Lebanon Care Center was old, rundown, and depressing. The residents seemed content, though, and were eating lunch at the time. The place was very clean. The staff who assisted us during the visit was very friendly and helpful. There were groups of people in different rooms depending on the assistance level that they needed. It looked like residents were getting plenty of help with cutting up their food and things like that.

My father moved to the memory care unit in Lebanon Care Center from another nursing home. In the month that he has been there, my family has been very pleased with the care. While it's not the fanciest, best looking showplace that some other homes are, the staff and personal care that the residents receive have been exceptional. So many homes focus on providing the necessities and forget about treating the residents as individuals. We've been impressed with how they are attentative to the little things that makes life happier for my dad. They show genuine interest in their residents hobbies, mental outlook, and quality of life. Each month they host a family event where they residents can get together with their family and have a good time. July was a "Christmas in July" party honoring Veteran's. They served refreshments, had a wonderful sax player, and gave small treat bags to all the residents. The staff was smiling, happy, and went out of their way to engage the residents in moving to the music, dancing, etc. It was awesome to be able to share that with my dad.

Family member lives here. Once a consistent presence is established, staff is warm, friendly and most are caring-When problems presented to staff, some are motivated to find solution- some are not.Staff burn-out understandable- Lots of cna turn-over- not as much for LVNs.Food is mediocre, at best. Facility is shabby and dirty. No real outdoor break area for staff. Not enough supplies noted for staff/ resident use. Too bad- I believe all of staff would like to provide better care, if they were given adequate resources.
