This place is a dump. The rooms are dirty, paint and plaster falling off the walls. The cleaning people don't know what cleaning means. The staff does not care about the patents. My loved one was in a dirty bed for 2 days, no one changed the sheets or pillow cases. When my loved one asked for them to dial the phone for them the staff told them I will be right back, left , closed the door and never came back. If your loved one has a stroke don't send them there. When bringing in the food trays, they put it off to the side and leave the room. My loved one lost over 20 pounds in 2 weeks because of not being to get the food tray and when someone finally came in the food would be ice cold so they wouldn't eat it. Not once was my loved one taken out of the room for any activities except the so called therapy. When I tried calling anytime after 5pm the phone goes unanswered. I had talked to the head of the place and was told they would correct this problem but it was never taken care of. so save your loved one a heartache, don't put them there. If I could give zero stars I would.

Stop! Stop! Stop! Please do not take anyone you care about to this facility I had to have 911 take my beloved Aunt back to the hospital this evening because this establishment did not respond to her needs for diabetic management, they allowed her glucose to get up to 400. She is weak and had recently fallen and had been pressing the call button for over 2 hrs. I called 10 times and no one answered the phone. I went there in person and rang the bell for over 10 mins and no one came. I looked all around the building and only seen patients in beds, no employees. Finally I was let in. My aunt was in the same spot I left her 3 hrs prior. she can not walk by herself and could not toilet herself. This has been a very horrific experience. It's a crying shame!!

If I could give zero stars I would. Husband was admitted on Friday before labor day and nothing was done over the weekend and everyone I spoke with knew nothing. My husband was moved to a room closer to the nursing station to keep a closer eye on him (shouldn't they be keeping a close eye on everyone?) but clearly that is not the case. When I asked two nurses why my husband was confused and disoriented and agitated the first nurse was snarky and the second one, Ramon his actual nurse, had no idea. He finally agreed to look at the lab results and gave me an answer. He also did not get my husband to dialysis on time, which is why he was disoriented and confused. I do not feel like this facility has the patient's well being in mind. Food is nasty and the room is really gross. Floor is so sticky. Find a different place.

Very unorganized, never saw caseworker unless I tracked her down. Never saw or heard directly from nurse practitioner only staff called to update. I had to wait four days to finally get a phone call from Dr. Qamar and he was very abrupt, abrasive and not helpful at all. My father's room was filthy, toilet in his room was not working. I would not recommend anyone ever go there for care.

Run while u can . This place is like an armpit without deodorant. NOONE knows what thier doing one person say this the other days something different. My father at she 82 broke his hip the gave him a gracious 7 day stay what a joke. I spoke with a person in chg supposibly who said oh no it will never be that short of a stay 7 days later he's being discharged. What a joke. NOONE ever properly evaluated my father he saw 2 doctor who didn't even examine his surgical area but he's good to go . I was never contacted by anyone except to get insurance info. The old mighty $$$. I wouldn't recommend this place to my worst enemy. Oh and of course everything that happened was the insurance company s fault never them Very very disgusted

My poor 82 year old Mom was in this place for 17 days. Problems started on day one! They couldn't get her Heart and blood pressure meds for 3 days, and wouldn't let me bring them from her home. She ended up being in AFIB for 2 days! She had been in the hospital for pneumonia for 9 days, before going to Desert Blossom. She only got 1 bed bath after being there for 8 days, and that was only because I kept calling to complain! I also had to call to complain because her TV didn't work, her roommate was yelling and crying all night! It takes over an hour for anyone to come when she hit her call light! No choices with the food. She is allergic to fish , we told them on day 1, and yet fish was server to her 3 times! She came home with bed sores, a UTI, and a yeast infection! Horrible Care!

We were promised we could have in room visits as my wife had received both of her COVID-19 shots. After my wife got her room, we were refused entry as her room mate was in quarantine.. For all we know, that person could be COVID positive. We asked she be put in a room with someone who received their COVID-19 shots, That request was refused. Other problems arose with her medications not being given, There was no response to a call light for 4 hours and my wife cannot walk. If you can to talk to someone to ask a question, they never come back with an answer. Please find someplace else for you loved one.

I was a patient here for 4 weeks and they really helped me to walk and help me with my oxygen and walking .and now I'm going home and I appreciate all of there help. The staff is very helpful and care about there patient and I'm going happy. I'm grateful for all of them and staff is wonderful hard working people in the world. The rooms are very bigand your with a roommates.

