautiful outdoor courtyards, and very attentive nurses. Even though he had been acting very unruly and not himself, they removed a certain medication we weren't familiar with and he got better after that. He was being very picky about his food and what he would eat or not. The nurses encouraged us to bring in any of his favorite foods and treats so they could get him to eat more of what he liked. They made space in the pantry and fridge and gave him whatever he wanted. Ice cream social day is nice... Visitors can enjoy a cup of ice cream with the patients and stroll around the facility and courtyards. The only thing bad that happened during his stay with them was that he rolled out of the bed. Not sure who was more at fault, there wasn't any thing that could have been done to prevent it besides large bed rails that stay up.

When I first walked into Evergreen Health and Living Center, I could tell that my relative would enjoy it very much. You are first greeted by a front desk assistant who directs you toward the room you're trying to visit. All hallways lead through the dining room, a large, well lit room with a player piano. This room is lined with open windows, as well as couches and chairs for residents to visit with their family. When visiting my relative, I always noticed that her food was delivered to her consistently and she was treated with great care by the staff at the facility. My relative also had to undergo physical therapy, which was painful and difficult, but the doctors and staff helped her through it tremendously. Before I knew it, my relative was walking and living on her own after breaking her hip months earlier.
