About This Community

We offer 24 hour nursing on-site, insulin injections, physical therapy, occupational therapy and speech therapy using a home health company. We also offer activities 7 days a week in each neighborhood based upon ones cognitive level, setting our resident up for success one small victory at a time. We also offer off campus activities such as Butterfly Garden, Breakfast club, lunch club, dessert club and so much more.

We offer a safe secured environment, with outdoor courtyards that are surrounded by the building with no exit, allowing our resident to take in the great Arizona sun and fresh air while still being safe and secure. Medication management, assistance with all active daily living skills, home cooked meals prepared daily by our own chef.

My dad is a resident of Avenir Memory Care at Surprise. The rooms are spacious, very nice, and clean. The staff is friendly, and they take good care of my dad. The food is average. The dining area is clean and has a modern look. They have different games, and they invite musical groups in. They have story time and game night, and they do Wii bowling. They also have a van where they can take them out in the community, either out for lunch or you can sign up to do some of those activities. They have a courtyard that has flowers and a place to sit out there.

I went to Surprise Memory Care. It looked very nice, it is more expensive, but it looked very adequate for what I'm looking for. I'm also looking at VA-supported places that the VA will pay for. They looked very capable of handling what my husband's issues are. The staff was very supportive and well-informed. I was taken on a tour, so I didn't witness too much interaction, but it was very thorough, and they answered all my questions in a very thoughtful manner. I was very impressed. I saw both shared rooms and private rooms. They're very nice. It's well thought out, and as cheerful as you can get under all the circumstances, it was a good environment for people who are suffering from these diseases. Certainly, they're locked down, nobody's going to get out, so that's good. The facility was very nicely lit and clean, and the artwork was exceedingly cheerful.

My loved one was a resident of Avenir Surprise Memory Care for a few years. The turnover for caregivers was high. There were some who I questioned why they were caregivers due to impatience and lack of common sense. I did not know of any dementia training. The bathroom needed cleaning most times. I saw only one housekeeper for the entire facility. Many times I would see a resident sitting at the dining room table for hours without being tended to. When this facility first opened, it was very good for the first two years. It has steadily gone downhill, probably due to lack of accountability and compassion and the emphasis on profit.

I went to Surprise Memory Care yesterday. It was wonderful, very pleasant, very quiet, and had a happy atmosphere. The staff was wonderful. They weren't being like salespeople, they were just being themselves. They were just kind people and you can tell they love their jobs. They gave me banana bread to-go. It was home-like, very clean, just quiet and relaxing. The outdoors was very nice, my husband would enjoy going there. It was open, but it didn't feel too spacious, it felt more like a home. I was pretty impressed by them. They offer television and Cox cable.

My mom has been at Avenir at Surprise for four years now. It has started out good. We couldn't visit her for a year because of Covid and when we went in and saw her room, we were not happy with the condition of her room and her clothes. They have so many staffing changes, so it is impossible to keep track of who's who. They have a different nurse every time and a lot of times the caregivers don't wear their nametags, so you don't know who's who. For instance, my mom was trying to get out of the door and the caregiver who was sitting on the rear end in the corner didn't get up to help her get out of those locked doors. They haven't really changed their activities. My mom is up to the point where she doesn't need to play bingo or do any of that stuff. She likes to sit outside but she was unattended outside on several occasions at Avenir. They also had several outbreaks of Covid and they had a hard time getting it under control. Their services have gone down drastically. They have a nice courtyard and her room was nice, but when we got back in there, we weren't happy with the condition it was in.

Surprise Memory Care was a fine community. The rooms were nice. The staff seemed to be interacting with the residents and there's plenty of them. The activities director was doing a lot and the activities were really good.

Surprise Memory Care was wonderful. It was a happy, peaceful atmosphere. The staff member was great. You could tell she really cared about the patients. Residents knew her by name and she knew most of them by name, so that was very impressive. The thing I liked the most is that they separate residents based on positive abilities into different "neighborhoods." I think that's why everyone looked happier and calmer. The food also looked very good. They did a lot of musical activities. I also noticed a lot of people were doing arts and coloring.

Surprise Memory Care was brand new and lovely; it just opened maybe 2 or 3 months ago. I went for a meeting after it opened, and it’s extremely noisy. They have about 25 residents, but there was no insulation. It was very noisy; I can’t imagine that would be comforting to the residents. This is something that they probably will address because the marketing person did mention how noisy it was. But the rooms were awesome; they were attractively furnished. Every employee that walked through or anywhere around when I went back the second time said hello, and it looked like they had enough staff for the number of residents that they had. That certainly was a possibility for memory care.

Surprise Memory Care is good, and we put my mother in. It’s unfinished as it’s new, but the inside is finished with a very open floor plan. There’s lot of room to roam around and a lot of access to TVs and activities. The staff is top notch -- very good hire and practices. I would recommend them.

My mother is in Surprise. I chose it because they seemed to be able to manage my mother?s level of dementia without having to move her yet again. So far, it's been great.
