Leominster Rehabilitation and Nursing Center (formerly Keystone Center) is a 74-bed nursing home located in Leominster, Massachusetts. We are conveniently located in North Leominster, near the Lunenburg and Fitchburg town lines. We have been serving the Leominster, Fitchburg, Gardner, and Lunenburg towns for over 35 years.

This facility is absolutely horrible. My sister is here (working on finding somewhere else). The food is disgusting and she can hardly stomach it, they do not give her a shower unless WE tell them to, they refuse to honor their contract to provide transportation to medical appts and the nursing manager is sarcastic, rude and demeaning.
The facility itself is filthy and desperately needs a bleaching.
I will say that there are about 3 CNAs that are good there. But they are woefully understaffed and overworked.
No amenities. Just sit and stare at the wall or tv.
