The staff here at Avantara Chicago Ridge is very caring and attentive. The rehabilitation is excellent. Their physical therapy and their OT staff are really good. They help me a lot. The menu most days is very good. They have movie dates, exercise, devotionals, a library, and games.

My brother-in-law was at Avantara Chicago Ridge. He was getting rehab there until they let him fall and break his femur and did not even know it was broken for two weeks even though he asked to have an MRI and told them straight out that he could not lift that leg. They thought it was his knee and it was arthritis, so they just totally ignored him.
He is on a G-tube and they didn't pay as much attention to him I felt. They also let him get a bedsore on his arm when he was there for rehab because they never bothered to take the arm out of the sling. Nothing was the "staff's job." Everything was somebody else's job. They said, "Oh sorry, that's not my job, I don't know." I felt like, "Whose job is it? How do I find out?" When they had a care review, we didn't have half the people that we needed. I didn't get to talk to a doctor. I was not happy with it at all. They were too busy fixing the place up to care about what they were doing.
There wasn't even a chair to sit on in his room. You stood there and then they wanted you to go sit in the room when you were doing stuff. I kept taking them to the lunchroom because there was no place to sit and talk, but they wanted to talk in the patient's room. The room was small.
The dining room seemed like it wasn't getting used very much, but that's because they're in the middle of revamping everything and it seems like the rehab is going to go into the dining room, so I don't know where the dining room is going to go if they're gonna have one.
My problem was when they let him fall and wouldn't take responsibility for it. The facility upkeep and maintenance were pretty good. I didn't see a problem with that. It's not like it smelled, but he was supposed to go back to his doctor's appointment and they didn't have any of his clothes cleaned for him. They tried to expect us to come up at 10:00 at night to bring him clean clothes. I was not happy with that place at all. The atmosphere was not good. I didn't find that they were very communicative with my brother-in-law. He couldn't talk very well, so they weren't going to bother trying to talk to him. He might not be able to talk, but he was able to understand, so that made me a little upset, too. A couple of the staff were good, but most of them said, "It wasn't their job."

My mother have been in this facility for 3wks and it’s awful!!! She was told to use the bathroom(poop) in her diaper instead of bedpan because they were out. She also have a sore with a bandage covering it on her bottom where the lazy nurses have to change every so often. Yesterday I asked for the nurse to change it because it had feces on it and it will set up another infection that she’s received since being there. I intentionally stayed up there to see how long it was going to take but eventually I had to leave. I’m learning they didn’t do anything about it til 12 midnight and I made the request at 6pm(ridiculous)! My mom is very alert of how they are treating her and I’m documenting and taking pics of EVERYTHING! Haven’t spoken with the D.O.N. Theta make next step to following protocol. I’m taking this to the highest. Oh they ran out of diapers and wipes and towels…HOW????

My husband went for rehab at Avantara Chicago Ridge twice. I think he liked the food, as far as I know. They did do therapy, but they couldn't go downstairs to the facility because of COVID. He had to actually do his physical therapy in his room. When I went there, everybody was nice. I had no problems with the staff. I do like the fact that they have you come out, so I have my husband's progress. They are very cautious about COVID. They had to put on a gown and mask up. I thought the staff was knowledgeable and helpful. The building was clean. The waiting area that they have you sit in until they come to get you was clean, but the furniture was a little old. They do accept Medicare and Medicaid.

This is a horrible facility!! Please don’t take any of your love ones to Lexington. The staff never check up on you at all, when you press the call light for someone to come in the room it takes them 10 to 15 minutes, they are very unprofessional especially when it comes to your paperwork, it’s a rehabilitation facility that only cares about how much money they can get and not about helping the patient, no matter who you call you never get results or see results, the social worker/case worker Jenny mentions how she has other patients besides you to deal with that’s why she can’t get any of your paper work done, the food the facility serve is horrible ( I had to have my love ones to bring food up to the Facility daily), the silverware and utensil that they give you are so filthy, every time a CNA entered my room they never knew the reason I was at the rehabilitation I had to explain to them why I was here which I felt was so unprofessional and they should of given each CNA, nurse, or doctor paperwork stating my condition, and the staff has a hard time listening to the Patient needs & wants they neglects it!! I thought my experience would of been better than what it was and it wasn’t at all.

