It is with great sadness I write this review. COVID has certainly strained our health care workers unimaginably. However, my dear friend entered Carrollwood with sepsis and wounds. After two weeks I visited to see what her blood work indicated. They had not tested her. I spoke with the director who assured me blood work world be done. Two weeks later it had not been done. A new employee said she “would take care of it” Shona. She did. The day they received the results my friend was sent by ambulance to St Joseph’s hospital for a blood transfusion. Touch and go. The two wounds she entered with became 6 and one grew to 16 cm which required surgery. She passed away 17 days later. Please reconsider sending your loved one here. I have much more to say but will not. You will find the facility clean and nurses “busy” not with patients but on their iPads. Be warned and wary. Things are not what they appear.

My father went to this rehab. He was in very bad shape and they put him in a wheelchair. He fell out the chair and hit his head ( brain bleed) and we never got an explanation or an apology! He died a few weeks later!

We visited Carrollwood Care Center. It's bad even though the people were nice. We looked at the rooms very quick so as not to invade their privacy. But everything was walk-in, it was right there, the receptionist was there and you could see the rooms. There were three people in some of the rooms, they were just divided by curtains and their beds were almost touching. The people just didn't seem happy, and it was just crowded to get around with people also in wheelchairs. When you went over to their therapy for rehab, it was just a very small room which was sort of crowded. The staff we met was very nice, but it was just a busy place and seemed more than they can handle. The staff overall seemed caring.

My mother had a major stroke and aftetlr six days in the hospital, we came to Carrollwood Care Center for rehab. She came here unable to speak, unable to swallow, incontinent, and unable to move anything head to toe on her right side. After only two weeks, she is speaking some, is swallowing, and is able to move about in her wheelchair and bed pretty well. The facility renders exceptional, acute, aggressive care ... for purposes of rehabilitating a person back to home. They are amazing professionals ... Admin personnel, RNs, LPNs, CNAs, social worker, admissions, PT, OT, Speech Therapists, and more. The facility is very clean (wonderful cleaning staff!) and the atmosphere is jovial and positive. I cannot say enough how grateful I am for the care they have given my mother, Sally. She is coming home to Connecticut with me tomorrow, and I will greatly miss the sweet faces we have come to know at Carrollwood Care Center. This is a quality rehab center and I highly recommend it!

A complete NIGHTMARE!!! Upon arrival my mother needed to be catheterized to void her urine. 2 hours later, after numerous calls to the nurse, my mother was catheterized AFTER I THREATENED TO CALL 911! My mother was in intense pain. The nurse very casually informed me that there was so much urine that they had to clamp after 450 ml otherwise she’d go into shock. Came next day mom was in her chair in her own waste. They lost her cochlear implant device. (Thousands of dollars) and there was no urgency to find it. Once found, she was punished for it. Not allowed to have it back without supervision from a family member. So my mom had to be completely deaf for 3 weeks. Staff is utterly rude!!! Also lost were my moms dentures and about 7 outfits. Tried from the day she got there til the day she was released to get her released. The absolute WORST experience of our lives. I could go on and on about the incompetence but I’ll just say, do not place a loved one here for any amount of time!

My mom was admitted here for several weeks in 2016. The facility was clean and mom was well taken care of during her stay. They have a great staff and rehab really worked with her till she was able to return home. Thank you Carrollwood Care!

I recently stayed at Carrollwood Care Center as I was assigned to this location by the James A. Haley VA Hospital. To make a long story short, and overall, I was very pleased with patient care and services provided by the staff there. Between the medical staff I had at the VA and nursing staff at the Rehabilitation Center, I learned a new appreciation for nurses and other support services. I am surely not an expert but I was very pleased both at the VA and with follow-up care that I received at Carrollwood Care Center. Specifically at the rehab center, I was amazed how the nursing staff and their CNAs balanced all the work between patient comfort and care and still have time to explain required knowledge to me (patients) on medication, anatomy, and medical issues. I must be honest and point out, at times there was too much "chit-chat" loud conversation, and even music in the hallways that I found too loud in cases when patients are trying to sleep. regardless of what time it may be in the day or night. One morning I noticed this level of noise or music at 5:30 in the morning. There may be times during the day when it is expected and reasonable to have a hectic environment and the noise that comes along with that. I found it necessary to wear ear plugs 100% during the day and most of the night. The medical staff and service workers and CNA's seemed to yell up and down the hallways with no regard to sleeping tenants. The food was excellent and made me very happy. I consider that the facility was well maintained and clean Again, I am not an expert, I was very pleased with the rehab services (PT, OT, and Speech) that I received from the very professional and caring staff. I feel that I started there with 20% level of mobility from my medical condition,and left with 75% level of mobility after six weeks. I was provided with a plan with goals and a reasonable road to move forward and continue with my recovery.

I would not recommend Carrollwood Care Center for anyone. We were not happy there at all. It was more like a nursing home than assisted living. It was cold and seemed state-run. The staff and food were horrible. The floors were cold, smelled, and didn't have carpeting. They need to improve their food and staff.

