Specialized Skilled Nursing:
At Plainfield Health Care Center, each physician, nurse, and nursing assistant is here because of his or her dedication and impressive experience caring for a diverse range of medical diagnoses and conditions. Every short-term, rehabilitation, respite and long-term resident receives around-the-clock licensed nursing care seven days a week. Our complete list of nursing-care amenities includes daily monitoring of patient vitals and assistance with bathing and dressing that’s both proactive and respectful.
Long & Short Term Care:
Even when looking forward to a well-deserved vacation, we know how stressful it can be for families to place a loved one into the care of others. Whether it's a week, a year, or you simply need respite care for just a few days, our thoughtful and brilliant team of nurses is on-call 24/7 for your peace of mind and convenience.
Rehabilitation:
At Plainfield Health Care Center, we are rehabilitation experts. Our facility was designed and recently remodeled to maximize patient independence for residents in our Stepping Forward program. In addition to some of the most advanced physical therapy equipment and our state-of-the-art rehab gym, our facility now boasts a total of 22 private rehabilitation suites.
Memory Care:
At Plainfield Health Care Center, we've designed a specialized and separate unit to meet the distinctive needs of individuals with Alzheimer’s and other dementias. Designed to maximize resident safety, the Director of our Caring Hands Neighborhood, Dr. William Arnold, implemented numerous different types of security protocols and capacities such as 24-hour supervision provided by our experienced and professional nurses and a keypad authorization to open every exit and entrance to Caring Hands facilities.
Hospice Care:
Hospice care at Plainfield Health Care Center includes short- and long-term care. Our nurses will coordinate with hospice care providers to ensure that the resident is well taken care of throughout the stay in our facility. We strive to ensure each of our residents is happy and healthy. We know that times can be difficult, but we will make sure you or your loved ones are with those that provide incredible care and give the utmost respect.

I would like to take this opportunity to express how thankful and appreciative I am with the care my loved one is receiving at Plainfield Health Care Center. The kindness and support of his caregivers interacting with him daily does not go unnoticed. His caregivers go above and beyond their duties, he is so blessed to have each and every one of them. I would like to especially thank Erin the therapy director for her compassion and unrelenting support to make his daily life a little easier. I realize not everyday can be perfect , but it's not for lack of trying at this facility. Thank you All!

I was so impressed with the Plainfield Health Care Center. They were organized, kind, and orderly. The place smelled great as they do not allow any carpeting in there, and so it was just impeccable. They were doing construction, but they kept it away from them. So, it was just a nice atmosphere.
The gal that did the tour had nothing to ruffle her feathers. She was just calm, delightful, and pleasant. She wasn't an alarmist. I've had to deal with her several times throughout the process, and she's always been the same all the time.
The rooms are also big. My mom and dad are going to be in the same room, and it's bigger than their apartment, and I'll be able to put a lot of their furniture in there. There'll be a couch, a recliner, and a coffee table, too. It's going to be like a little home with their bed in there.

Our very elderly mother broke her back and needs a great deal of nursing care and comfort care. She was first at another Plainfield Care Center and had several disturbing incidents of her medication being administered wrong causing real problems. She has been at Plainfield Health Care Center for a month and the nursing staff has been excellent. We all met with the care plan group which addressed her needs from medication to comfort care, her likes, dislikes, expectations and the offerings they had for her. The nursing staff is all employed by PHCC and not contract staff on the weekends so care is very consistent. We have gotten to know them all since we live close by and they are all on the same page and up to date if medication changes. The nurses and doctors are doing all they can to help her. They are kind, caring and truly concerned nurses. There are several nurse aides who have worked in the field for years who know the best ways to aide in her comfort The newer aides learn much from them as well as from their own training. The nurses, aides and all are show real care in addressing issues of nutrition since she has difficulty swallowing normal meals. Everyone treats her with respect at all times. Even the head of maintenance has the warmest greetings and steps in asking her often how she is doing and gives her a smile and has words of good cheer. The administrators have put a together a very caring, cohesive staff that does their best.
The facility is older but clean and the room décor is up to the family. The food seems to be very institutional but residents can request soup or an alternative to some meals. At this point we are very satisfied with PHCC and are grateful to those who recommended it based on their experience with their loved ones.

When my dad first got out of the hospital, he was put in a nursing home called Plainfield Health Care Center. I wasn't happy with that place. The staff was not friendly. It didn't seem like he was getting the proper care here. He ended up going back to the hospital. They need to take care of their patients. They need to be kind and friendly to them. They need to talk to them. A lot of the issue stems from the staff. If you wanted to get answers to what his status was, I couldn't get answers from the staff. The rooms were very dingy, and it didn't have a nice clean atmosphere or smell.