PLEASE, DO NOT BRING YOUR LOVED ONES HERE!
1. Mom was left lying in urine overnight
2. RN left meds out in an open area
3.Mom is on a diabetic diet, I was given the menu sheet for ANOTHER PATIENT! the was given SOMEONE ELSE'S menu. #HIPAA
4.The décor is horrible and dirty!
5.Arrived at 9:00, mom's soiled linens were laying on top of the garbage can. At 3PM... still there... I left them there purposely to get a picture.
6.Receptionist, with the hat on, acted as though I was bothering her when I had a question. I'm sorry that I interrupted your personal cell phone conversation and also sorry that I interrupted your personal conversation with your co-worker. No need to roll your eyes letting me know that I was "bothering" you!
7.The screws need to be tightened on all of the toilets on the 1st floor! I had on gloves... and did a few for you, BECAUSE I CARE about the next person!
8. I was waiting for mom to be served but I had to wait until the staff stood there and ate and talked about their hair, nail, babies... first.
9. Patient was sitting in the lunch area at 9 AM and was left there.. when we came back for lunch... the poor baby was still sitting there, alone!
10. The tech's have the worst attitudes in the world
11. The poor patients are there for rehab, right? They sit them in this big room and just leave them. What kind of rehab is that?
This is some of the saddest treatment I've witnessed. JCAHO needs to shut this place down!!!!!

My mother is a patient there I am currently trying to get her out of there. There is no caring going on in this home. Patients are not taken to the bathroom, sit in urine and feces for hours, nor do they bathe patients regularly. You have to beg staff to provide any services. No one there washes their hands either, they only wear gloves when changing adult diapers cdiff infections are rampant due the lack of hygiene. Rude, lazy and incompetent describe the staff.

Father placed here by young wife. Home always keeps him over medicated in an out of E R. . Medical patient mixed with mental patients on 3rd floor. Been there three years. Wife won't allow daughter to take him for visits hone allows him drueling at the mouth an disoriented. Over medicated under nourished teeth rotten etc.. No improvement since thee63 years. Horrible place

My mother was a patient at Lexington and died there in November of 2015.the first time visit eight years ago was so pleasant. I recommended itto everyone.T the final and last visit was horrid. my mother was neglected, frightened, and bruised..I will never have a good thing to say about this place again.

Lexington of Chicago Ridge was closest to my house, and my father in law went for rehabilitation when he had trouble walking. We like the staff and how they took care of them very well. He likes the physical therapy, and he likes the food, games, and movie nights. He has no complaints.

Mother is in Lexington, but she's only been there four days now. It's nice and clean. My sister said the food wasn't bad, and they're friendly. They seemed to be taking good care of mom.

Although I'm not one to write reviews, I just could not let this experience past without informing other NOT TO PUT THEIR LOVED ONE IN HERE FOR ANY REASON!!! My grandfather was admitted here for a post-hospital rehab stay. He was discharged before he even stayed two weeks despite the fact that he is still not walking. I encountered dirty bed linens and soiled undergarments in his room where the nurse left them. They also place residents who are a fall risk at the nursing station for hours on end and get upset with the patients when they complain or have to use the bathroom.Then, his cellular phone came up "missing"! Upon asking the nursing staff about this, one nurse took me downstairs to their laundry room and the staff told me they had no idea what I was talking about (although I could tell then they were trying to cover up something). After three days, the phone miraculously appeared and had been washed in their washing machine. Without notifying the patient or our family, they took the liberty of placing his phone in rice and allowing it to absorb the moisture before they returned it to him. It now shows clear signs of water damage and his insurance will not cover this. Of course, Lexington's staff(CNAs, RNs, and social workers) LIED to us about knowing where the phone was to cover up that their staff DAMAGED it and now they are claiming that there is nothing they can do about it. One nursing supervisor even told us they were doing a good deed by placing his phone in rice without asking and making the situation worse. We were given the run around for a week straight and then they tried to act like they didn't know what we were speaking of on his discharge day. In summary, I would never recommend this place to anyone. The staff is just simply collecting checks.

My mother was at Lexington Center Chicago Ridge. It was OK for what you pay. The staff was OK. The dining area was adequate, but it wasn't that great.

The Lexington is a clean place, wide hallways, food variety, and with some good caretakers, like Maggie. However, my mother was hard of hearing and never received any assistance to get a hearing aid, so it was extremely difficult to talk on the phone with her. The facility could use a little more help with special patients like her, and it would have been nice if the social worker had been more active in getting her some sort of hearing device.

My mother was there and she hit the nurse button 45 min and no one showed up. Two nurses Amanda and Angela are the only two that would stop in and asked if she need anything. I witnessed the lady in the next bed hit the call button and she ended up having an accident because no one showed up. It's a clean place.

So far everything has been very good. Mom has been very happy at Lexington. She has dementia and she couldn't remember things at all. But they did a good transition and she seems fine.

This facility located on the south west side of Chicago Ridge area has grounds that are well kept. This facility accepts no psychiatric referrals, but will accept long term and short term referrals, skilled level care and other referrals per the clinical team's decision. The smoking is supervised by the staff. The meals are served buffett style. This facility is not veteran contracted, and has no on-site dialysis center for the patients. They do accept medicare/medicaid insurances. There is also a slim waiting list. While there I noticed how well connected the residents were to the staff. I would rank this facility a four out of 5 stars.
