My Sister gets UTI from poor care of her catheter and her mental status is effected and she become upset. Now they are kicking her out because they say she is mental unfit for there facility. If she had better care this would not happen. What place kicks you out because of there bad care.

1. Day staff was responsive to patient needs. 2. Night staff was unresponsive to toileting and medication needs of bedridden patients. Waiting for over an hour for pain meds and toileting assistance 3. Bedridden patients do not have access to any activities, library or computer. 4. No transition to home services were provided over one week post discharge. Medical equipment needed for daily living came 5 days after discharge. No caregiver training given. 5. PT was done in the room and hallway not in a state of the art gym as advertised. 6. Many patients never left their room for the 3 wks my mother was a patient there. 7. Director of Nursing and Transition coordinator did not return calls. 8. Roaches were present in the clothes closet and drawers. 9. Food was terrible 10. Mental health of patients was not addressed

Been here nearly a week. TV hasn't worked right yet. Staff is too busy to interact with us. Right now I've been waiting 5 hours for pain medication. If you truly care about your loved ones, don't send them here.

this was the worst place i have ever heard of. The food fed to pigs was better than this. Waiting for meds is unheard of. This facility should be condemed. Short of staff is no excuse that a patient has to ask for meds several times them told that they do not carry that medication. ALL I CAN SAY IS DO NOT USE THIS PLACE. i was very up set my family member left this place on her iwn after speaking with her primary drs office

The staff is friendly and take good care of the needs of the patients. The short-term rooms are as spacious as a shared hospital room, but the long-term rooms are much smaller. I don't know if this happens everywhere, but my mom is less aware and things go missing from her room. Blankets, jackets, books, reading glasses, stuffed animals, etc. Books stay if we write her name in sharpie on all three sides and on the feet of stuffed animals. It's frustrating to keep buying her blankets and jackets and having gifts from friends stolen. They are often short-staffed, but they stay cheerful and still give excellent care. I love the people, just not the disappearing items. There are a lot of activities during the day that residents can do, like movies, bingo, crafts, and sometimes there is an outing or a shop will come to the center. People came to cut hair and do nails for a small fee, like $10 for a haircut. The staff also takes bedridden patients into the hallway right in their beds so they get to socialize. Groups of residence gather in the hallway to talk. It's a friendly place.

My wife is in Palm Valley Rehab & Care for rehab. It's excellent. They have activities, like going over to Dairy Queen to have some hamburgers. They play bingo, they have contests, and they do their nails. The place is very clean and the nursing staff is excellent. They really take care of the residents, they know who the patients are and the relatives that come to visit. You've got to check in before you go down the ward because it's a locked ward. I'm really happy with them. The food is institutional, and mass produced. It won't be like fancy restaurant type of food but it's still good. They physical appearance of the community is very good. When I'm out there the activities that I see are good. I also think that the value for the money is pretty good.

My mom was at PV to recover from hip fracture. The first visit there right after admission from hospital we went in to a room that was a mess, mattress from adjoining bed was on floor and dirty linen was all over the room, and the smell was bad. All the time we were there, for over an hour, no one came in to check on her or clean up the room. Although I waited for well over a half hour and made several calls inquiring about her personal belongings, they have yet to be returned, While a patient she became dehydrated so severely, had severe UTI, and had to be re-admitted to the hospital as the distress caused chest pain. No one returns calls. I would not recommend PV to anyone.

Therapist were fantastic, CNA were good, nurses poor are delivering meds (I ssuspect pain pills were stolen) one nurse in particular was often 5 hours late at delivering meds scheduled every 4 hours and he usually only gave me one pill instead of the two needed. Secretary was worst I've ever seen, she omessed up two transportation appointments causing 2 weeks delay in very important doctors visits and ultimately caused me to be discharged when I could have stayed to continue recieve much needed rehab and not the hardship my wife and I now must endore. I loved some of the staff hated administration special boots to help me deal with pressure points on my feet were ordered at least twice but within the 2 months time I waited and suffered constantly painful days.they never arrived.the next facility Cobalt Rehabilitation Hospital provided something within 30minutes.

This place rated 5 out of 5 stars from Medicare, how they got this rating is beyond me. My husband was there for rehab for a knee replacement. Most of the residents are elderly and I guess they don't or are afraid to say anything, my husband is 47 years old, and he certainly said something. This place has to be the worse I have ever seen and I have had parents stay in facilities like this one. They are extremely understaffed, the doctor there, which there is only one, is unprofessional and doesn't seem to care. My husband pulled a staple from the physical therapy people pushing him too soon and his knee kept bleeding, the doctor said it was old blood or an infection, and tried to keep him for another week, all they did was keep putting pressure on his knee, which didn't stop the bleeding, he bleed one night from 7pm until I told him back to the surgeon at 3pm the next day. The doctor at this facility couldn't even tell that. My husband had to have 4 more sutures put in. They promised him he could go home and then changed their minds and tried to get him for another week, stating he needed more physical therapy, which was a crock. No one ever iced his knee or put him on his CPM machine to get his range of motion going, pain meds were always late, the nurses seemed nice enough, but they don't say anything I'm sure in fear of losing their jobs. Do not put your loved one at this place. They may not leave! I had to break my out.

My mother-in-law has been in Palm Valley Rehab & Care Center since 2004. Her diagnosis is Dementia. She is in The Veranda Unit for alzheimers' and dementia patients. She has always received excellent care. The nurses and aides all know her and call her by name and are very attentive. At first sign of illness the staff is always on top of things. Calling the doctor and getting orders for treatment. The one problem I have with this facility is that the cleaning staff doesn't seem to check for misplaced personal items (i.e., glasses, hearing aides, clothing) My mother-in-law has thorwn articles of clothing in the trash or her glasses getting lost in he bed lnens and they are never retrieved. I am constantly having to buy more clothing for her and replacing her eyeglasses. Overall, Palm Valley is an excellent facility. Clean, caring and couteous staff. I feel like my mother-in-law is in a good place.
