My grandfather was at this facility in 2021. When you walk in the front door, you can just smell a foul smell of urine. Visitors were not allowed in the patient's rooms, and visiting hours were restricted to 1 hour in a tiny lobby area. Calling the facility to speak with staff about how a loved one is doing was extremely difficult. Many times the phones weren't answered or the staff didn't call back within any reasonable time frame. I was so glad when my grandfather was released from this facility.

My mom is a resident here. She's always well taken care of. Her hair is always clean and get clothes are. She's always dressed and I'm her wheel chair when I come visit and get bed is made and clean. I'm very happy with the care she is receiving. She has dementia but the care team love her and are always talking about the way they live to hear her sing up and down the halls

Be prepared to work if your there for rehab. The care team is great and you have to be patient they do their best. The rooms have the necessity to keep you comfortable.

There are not enough staff so things fall through the cracks. Patients don't get the care they need because staff are spread to thin. As it is now I would not put a family member here. Just don't do it.

My Aunt was sent to this facility for rehabilitation services. She was at Brian Center Health and Rehab for a week, where she was severely neglected from dehydration, nutrition, and her physical needs were neglected. The staff were of the upmost unprofessional, non caring individuals I have ever encountered. My Aunt left this facility in an ambulance and was immediately admitted to ICU due to the severe lack of care she received in a matter of days at this facility. The care and treatment my Aunt received were deplorable. I have seen animals in shelters treated better than she received. She came to this facility with a bright outcome for physical therapy, but instead left fighting for her life. She is still fighting for her health and healing massive skin sores this facility caused by the severe neglect. I have seen many facilities throughout the years and this facility is absolute worst. Please, do not send a loved one here. There are many other choices out there that can offer your family member a better quality of life and give them a chance to heal and recover. This facility needs and should be shut down. Do not send someone to this facility and if your unfortunate family member is at this facility, look elsewhere. This is not a safe nor loving place for anyone.

I was a resident here for two weeks. Looking from the inside out I can see things from a different perspective than those looking from the outside in. Being here 24/7 I have found everyone works as a team and are always willing to step up and help their coworkers. Although this facility has a lot of character in the building, the patients are cared for very gently as if they were the only patient here. The facility is always spotless and the meals are great. The nurses and caregivers are always close by if you need them. The therapists are the very best. All in all, I would not hesitate to bring my mom here or another relative in need of Brian Center Rehab’s services.

My grandfather had a stay here at Brian Center, the staff was wonderful, knowledgeable and caring. Everyone took time to meet with us and explain treatment in a way we could understand, and you could tell they truly care for their patients. Nurses and Therapists took time to listen to all of our individual concerns for Papa, and then worked with us explaining how they would administer his treatments to meet those personal needs. There was one evening Papa had said he was cold as his room mate preferred to leave the window cracked at night, and the staff got us moved to a new room the very next day so he would be more comfortable. We couldn’t be more pleased with the care provided at Brian Center!

My 93 year old mother was sent to the Brian Center for rehab after a bout with pneumonia and a UTI. I found the facility to be clean, no putrid odors, good meals for patient's, staff were always courteous and professional in dealing with patient's and families, and most importantly my mother was taken care of, she has incontinence issues and she was always clean. I do not understand the families and patient's who are posting bad reviews. This is a Nursing Home and Rehabilitation Center, not a hotel. Having been a Registered Nurse for over 30 years, I have seen my share of bad facilities, and this is not one of them. The Brian Center is not the newest facility but the care given is outstanding. I would not have any issues with sending my mother back or going to stay as a patient myself. Thank you to everyone at the center.

My mom was in Brian Center, and this place is a lot nicer. The staff was very nice, some were very personal with the patients, they seemed happy, and it impressed me. My mother enjoyed it better.

Please find another place for your loved one. Staff is over worked and under qualified. My mom was dehydrated and had low kidney function once we finally got them to move her to hospital. She got sicker and sicker while under their care and we couldn't get anyone to listen to our concerns. Dr never visited her. Only looked at her lab results. Staff lied to us re meds given and tests results. Mom spent additional week in hospital to get well enough to go to more reputable rehab facility. So much heart ache they caused mom and family by not doing their job. Thank God mom is still with us.

My father is at Brian Center. It's not the nicest place, but it's really close to me and my brother, so it works out better for me. The food could be a little better, but the dining area looks fine. They have Bingo, but he doesn't participate. They could improve on the food, the cleanliness, and it would be better if they have extra nurses.

My Uncle was placed here from a Hospital on Saturday 9/24/2016. He has a type of cancer that has left him with a wound so bad on his left forearm that the bone is exposed and a similar large wound over at least 1/4 of his head. He cannot use his left arm. Both areas need antibiotics daily and the dressing on his arm needs changed daily. As of 6:45 last night, 9/27/2016, his bandages had not been changed AT ALL during his stay. He has had this nasty bandage soaked in blood and infection on his arm since Saturday morning. He is very weak and can barely get out of bed. He cannot bathe himself and certainly cannot get into the shower and he cannot have water beating down onto these wounds. Since they could not get him into the shower, they simply have not bathed him at all. What is worse is that on Sunday night, I reminded who I thought were two nurses (turns out one of the women was actually the Nursing Home Administrator) to change the bandage on his arm and apply the antibiotic and they assured me that they would but did not. I again reminded a nurse on Monday night to change the bandage and even though she assured me that she would, she did not. The same nurse was there on Tuesday and the bandage still had not been changed. This is no small bandaid - this wound covers his ENTIRE left forearm. The nurse (she is an actual nurse) from the last two nights gave me the excuse that this particular wound was not on her "to do list" so it wasn't her responsibility. If they can neglect such obvious and basic care with a family member reminding them EVERY DAY, what are they neglecting with these patients that isn't so obvious and who have no family to check behind them? My uncle will be coming home with us and I will get in-home care after this Medicare stay. No way would I leave him in here long term now. My advice to anyone considering this place for a loved one is to keep looking.

