The rooms are outdated and grim. Staff behaved like bullies, and not knowledgeable of social services. Unprofessional and arrogant.

Even though our family researched the quality of College Oak Nursing and Rehabilitation Center, which was rated five out of five stars, I strongly recommend that others do not place their loved ones in this facility. Granted, the staff is friendly; however, my mom was left unattended too often, denied phone privileges, on occasion, to call family, and was denied reasonable care. As a result, my mother nearly succumbed from life-threatening sepsis and thrush of the mouth and still remains in the hospital due to negligence. The doctor on call never properly diagnosed or simply didn't notice that my mom had severe sores on her legs and severe thrush in her mouth while nurses never commented on the severity of her condition. In addition, other patients are left unattended in their wheelchairs in the hallways, which is unconscionable. Under no circumstances should family consider College Oak Nursing and Rehabilitation Center a safe, conducive environment for medical and emotional support.

My Mom has been in College Oak Nursing and Rehab for approximately six weeks. Upon entering the facility to visit I was walking down the hall way and the Nursing Director ask me if I need any help, I explained to her that I was visiting and wanted to get a tour of their facility. Even though she was on her way to complete another project she stopped what she was doing to show me through the facility. When we were done she took me up to the admitting office to see the Admitting Manager and she was not there, so she started pulling all my paperwork together and started going over our paper work until the Admitting Manager arrived. She definitely went over and above to make sure I was taken care of. The transition from the hospital to the facility went very smoothly the Business Manager and Admitting Manger spent as much time as I need to help start and go through the process. . Two weeks later we had a care giving meeting for my Mom regarding her status; all members of the team express their thoughts and concerns. In addition, they all took the time to not only listen to my questions and concerns but my Moms as well. They have done a very good job taken care of my Mom. Our only complaint is that parking is limited.
