The staff's total lack of empathy and complete disregard for it's patients is beyond reprehensible. I implore you to seek alternative options if you are looking for a rehab/nursing facility for your loved one. Just the THOUGHT of my mother returning to Coral Trace makes her break out in tears. Me too,,, Disgraceful.

The place is VERY depressing, sad rooms, sad furniture, and the outside areas are unappealing. Staff is aloof, uncaring, and appears to be annoyed when you try to visit. the people at the front desk have a really poor attitude. they will lie and BS you when you call them out. the whole COVID thing makes it easy for them to slack off and it is apparent that they prefer all the "regulations" and limited visitation by family members. the management needs to be removed and the place needs to be updated.

The staff doesn’t return phone calls and on the very rare times you talk to someone they are very rude and short with you. My father went in with bowel issues and they didn’t run any test to figure out what the problem was until we told them to. Then that didn’t happen until after they told my family he was going to be sent home. Turns out my father has a very contagious bowl infection that we were not told about until the night before he was to come home. Plus they said he was bed ridden so he did qualify for oxygen or a wheelchair, which I asked if he’s bed ridden how am I supposed to bring him home? Really not happy with their services at all! They don’t read his chart nor write things down in his chart. This place should be shut down!

My Aunt has been there now for 5 days. She was transferred there from a hospital after a fall at home. She has to be in quarantine even thought tested neg so she has no visitors. Her daughter is out of state and has been trying to speak to someone caring for her mom multiple times everyday. Not hippa related. Sometimes the phone is answered and a message taken and sometimes it just rings. My Aunt has a phone in her room but she only answers sometimes and appears to may have suffered a stroke??? She doesn’t make sense at times so to get info from her has been impossible. I am heartbroken that this facility can do this to a family. I am an RN with over 42 yrs experience and I have never seen this level of poor communication.

My mom has been there with dementia now for 8 years. She has always called it her home. She is cared for, always clean and enjoys everyone that takes care of her. They communicate well with the families and listen!

The rooms seemed a little small. However, my sister had an oversized bed and a oxygen machine. The staff was very helpful. My sister liked Rico RN aka Dr. Duck. The hallways were very clean. I ate a meal once and it was delicious. My sister had issues keeping food down the whole visit. Dietary came in to try different thinks to help. Administration and all stAff were willing to do everything to make stay more comfortable. My sister was in hospice care and she died 15 days after being admitted from the hospice house. The beauty shop employees made her feel special. My nephew and I visited all the nursing homes and this was the best. Most of the staff had a hood since of humor. No regrets.

I just wanted to let anyone who may be considering this facility for a family member not to place them here! My father was placed here just a few weeks ago and he was treated very poor by 95% of the staff. Their was about 5% of people that actually cared about his well being. I filed a complaint within the first week of his stay and continued to voice my issues for the remainder of his stay. 95% of the staff obviously does not want to be there you can see it allover their face and the aura they give just by walking past them. I was told I would get certain medical supplies needed before leaving and I'm still sitting here at home 72 hours later without them. I'm appalled with the staff and the care my loved one received from this facility.

My father went there for rehab-we had no other choice and only for a short period. It was deplorable !!! The food I wouldn't feed to a feral animal. My dad was usually sitting in a corner in a wheelchair sleeping. He was always dirty and had TERRIBLE bedsores after I had asked them to roll him over after 2 days. The staff is rude and nobody seems to know who is responsible for what job. After I had visited my father one night later the next evening 911 was called. He had next to no blood pressure and was non responsive-the nurse told my sister she had checked on him about 11 hrs earlier !!!!! My dad was transported to the hospital and within 24 hrs to Hospice House where he passed away 4 days later. Such a terrible situation for the family and especially my dad. How the Government can continue to let them keep their doors open is beyond me. They LET my dad expire.

Having seen how staff reacts with POA's, I have witnessed the following. Many POA's have little interaction and visit seldom. The facility is a revolving door as to CNA staff and care is mediocre to poor at best. Most all the CNA's are not US citizens and have cultural issues with white folks as they have a basic dislike, especially if the caregiver for the resident is involved or attempts to show a CNA what works best for their loved one. The CNA, rather than listening and learning, immediately gets defensive and everybody covers for them rather than trying to educate and correct so the problems persist. The fact that Consulate is a for profit facility, its,all about profit for the corporation and the residents are for the most part looked at and treated like cattle. No compassion, no communication, no respect. A sad state of affairs.

We were very pleased with the facility and staff. On our recent visit we found the home clean and well maintained. The staff was professional and very pleasant with the residents. Highly reccomend.

My mother is a patient at Coral Trace. She is very happy and well cared for here. The rooms and common areas are clean and well maintained. The staff are patient focused and very pleasant.

My mother was sent here for rehabilitation after being discharged from the hospital. Long story short... my mother was fine when she was discharged from the hospital and then started declining once she was at Coral Trace. I visited her one morning and found her with no sheets and unresponsive. I went to the nurse and she told me that she had been unresponsive "for about an hour". I told her to call 911 to get her to the hospital (which should have been done in the first place). The nurse then had the CNA clean her up and gave her sheets before EMS arrived. My mother had ulcers all on her backside and front pubic area from not being cleaned properly and had sepsis from an infection from the Foley catheter that Coral Trace had put in. My mother had to be intubated and admitted into the ICU. God only knows what would have happened if I didn't get there when I did. The CNAs could care less about their patients, the LPNs seem like they're only there to hand out meds, and the doctors should lose their licenses. DO NOT PUT ANYONE YOU LOVE HERE!