My step Dad as been at the Brian Center since October 2014. He had a severe stroke, and cannt use left side of his hody. Numerous times I have visited him, and he is in same position, in bed, uncomfortable, and depressed. His Air conditioning in room broke, and they promised a new one weeks ago, still not replaced. He has developed kidney infections due to catheter being twisted and no one checked him for days after complaining of severe pain. Room is always dirty, soiled diaper on chair, sweat pants soiled on floor, pillows always on floor under his bed, lunch trays still in room at 3:30, lunch was at noon, clothes, socks always missing. Reading glasses gone. Lack of care, concern and hygiene a big issue. Hopefully moving him out of this facility soon. I do not recommend this facility to anyone. They deserve 0 stars.

We are pleased that Brian Center took care of mom very well. It was an older place, and the furniture in there was older, but they were fine and clean, and they took care of them. Mom didn't like the food at all. She was there for rehab. They had some things for them to do, but she didn't participate in that because she was there with a broken pelvis. Their physical therapy was great. We had used it before; the nurses were fine, and all the staff was good. We got really close to some of them. It was a clean place.

My grandmother has been at this facility for over a year. I've never been very impressed with them. Right now she is in the hospital due to not being paid enough attention to at the Brian Center. She was rushed to the hospital due to extremely low oxygen levels and extremely low blood pressure. The recently gave our family a call and said they had been mistaken about her weight for a while. Her real weight was that of a 10 year old. This was even after repeated concerns expressed to them about her weight. Once she arrived at the hospital, doctors said she had a heart attack, in kidney failure, has a stage 2 bed sore and in their opinion, emancipated. Completely inexcusable. I understand elderly people can change in a split second but I truly believe that if they had been paying more attention, some of this could have been avoided or lessened in severity. The bed sore, well that's a complete lack of someone doing their job.

i stayed there for 2 weeks after a hip replacement. The staff was great and the therapy was too. I rate it 5 stars

I was a patient at Brian Center following a heart bypass last April, and was involved in short term rehab there. I was treated well and the physical therapy was excellent and effective. Being an RN I appreciated the level of care and promptness of answering call lights, and assistance requests. I felt safe and found the building quite safe. It is well lit and pleasant and the food is very good and the staff responsive to requests as to likes and dislikes. Questions were answered promptly and accurately and staff actually spent time visiting with me as well as doing their jobs well. I found the aides attentive and helpful, and liked the dayrooms and a wide variety of other patients there who seemed well taken care of. I saw no one who appeared to be in pain or needing attention. The windows are big and the place is perched atop a hill with good views of trees and greenery. There was no trouble parking or my family being allowed to visit. I liked Brian Center very much and would recommend it without reservation. The staff, RN's and LPN's and CNA's were pleasant and efficient. Even the dietary people often had a smile for you.

I recently visited this facility with a group of singers on Oct. 26, 2012. They wheeled a group of residents into the the small dining room with a microwave and a piano. No caretaker stayed with them. Most dropped the lollipops they had just been given. One lady kept loosing her grip on her coffee over and over again. We had to keep placing her hand upright so it wouldn't spill. Finally it did spill all over her, the lady next to her, and the floor. There were no napkins or paper towels in the room to help sop up the mess. Another singer and myself went to the desk to ask for help, or at least for something to clean up the mess. Those ladies said there should be something in the room. There was not. We then asked a group of caretakers down the hall. They said there should be something under the microwave. There was not. We finally used a table cloth to sort of clean up the mess on the lady, but she was sitting in it and uncomfortable, etc. Finally after about 40 minutes, at the end of our program we were able to get help to take these residents back to their rooms. We passed the information on to these people that they needed to have their clothes changed. It was so sad that the desk or the other helpers didn't try to page a worker or something to help with the residents. I really don't think these people should be left unattended for more than an hour at a time. I would give it 0 stars, but the computer did not accept that.

they have good help, the workers are always happy to help even with the jobs they have to endure they are always a pleasure to be around the amenities are commendable. clean bathrooms and the facility is in tip top shape. None of these places are perfect, but my grandmother was in another rehabilitation center and they were more worried about how long they could bill Medicare than how she was progressing. I have been much happier with the Brian Center, they keep me informed on her progress and the staff seems nicer. she is bathed and has clean clothes on too. she is well fed and seems to be happy with the other patients. The lighting and the overall atmosphere in the place is a tad gloomy but considering it house elderly individual it is to be expected. the parking is good for those visiting day where most of the families come to see their loved one's and the visiting hours are convenient for everybody involved. The security is good and i think i am more at ease with her there than i was at a previous facility
