Our mother spent the last four months of her life in Able Hearts/River Crossing of Elgin, IL. This review is not based on the caregiver side of their failures, but, the administrative/corporate/management side. We've had experience with five other nursing home facilities, three in IL and two in FL and Able Hearts/River Crossing is the absolute worst. Mom's last night there was September 6, 2022 and on the 9/8/22 we attempted to contact their business office manager to settle up on the financial end and clear out Mom's belonging. It was at this point we realized we were in for a major challenge because there was no response whatsoever from management. After multiple attempts to get information regarding the 24 days of refund the family was owed, they insensitively replied "there is no way I can give you this information and can't tell you when". We expressed that we needed that money to use toward Mom's funeral as well as help with family travel and they could care less. We tried going to a higher level of management which is the corporate offices in Tampa, FL. and it was actually worse . . . zero assistance or understanding of what our family was going through. Shame on them!

I visited the River Crossing of Elgin and it looked nice. I heard bad things about it when it was under a different name, but this being that it was different people, I thought I'll go into it. It looked nice and clean. It looked like there were quite a few people there. They were down in the dining room playing bingo when I had my tour. They share rooms and they have a bathroom on each one. They have an exercise room for physical therapy, but that's also a rehab facility. I'm sure that's why they have to have the physical therapy room. There was just one lady who gave me the tour and she is very nice. She explained everything. We sat down and talked, she took me through the tour, and then we sat down and talked again. I told her all the things that my mom would need help with, and she didn't seem to have a problem with any of that, because my mom needs help with everything, dressing, going to the bathroom, showering.

I wanted to thank the staff here. Especially Lauren, Admissions. She was extremely helpful in getting my concerns and messages across to the appropriate staff. She facilitated what we needed taken care of for my mother-in-law who was there in Rehab. As a healthcare worker myself I understand the challenges COVID has made for all who have loved ones being cared for away from home. I understand the facility is under new management and from what I experienced they are doing a great job. Thank you all.

Basically, they hire people that are not compassionate and they just come there to collect a check. My mothers is in this place came in with a small wound and now she has 2 because it goes 15 hours and 10 hours and her diaper don't get changed. have anyone had hired a lawyer that want to share that lawyer name with me???? this company needs to be shut down again for negligence tours patients and employees need to be fire for negligence and mentally abuse by screaming to to residents/patients and treating them as crazy by their CNA and Nurses. please let me know if anyone find another facility that is good with their relative and a lawyer that can fix them and close the company down for negligence.

The communication is terrible. It is very difficult to have them answer a call. After 3pm, it's like a ghost town there. The staff doesn't share information among each other. They don't read the charts to become familiar with the patients. Patients spend a great deal of time in their beds. Half the building has no phones in the rooms. There is no or terrible internet access. Patients are moved from room to room with little or no notice. There was no addition support or communication aid during COVID-19.

I'm a current resident here at Fox River Rehab. Formerly known as Rosewood. I've been here for about a week now. And I'll say this, if you care even a tiny bit about the person you're considering sending here, DON'T! If you are thinking about coming here, keep searching. The staff here is completely inept. They don't know what they are doing. They spend their time talking to each other and mocking residents. My roommate who has a catheter in, apparently they forgot to do something that allows it to work properly (I've never had a catheter) I've spoken to multiple people here and they are all miserable here. I had a nurse completely lie to me yesterday about something and I caught it. Now she's just staring at me like she doesn't know what to do. I'm begging anyone who reads this... please do not come here, do not give these people business they should be shut down for abuse. I'm getting ahold of a lawyer as soon as possible on behalf of the residents here. This place should not be allowed to continue to stay open. Again. Avoid this place like the virus it is! The food is horrible and tiny portions. The rooms stink Tiny TVs that barely work. This is supposed to be a rehab facility? They have 1 tiny rehabilitation room. With almost nothing in it and I've yet to see anybody use it. And I hope the read this because I am still here! -Matthew Gjoen If I could I would rate this place 0 stars. Facility is clean, admin is wonderful and beyond helpful. Nursing staff that sits behind the counter is awful. Why should beeping and lights flashing be constantly going? Why should these things be ignored? Why does a patient have to wait FOURTY MINUTES after pressing the call button before anyone shows up. I could go into detail with several stories, but I don’t have enough space. And this is only after our mom is there only 2 weeks. I HATE that she has to live like this. Unfortunately our mom is beyond home care. Shame on you people. We totally get it that she is not the only patient there. The patients are ignored when help is needed.

This place was terrible. My mother has been there multiple times, while we thought it was alright at first, the second trip proved to be an experienced never to be repeated. The food was terrible and at times in edible, and always cold. The worst part was, she got very bad scabies. They have persisted for months. She left in early September and they are still not gone. Both she and my Dad have had to have many rounds of medication, and house cleansing. When we called, they did not care and we're not helpful.

The facility is all good, actually. It's nice and it's not really expensive. The people working there are very nice. I got an apartment which is pretty much a basic room. I've been here two months. It's close to everything, like stores and stuff. They offer a lot of activities like board games.

My relative was in this facility for a few months. She was 98 years old, and in perfect health. She was in this facility to rehabilitate her strength and physical attributes while recovering from a fall. She was treated poorly here. Her health declined to the point where she was deemed "unresponsive" and "close to death". She was in PERFECT HEALTH! before entering this facility. When visiting, I noticed her diaper was completely full. When I asked a nurse, she told us they only change them a few times a day, and cannot break schedule. When I asked why my relative was hungry, the nurse said she missed the meal because she was asleep and would not eat again until the next day. When my family took her out of this facility, I took care of her on my own for the following two years. The facility told us she wouldn't last a week without their assistance. All she required was food and care. Those two things were NOT given to her at a nursing home! Disgusting.

This facility is located in a safe residential area but sits on a relatively busy street. The entire residential area has well maintained sidewalks so it is possible to go on nice long walks pushing a wheelchair if one deisres. There is an outdoor space for residents and families to utilize though it is not large. The common areas are adequate but not overly spacious. Resident rooms are of typical size,, meaning not overly spacious but adequate, and most are shared. Some rooms have a pleasant view of green space from the windos and others do not. The staffing patterns seem to vary and at times it seems there is less than adequate staff. The food is average with some items appealing and others not.

When I first walked into Rosewood Care Center, I was struck by how well-maintained the facility seemed to be. As I proceeded with my tour, this thought was confirmed. The facility itself is beautiful. The rooms were spacious and appeared customizable to make each resident feel more at home. The staff that I talked to seemed to be friendly and knowledgeable. The physical and occupational therapy room was very large and had a lot of equipment that would suit a variety of needs. They even had a dog in there, at that time, to bring some joy to some of the residents. The only negative that I saw during this trip was the overall layout of the building. The hallways seemed to intertwine and I found myself feeling lost on a number of occasions. As their clientele includes a lot of residents with dementia and Alzheimer's, it may be difficult for them to confidently navigate throughout the building.