For the last year, my mother has been fighting with this facility to actually CARE for my grandmother and now it's time to move her. My 89 year old grandmother has dementia. About 2 years ago, she fell and broke her hip. Now, she's wheelchair bound and she doesn't even recognize herself in a mirror. The people at Coral Trace don't care at all! When my mom first brought my grandmother to Coral Trace, her clothes would get lost and what clothes she did have were wet with urine. Now, my grandmother has a horrible, painful rash that hasn't been diagnosed for several weeks. Coral Trace blames my mom since she does my grandmother's laundry! Well, it's been several weeks and my grandmother has been seen by local private physician, but the rash is getting worse. It's developed from a patch of red bumps into an all over rash with some blisters. If my mom weren't there to intervene, would the staff at Coral Trace just let my grandmother suffer in silence? Avoid this place like the plague!

My dad was in Coral Trace in Cape Coral. The people there were fabulous and really nice to us. It was a long road with my dad with 14 years of Alzheimer’s, and by the time he was actually put there, he was pretty well on the way out, and they could not have been nicer. They really tried to get some sort of improvement, but it wasn’t really going to happen. He was still somewhat mobile, so they had to put him in a locked down area. It really was a very nice group of people. I wasn’t as impressed with their medication regiment. He had a roommate, and the room was fine and seemed relatively clean. I wasn’t that thrilled with a lot of the stuff that I found over the 14 years with my dad, but I think that the people who worked there did the best they could. And given what they’re dealing with every day, I was very grateful they were there. It was definitely not a Ritz Carlton, but it wasn’t a bad place, and I didn’t feel like we were just shoving him somewhere horrible. It was OK. Like I said, the people there really cared and did the best they could.

I was admitted after a debilitating injury. The filth on the TV remote was so bad I refused to touch it. I watched and listened to staff yelling and screaming at a woman -roommate-who obviously had cognitive issues. I was bitten alive by some kind of bugs. My back was a mess after one night. Shortly after reporting this I noticed maintainance folks walking around spraying rooms after telling patients to sit in the hallway. I was appalled! An hurricane alert drill occurred and it seemed as though no one knew what to do. There was no communication to me about what was happening or why. Mind you, my issue was fracture related, I was not suffering from dementia and would have understood had someone explained what was going on I discharged myself after a day and would never, ever, recommend this place to my worst enemy..

Coral Trace Health Care was clean and everybody was nice. I didn't have a problem with any of them. My mother was there for a while, and it was nice, but I didn't feel comfortable with her being there because she wasn't well. I'm not blaming them; she needed more help than they could give her. The food was good, and they had movies, bingo, and a beading class where you could make a bracelet. She was happy there. It was a wonderful place.

Coral Trace Health Care is amazing facility with great leaders who are so accommodating. The administrator and the director of nursing and other leaders along with the staff goes far and beyond their job role. Some people made negative comment about this facility but for my experience, what a lovely group of people, smiley faces, helpful and so accommodating. I will definitely recommend this facility for skilled rehab and long term care.

I found Coral Trace Helath Care Facility a nice facility with pleasant staff, always smiling. They have several new faces and a lot of positive comments that the facility's new administration and management leaders are very accommodating, people person and working together. I found this facility great and will recommend to any one. the staff and leaders go way anew beyond their job role.

My mother found out through social media that her mother had passed away. Although she had been asked to be notified and she was assured she would be, no phone call was ever made. The director would not return my messages, and the two receptionists I spoke with were extremely rude. What a nightmare to find out a loved one passed away through Facebook, and there was no apology for it. We are still waiting on a phone call to find out how she passed and what her last moments were like. We live in Indiana so we relied on this health care facility to keep our side of the family informed. I would NEVER recommend this nursing home to anyone as a result of this experience.

will start with the rehabilation portion of this facility was good and they cared and did their jobs well, now here is the bad part. If you can not be there to check on the care for your loved one WATCH OUT. Their is a Dr Nassiff that prescribes meds as he pleases against the health surrogates wishes. They drug patients and put them in the hallway if they have memory health issues, I gues this is how they treat dementia. PATHECTIC Ring the bell for help and it could be 5 minutes, 15 minutes or no one comes at all. Seating when you come to visit is non-existent. Wound care is nonexistent along with just plain being rough to the Elderly. This is a POOR POOR FACILITY

My father has been in Coral Trace nursing home for two weeks now. It seems to be a nice facility. The location, for us, is very close. It is only 3 to 4 miles or so for me and my sister-in-law. They seem to be nice in their caring also. Their staff is nice and they are very helpful and informative. My sister-in-law and I go over there to visit my father everyday and there is always somebody around. Also a couple of thing that we asked about got done right away, like my father had no television in a semi-private room, but they got him a television right away. Everything seems to be going good there.
