The nurses at Central Park Healthcare and Rehabilitation Center were nice to my sister. She was watched OK, but as far as changing, I don't think they gave her the attention that she needed. The services were good. I would sign in and they were friendly. The ones that came to serve my sister's tray were also nice. The facility looked clean. She was sharing a room with another resident. It was clean and she was able to talk with the other resident. When I was there, she would eat everything. So she liked everything about the food. They had a room where you could go through to this little patio in the back, but it was just limited. They had refreshments in that room, too.

Thank you & with heartfelt gratitude to the hard working staff at Central Park, with extra special prayers and appreciation to Cassie & Tonya!! My sister received excellent care & the support she had was over & beyond expectations in meeting all her daily needs, as well as kindness & support for me too!! I highly recommend this facility for your loved one's individual needs for compassionate caring & rehabilitation. All of my interactions with the staff were most professional & great attitudes; her room was always kept clean; meals were tailored to her situation; sweetest activities representative brought her items for coloring; everyone works together to help each other. Always pleasant visiting!!

Patience, kindness, and the understanding of transition makes for an amazing staff. Paula, Rebecca, Gigi, Carmen, Cheryl and Angel are just that, angels. No transition is easy and they understand this. They see, feel and breath transition, many tirelessly. I was a fish out of water when I arrived…as professionals, they knew that. I left today with new friends and a great deal of gratitude for those that brought me out of my darkness. Shout out to PT, OT…Bill, Eric (Don’t clean the chicken before you cook it), Joey and my girl-Brittany. From the dietician to the social workers, a big thank you. Clear throat, Please address the patient to RN/LPN/CNA ratio, the continuity of health professionals, and billable hours whether well-insured or not. A boat will only stay afloat with the right commanders. All the best to your staff, residents and visitors. God Bless!

I visit my mother daily at this rehab center and I am very happy with all of the doctors, nurses and staff. They are wonderful! Very Thankful !

My mother had to stay there from 10-04-20 to 02-20-21and believe me if I could of had it different I would have. Some staff act like they might have a little compassion when most of them really could care less. I could never get updates like I had ask to get. Don't think that they exercised, nor bathed my mother like they should have. You could call and the phone would ring all the time with no answer. My mom's room phone didn't always work. I wanted to wash my mom's clothes and they said because of COVID19 that they couldn't allow that. So I sent clothes and of course my mother was moved finally thank god but she didn't have any of her clothes. I could hear my mother ask for something and they would just ignore her or say okay and never come back. My mother has dementia and they use that by saying things like no she didn't ask for it or they did something and she just didn't remember. They moved her 3 different times and really confused her more. I would say really look around for another place, not recommended.

Most of the staff are overworked and just don't seem to care for the patients anymore. The Staff in charge do not listen to concerns or they are quick to dismiss and say that could have not happened. If asked to do better their answer is you can always go somewhere else, Really ? In the middle of a pandemic? The AC was allowed to stay broken without putting even a fan in their room! In fairness that cant do any activities right now, but they don't even bother to help patients out of their beds for days at a time. They just let them lay. They let the patient sit in diaper for over 26 hours because CNA too busy to notice and the Nurse in charge thought she was too good to change a diaper and said the CNA had to do it! Really? This is just so sad. Please dont put your love one here!

My mother spent the last month of her life declining by the day at this facility. The day to day nurse, CNA, worker interaction I would rate as FAIR...however...I would not leave my worst enemy in this establishment overnight after all of the medical blunders we encountered. Unfortunately, they were our ONLY choice. My mother was left undressed in the middle of the night for over an hour during a bathing shift. Her oxygen was left turned off twice by workers taking her to and from therapy. THE ABSOLUTE WORST TREATMENT WE RECEIVED WAS BY HER ASSIGNED DOCTOR! He came once a week, unscheduled. He never contacted us about her care. He argued with my father and I as my mom declined GREATLY that she was doing ok and nothing needed changed. My mom was UNABLE to ask for help and we stayed with her for EVERY waking minute, yet overnight they would withhold meds and start her decline for the next day because she didn't ask for her assigned medication....which she was UNABLE to do. My heart aches for EVERY resident in this home as I know the medical care is SEVERLY lacking!

My Mom got placed here for a few health reasons. I read previous reviews and was very worried about her staying here. I visited to do a little research, met with some admin staff who were very understanding and patient with my questions. After checking the grounds out a bit and getting some pertinent questions answered, my fears were allayed. My Mom stayed there for over a week, and was treated VERY well, her medical issues were handled very professionally, they were very kind and attentive to her needs as well. I visited her practically every day to bring her clothes and goodies from home. Everyone I talked to, every admin made sure to give me their card just in case I had ANY issues. VERY happy with my Mom's treatment and recovery here. Many, many sincere thanks to all the staff who helped make my Mom's stay there comfortable and productive. You guys saved her life. THANK YOU! They should not be in business. They left a patient in his own filth for at least 24 hours. We complained, they said it would not happen again. The next day it was even worse. He had a bowel movement & was left in it for god knows how long - the urine odor smelled like ammonia. This gent requires bolus tube feedings - how can it be that a nurse (who's the only one who can do these feedings) didn't see the condition this poor gent was in. When questioned as to when he was receiving these bolus feedings - the answer was "we know his daughter left us his schedule but we have our own schedule". The first day we found him like this both the nurse on duty & caseworker said "This will not happen again". Well it did!!! One of the CNA's said "I'm so glad you are going to complain - something should be done. They should be investigated by the state. NOONE should be treated like this.

