About This Community

At Elevate Fort Wayne, we understand the challenges of caring for those with dementia. Unlike other types of senior care, dementia care services must be tailored to the specific symptoms, risks, and challenges related to the different types of dementia. Dementia Care services offer specialized in-home support to those with early-stage, mid-stage, or late-stage dementia. Our caregivers help our clients maintain quality of life so that aging in place remains possible. We also provide family caregivers with much-needed respite.

IT’S ABOUT HAVING OPTIONS

Dementia Based Behavioral Program HOPE SPRINGS is a 33-bed dementia care unit operating within our skilled nursing facility. Our dementia-based behavior programs are set up to meet the specific needs of the resident. Both male and female residents are accepted at HOPE SPRINGS. Case by case assessments are performed to help determine how HOPE SPRINGS might serve an individual’s needs.

Care & Attention

We provide 24-hour nursing care in a warm, personalized manner. Our care is a flexible, interdisciplinary approach to compassionate care in a comfortable, homelike setting.

I'm very happy with it. They have an excellent activity director who keeps them as busy as she possibly can. My sister-in-law's studio is very nice and spacious enough for her. Their menu is pretty varied, and they have two menus that they can choose from -- one for specials for each meal and a restaurant-type menu with other things they can choose from if they don't like what the daily menu is -- so they have a lot of choices. The food is good, and they try to accommodate what everybody wants. One of the main things I like is that they can transition from assisted living, to memory care, to nursing home, more or less in the same building, so they have all three in the same area.

The people were very helpful and very friendly. It had an apartment atmosphere with long halls and a lot of doors, so it felt more like an apartment building. The residents seemed to be happy.
