The most notable feature of the facility was the general lack of attention to appearance. It smelled filthy, like a mixture between feces and moth balls. The floors were evidently dirty but the lack of consistent lighting made the whole place very dingy. There was little to no privacy provided to the residents. Each resident was provided with a twin bed and a few drawers in a dresser. There appeared to be no private place for changing clothes and no way to have any alone time. Some rooms had up to 6 residents in each room. The staff seemed inattentive and uninterested in the residents. They were busy reading or with tasks that didn't involve interacting with a resident. I did not see a single staff member speak kindly to a resident. There was one woman in the back room who appeared to be suffering a number of signs of neglect (rocking, repetitive vocalizations, and self soothing). Upon entering one residents room I found him laying on his bare mattress (there were no sheets on the beds of his room mate either. When I inquired why there were no sheets the staff said that one of the residents had urinated on the sheets. The same room also had no light in the room at all. The window was blocked with what appeared to be cardboard, there was a fan that had no working light, and a lamp that had no light either. The staff said they were "working on fixing it". The staff said their reasoning for removing the bulb from the lamp was that the residents kept breaking the lamp, though had the ceiling fan been working properly there would have been no need for concern.
