Traditions at Brookside is a licensed senior retirement community with no large entry fee or endowment. Residents are served by providing superior, yet reasonably priced accommodations and services in a peaceful, quiet country atmosphere. Living options include licensed assisted living, assisted living memory care, and independent living garden homes. We provide a home-like setting for those who need assistance with personal care and daily living activities.

The personal service plan is created to allow residents to live life on their own terms, while enjoying the company of others in a community setting. Licensed 24-hour nursing staff are available to monitor healthcare needs for those residents needing assistance with personal care and daily living activities.

ASSISTED LIVING IN MCCORDSVILLE, IN

Traditions at Brookside features home-like residences in a setting for seniors who may need assistance with personal care and daily living activities. Our Wellness Director creates a personal service plan specifically tailored to each resident. This plan may include assistance with daily activities, such as medication management, bathing and/or dressing assistance and much more, all provided by a 24-hour licensed nursing staff.

The Independent Living Garden Residences at Traditions at Brookside offer maintenance-free active retirement in a variety of spacious floor plans. Our Garden Residences are all on one level and feature fully equipped kitchens, relaxing sunrooms, and beautifully landscaped yards. All appliances are provided and most Garden Residences include an attached garage. Best of all, no utility bills!

The Garden Residences are conveniently located on our beautiful campus, with easy access to the main building that features a full-service restaurant, chapel, movie theater, beauty salon and fitness room, as well as a monthly calendar of events.

MEMORY CARE & DEMENTIA CARE IN MCCORDSVILLE

Traditions at Brookside serves seniors who experience various forms of memory loss and cognitive decline. Individuals are assessed by our staff to identify their cognitive and physical abilities, as well as learn about each person's unique life story to develop a comprehensive plan of care. Both physical and emotional stimulation are then incorporated into the plan to support and maximize quality of life.

A state-of-the-art security system is also provided which allows residents to live independently while remaining within the comfort of a secured environment.

Traditions at Brookside incorporates, along with group activities the innovative Behavior-Based Ergonomics Program (BBET) to improve the quality of life for our residents with Alzheimer’s and Dementia.

We toured the Traditions at Brookside. They had very nice facilities. The place was very nice. They showed me a one-bedroom. I saw their courtyard for walking. They had physical activities, and they had a chapel. I liked that it was super clean; it was my most favorite thing about it.

My mother is in Traditions at Brookside. The community is great. The staff is fantastic. Her room is very nice and very clean. They've got everything there, a family room, dining room, movie theater, a chapel, beauty salon, and computer room. She loves the food. The meals are great. She really enjoys it. She has participated in bingo. She has done some cooking and some arts and crafts. They're taking care of her very well. It's a little expensive, but she's happy. It's a great place.

I went to Traditions at Brookside but it was too confusing. There were too many hallways and zigzags, and my parents would get too confused. It was out in the middle of nowhere, and it would be hard for my parents to find if my dad was trying to drive. So I had to take that into consideration. It's a beautiful building, everything was in good repair and everything was clean. The staff was very friendly, everybody said hello and everybody was cordial, that we passed. You could tell, they cared about each other. The facility itself is too big for my folks. Their courtyard is very pretty, they had some raised beds that they could garden on, and that was nice. They have microwaves and refrigerators in the rooms.

I had a very good experience with Traditions at Brookside. I was very impressed with the place and all the things they had to offer. The representative did a fantastic job making me feel welcome and introducing me to people. It couldn't have been better. The rooms were exactly what we were looking for. It looks like it's well maintained. There were quite a few people that were cleaning and vacuuming and serving breakfast while I was there, so I was very impressed. The dining area looked very nice. I saw where they had their activities. It was early in the morning, so not much was happening at the time, but it seemed like they offered a lot of different activities and areas to have activities.

Probably what I liked about Traditions at Brookside was the cleanliness, the size of the rooms, and the facility. The staff was very professional and knowledgeable. I only talked to one person. She maybe was Director of Sales. The size was adequate for an assisted living apartment. The dining room was spacious. The other rooms that they had for activities were clean and spacious. They had a theater room that was unique that I hadn't seen before, and that was nice. They had an enclosed courtyard, which I didn't get to see. I just looked at it from afar.

We went to Traditions at Brookside, and my mother-in-law liked it so much that she didn't want to see any place else. We were very impressed with it. The facility itself was very nice and well taken care of. The staff seemed very knowledgeable and competent. They answered any question we had. The services they offered were what we needed for my mother-in-law. Our overall impression was good to excellent. I saw the assisted living and memory care. The staff felt after talking to her that she really belonged in memory care. We originally went there to look at the assisted living part; but after seeing that and talking to my mother-in-law, the staff felt that she would do better in memory care. They seemed very concerned, and they knew how to talk to my mother-in-law without coming on too strong. They asked her questions directly and were really good about including her in the conversation. They answered all of our questions to our satisfaction. They were very friendly, concerned, and seemed very competent. We met the head nurse and the head of the memory care unit, as well as the executive director. We did not go during mealtime, and we did not try the food. We asked them about the food. It's a menu that has the same things every day, plus a daily special. The dining room seemed very nice. The staff has been there for quite a while. They don't have a lot of turnover, which was important to us. The facility was clean, well kept, and very pleasing to the eye. It looked brand new. it was just very nice first impression inside and out.