My mother has been in Central Park Healthcare and Rehab less than 24 hours and it's been a complete nightmare. Staff is combative giving you dirty looks and argumentative posture and response to the families concerns. I've already started the process to find another nursing home. What a joke. I cannot believe our sociality and government allows this type of treatment of our elderly. Shameful, simply shameful.

My grandmother was there after she fell and broke her hip, we had to stay on top of the staff to give her medicine and to change her. She was unable to leave the bed so had to wear adult diapers, they would leave them soiled, and wet, she would call them and the would come turn her light off and say they would be right back, and not come back for hours. Nana found one of the NURSES sleeping in the chair in room in the middle of the night. We signed her out early thank goodness we did, she had a diaper rash from hell, so bad that in a few more days she would have became open sores, had to visit her doctor to get an antibiotic cream. This facility is very unorganized, Their was no Bio-hazard bags in her room. You have to put your name on all items, or you will never get them back.. They also wanted her to get up and walk around her room unassisted, She is 84 with bad knees, and now a replaced hip. She told me that they were crazy if they think she is going to walk all by herself., with no help. She did have a couple of NURSES that were VERY CARING and nice but the rest were mean and didn't care, also I saw a lot of them just talking and acting like they were busy, I would hate to think what would have happen if Nana's family would not have spent time with her and check on her, Their is a lot of folks who have no family or limited, I hate to think of the care that they receiving since they don't have someone to stick up for them. If your loved needs rehab PLEASE AVOID THIS PLACE,.

My mother went to Central Park Healthcare and Rehabilitation Center when she was ill. They had her in a second story room that was very bare except for a bed, a dresser, and a bathroom. There was no TV. The place doesn't look clean, it's dingy, the lighting is horrible, people are sitting around, and I think there isn't enough nursing staff. Some of them act very caring while some of them acted like they didn't want to be there. When my mom came out of there, she went back to the hospital because she had a blood sugar of 29 and was comatose. I would not send an animal there.

Went in for shoulder rehab, had surgery 4 days prior. IF YOU LOVE YOUR LOVED ONE THAT YOU ARE CONSIDERING USING CENTRAL PARK ---THINK TWICE, THEY WITHHELD MY MEDICINE, WOULD NOT GIVE ME WATER NOR HELP ME OUT OF BED. I WALKED OUT WITHIN 4 HOURS OF THIS HORRIBLE PLACE. I honestly witnessed dirty bathrooms, screaming, begging patients, patients begging for their meds, open sores on patients, etc. Patients roaming the halls after midnight, I am just glad I had my right mind to get out of this facility. I was scared for my life. Thank goodness my husband came to get me and that I had my cell phone with me to call him.

While we found one or two staff members who were attentive and cared for our Dad while he was there, overall the experience was a nightmare! They did not answer call lights, some of the staff were close to verbally abusive at times. Our dad was injured several times due to the staffs inability to transfer him from the bed to the wheelchair or chair without incident. (not just bumps or scratches-significant injury) If the family had not been there to hunt down staff members when something was wrong, we are not sure any care would have been given. Even the doctors suggested we transfer out of there if we had the chance. The Rehab staff did a fair job when they did work with dad, but they very quickly decided he was not making fast enough progress and stopped the therapy. And btw, the FIRST day we had dad in his new rehab facility- they had him up on the side of the bed and transferring to a chair with help- and with therapy was able to walk with a walker--something this rehab department said would never happen and he was unable to do ever again. They were willing to just let him lay in his bed 24/7. Please find a different facility for your family member!! If I could have chosen zero stars I would have!

This place is horrible Call lights not answered. My mom was told she had to have a diaper on in case they didnt make it back to answer the light. Asking Nurses questions they act like you are bothing them. My was very sick and the nurse said we had to wait on Dr. approval when My mom was obviously in distress . I called 911 from her room, the nurse was to busy filling out paperwork to talk to EMS. She was sent to closest hospital because of her breathing . She was very critical . Super low blood pressure and heart rate. How did these nurses or Dr's not know how sick she was? She almost died. She was only their for physical therapy. Beware please when placing a loved here , these are human beings who deserve to be treated with respect and their needs met in a timely manner.

Mom is now at Central Park Healthcare and Rehab. The facility is very nice and very comfortable. The rooms were nice, and they try to pair the residents who work well with who. When she was in the rehab section of it, she had bonded with the nursing and the staff. They offer different kinds of activities. The staff is very maintained, polite, courteous, and good to my mother. They are also very on top of the needs for the patients. I haven’t seen any mistreatment, and we are happy with them.

As I do not have a huge amount of experience with nursing homes or long-term care facilities I would say Central Park is a mediocre to good facility. I have had some problems with the physical therapy staff, some problems with the dietary procedures (my mother has adult failure to thrive), and some problems with some attitudes of the staff (not treating these residents with respect when they , the cna, is really busy, or not treating the residents like people but like a 'job'.