Carrollwood Care Center was clean and seemed like it was well staffed. It was not as luxurious as the other place I’ve been, but it was nice. The people I met were very friendly and helpful. This would be my second choice. I saw some activities going on, and their dining area was very nice.

A friend from church had the call light on all night, no one came. She finally fell asleep in the morning and they thought she was unconscious. Called the ambulance and forced her, once she awoke to go the Hospital, even though it was rest she needed. She could not sleep because they did not give her the pain medicine, nor bothered to come by and help her. Also the bed had metal wires in it, that her back, which had surgery could not handle. New bed now, hard and uncomfortable. While there the following day the staff, was hostile, belligerent, and unwilling to help. One of my church friends hounded someone to finally give her the pain med's. How they treated her, the visitors, (us) and how other patients were man handled, ignored, is tragic.

I was impressed with Carollwood, the friendly staff, the cleanliness, and the good food. The staff was knowledgeable. They had an entertainment room and the very things that my mother was interested in like bridge. They also had a Wii system there, so they held Wii bowling tournaments, Wii shuffle board tournaments, Wii tennis tournaments, and Wii gold tournaments. The people we saw were very active.

When my Mom entered Carrollwood Care Center, she was bedridden, could hardly move herself, had a feeding tube, a fractured vertebrae, an injured lung, had trouble swallowing, could hardly hear or see and was disoriented and confused. Today she sits in a wheelchair most of the day, can stand and walk a few paces, feeds herself and brushes her hair and teeth, and is no longer confused or disoriented. They have taken excellent care of her: Her Doctor, the Nurses and CNs, the physical therapists, the nutrition staff, and housekeeping/maintenance have all tried to make her stay as healthy and pleasant as possible. The facility is always kept very clean and never has the bad smell that some nursing facilities have.

My mom is at Carrollwood Care Center. I like the whole facility, everything about it. The CNAs are awesome. The place is clean. The food is good. They have activities for the people. They have church, singing, and they have games that they play. The therapy is awesome.

Mom fell, and she's in a nursing home now. It's Carrollwood Care Center. This was also a long-term care facility. We looked at 2 or 3 different ones, and I like that one. The staff is really good. They treat her well. Most of them seem to be very helpful. They have plenty of staff members there. The place is clean. I can't say that the food is great, but it's not bad. She shares a room. It's pretty nice, and it's adequate. They have all kinds of games for them. They playe bingo. They take them out to restaurants once a week, or bowling if they're capable. They have a beauty shop there. They try their best to keep them active all the time. They have a dining room. The dining room is pretty nice, like the rest of the area. It's not a new facility, but it's clean and it's adequate. That's the main thing. She's been there about two and a half months now.

This nursing home is above average but is not excellent. I don't think any nursing home is excellent. Carrollwood is very nice.nursing home. It does have a little clinical feel to it. 2 beds per room, although there are a handful of single bed rooms, dividing curtain in between the beds. Nursing home has a extremely caring nursing staff. The CNA's for the most part are great. Like most nursing homes that have medicaid beds inside at times it is a bit short staffed, especially on the weekends. Their goal is to be checking on the residents that are bed ridden about every 2 hours or so, that happens about 75-80% of the time. There are those times when it is longer than that. My Father is a resident here and his wife, myself (his son), and my wife all take turns so that the only time that he is alone is during the 3rd shift. The staff take good care of Dad, regular hygiene, shower once a week, bed bath once a week, and regular washing up of him. I have had regular interaction with the administration which is strongly encouraged. Dad has regular care plan meeting to discuss any concerns that we have about his care or concerns that the nursing home has with Dad. The business office staff has been a pleasure to deal with and is very very helpful. The nursing home has an excellent kitchen and dietary department. Meals are not repeated very regularly which is very nice. Housekeeping and the laundry departments are on top of things also. There were other nursing homes that my Father could have ended up at which I have learned are not as nice as where my Father is, so I am very thankful for my Father being a resident at Carrollwood Care Center.

My fellow church members and I are part of a group that monthly visits the Carrollwood Care center in the Tampa area. It has kept this facility so clean and bright, nice smelling, newly decorated - all of these things mean a lot - but mostly, the caring and upbeat staff. We routinely go into the rooms of the residents to visit them unannounced when it is their birthday month, to say hello and bring them a small gift. Those who have been battling Alzheimers and other cognitive and otherwise physical impairments are cared for as well as cared about. Many are alone otherwise in the world due to the fact that their family or friends have all passed. It does not seem like a depressing "end of the road" place. My mother in law, by contrast, lives in another state in a place that is none of the above and we would love to have her here and closer to us. This facility is the one we would choose. She unfortunately refuses to come, as she is from the small town where she is now "confined". It is somehow familiar. But this place, literally, is where we would have our own mother/mother in law if it were up to us.

Very clean facility & very caring staff. My husband was there for 30 days for rehab after surgery. They were very accommodating. Good food and wonderful experience under the circumstances.