My mom moved into Traditions at Brookside. It is very nice, they keep it immaculate, and their staff is very friendly. The maintenance guys helped us move her in, and set up furniture. They did a whole lot more than what their job description called for. They were very helpful with that, they keep very close tabs with my mom, and they keep close contact with me. They have restaurant style eating under normal circumstances, but now they're serving them in their rooms; it's a lot of prepackaged food, and so my mom was not thrilled about that. The room is very spacious, lots of natural light, a nice bathroom with all the hand holds, pull cords, and safety related things. It's very tastefully done. They usually have a happy hour every day; they have a jukebox that plays oldies, and lots of different games. My mom has a dog, they have an area where she can get off her lease, and run around. They have some great rocking chairs out there, and she has made a lot of friends; other dog owners out there.

We moved my dad to Traditions a couple of weeks ago. I like the location, it's close to where I live. He has a door that leads outside and one that leads into the hallway and once covid is over we can come and go through that door without having to go through the lobby. The community has been great other than the restricted visiting due to COVID. They were very helpful. He said that everyone there is very nice with him. They go out of the way to make him comfortable. They walk the paper and hand it to him and not just leave it at the door. They deliver the newspaper every day. They are kind of spoiling him. With the staff, I have no issues with anyone yet. He likes his eggs prepared in a certain way which they don't offer. It is expensive but we had no idea going in how much it was so it was just figure-shock really. But they have a lot to offer so it's good value for the money.

My mom has lived in the assisted living section for two years. She is very happy and comfortable in her spacious one bedroom apartment. The staff is friendly and kind. They are incredibly caring to my mom. She loves the contained outdoor courtyard. She enjoys bingo and her bible study group.

My mother will move into Traditions at Brookside on August. Our experience was very good, the woman I worked with was very helpful and friendly, and the facility was just a couple of years old, so it's very well-appointed. There's a large brook or stream that runs behind it, and there's not a lot around, you have to drive to get to other places. It's laid out more like a hotel, it's bigger than a house, I think they have 75 assisted living residents, and then they also have memory care. Everything is updated, she's going to be in a studio apartment, it's a nice size, and it's got a little kitchenette. The dining area is lovely with white tablecloths and white napkin service, full restaurant service, three meals a day, and doors that connect to a patio. It's lovely like a resort. They have beautiful patios and a courtyard.

I think what was really impressive was Traditions at Brookside. It was new, only 2 years old, and the assisted living houses were nice. We saw a theater, dining area, and they looked very good. However, they did not take Medicaid.

Staff is caring and friendly. The facility is less than a year old and is absolutely beautiful! There is a nice variety of activities offered from arts and crafts to guest speakers to movies and musical entertainment, to community trips.

Overall staff is friendly, and facility is very nice. Very pleased with all of the activities that are offered, as our mother is very active. The only negative feedback is that we were surprised to hear the monthly rates were going to increase when my mother had only been living there for six months.

On 3 Feb 2017, two Traditions@Brookside (T@B) employees, including a supervisor, assessed my mother at another facility (MM). They were aware she was eating pureed food. MM prepared and Dr. signed a detailed 3-page Order Summary Report (Doc 1) that included “DIET: PUREE diet” on the top half of page 1. T@B accepted Mother and gave me forms to be filled out by MM to support a Mar 6 transfer. MM filled out and Dr. signed T@B’s forms (Doc 2) and also updated Doc 1 (Doc 3). Doc 2 had a line “__ Resident choice from regular diet menu”. MM did not make a check mark on this line, indicating she was not eating from the “regular diet menu”. Doc 3 included “DIET: PUREE diet” on the top half of page 1. T@B changed the transfer date to Mar 13, then Mar 20, then “hopefully in Mar”, then “in a week or so” and finally Apr 10. By then, Docs 2 and 3 had expired and were redone (Docs 4 and 5). The Doc 2 line quoted above was again not marked in Doc 4. “DIET: PUREE diet” was included again in Doc 5. Starting June 2014, my brother drove 1000 miles one-way and I drove 1100 miles one-way at different times each month to visit our mother. During T@B’s delays, by brother delayed his visit so we could both support the transfer, causing neither one of us to visit for about 3 weeks. He made a visit, drove home and then returned a week later to support the transfer. With each delay, I changed the rental-truck reservation needed to move furniture. While moving items into her room at 10 am on Apr 10 in preparation for her transfer at 1 pm, the supervisor informed us our mother was being rejected because she was only eating pureed food. The supervisor knew that on Feb 3. She claimed MM told her they could transfer my mother without stating she was eating pureed food. MM denied saying that. Why would an MM employee alter existing documents that would place the therapist and Dr. at risk when the therapist had not and was not planning to perform an assessment to determine if my mother could eat food processed “mechanical soft” or not processed at all (regular)? Because T@B does not accept residents eating pureed food, they should have inquired before doing the assessment on Feb 3. I had to go through the entire procedure again to find a new care facility. Mother was transferred on May 8. On Apr 10, T@B told me my $1000 deposit would be returned. I called on May 15 and the check had not been printed. I received it on May 19.

We are going to be moving my parents into Traditions at Brookside. It is nice because it is brand new. When I first visited, it was under construction, but now it is finished and really nice. They have a workout area and a movie theater. You don’t have to sit down at a certain time and eat; you can go and get what you want whenever you want in the dining room. That is something that my dad really likes because where he is now, you have to eat lunch from 11 to 1. They have a courtyard in the center. We worked with Kara, who was very nice, very helpful, and showed us the different apartments that they had.