Very poor. Cold for 5 days, cold food & not always appealing. Your call button may get help in 1 hour. Staff not attentive as they should. Extremly disappointing & 2 staff members lied to say they had 2 notes from my doctor saying I was to bear weight on my leg which was not true. My doctor gave me a paper to give them stating "no weight" . I still not allowed to put weight on it.

This place I woulded recumend to no one I was here for six weeks the food is so bad my dog wouldn't eat it .the place is always ditiy.the staff needs alot more training on medication I can't believe the place is in bissnuss .there is some good people on the staff.

This is the worst rehab facility to take your love to. The food was not made with love and care. After being release from hospital to rehab stayed in gown for 2 days without any hiygene care.

Worst place to send a loved one. They kept my mom drugged, did NO rehab with her, would not return calls, kept all ofher personal belongings. Mom came home with horrific bed sores that no one told us about. This facility should be closed, do not take anyone here!!!

Desert Blossom is a complete disappointment. Their Medicare rating doesn't reflect the poor quality of the facility. Please be wary if you are considering sending a loved one here, and be very prepared to be a very vocal advocate. Pros: -The nursing staff and aides are good overall, most are very friendly and helpful. One gentleman in particular (I believe he is an aide) always goes above and beyond. -Physical therapy seems to be good with a nice staff. -Good location with the fire department immediately next door. This seemed like a plus in the event of emergency. Cons: -They recently changed owners. Every problem across the board (and there have been MANY), will be blamed on the ownership change. -It's virtually impossible to reach the administration. You will have to be patient and persistent, they do not make themselves available to family once you're a resident. Expect to get the run around, expect to not receive return phone calls. -The social worker (I'm only aware of one on staff) is unpleasant and unhelpful, also very difficult to track down. Provided none of the assistance you would typically expect from a social worker. Only has talked with the family begrudgingly. I've resorted to using an outside social worker for assistance. Terrible! -Medications will be administered late. Some medicines have to be administered in a certain time frame, In our case 10 minutes before/after a meal. My loved one was typically receiving this med 1-2 hours after the fact. Our physician was furious when this came to light, as it rendered the medication essentially useless for the intended effect. -Medically restricted diets WILL NOT be honored without raising a fuss. I was baffled by this since it was quite clearly medically directed by the physician, yet completely ignored. -Patients are bathed on a set schedule, twice weekly with no sponge down or anything in between. Sometimes even twice weekly doesn't happen. There have been many times I've walked in and the smell of body odor in the halls is overpowering. -Oral care is usually taken quite seriously at other facilities we've used. No attention to it here at all aside from stashing toothbrush/paste in a nightstand drawer once I asked about it. -While the nursing staff is friendly, there is complete confusion when a different nurse comes on staff. -Communication is a big problem here, both between health care workers, and between health care workers and family. If you are visiting and need to ask the RN a question, expect to wait 30-45 minutes before anyone comes to the room. This has been consistently true. -The semi-private rooms are a little small if you have a roommate. -Small assortment of activities. Mostly expect to sit in your room and watch tv. -The parking lot is very dark at night. Seems trivial, but on more than one occasion there were neighborhood residents (not associated with Desert Blossom) sitting in the lot in the dark near the cars at night. Any time I've managed to talk to someone in administration I am reminder that we are free to leave and seek care elsewhere. They are always quick to point out that "there are far worse facilities." If your management team deals with problems by telling the family "yeah, but it could be worse," that's NOT a ringing endorsement for your facility. We did not transfer because of insurance, but if I could make the choice again, I wouldn't put anyone I cared about here. I do not recommend Desert Blossom.

The staff is there to help you at all time! Therapy was great. I used be against therapy but those guys changed my mind. They do stuff to help you and not wear you out. The food is very good. There were a couple of things I didn't care for but you can't win them all.

My mom just came out at Desert Blossom. The staff was very good, nice, and kind. She didn’t get as much rehab as she should because when she got put in there directly from the hospital, she had a kidney infection. She was bedridden, but the staff was very helpful in taking care of my mom.

The staff was very good, helpful, and dependable to my wife. Desert Bloom provided everything that she needed to recover. She was in a room by herself, and it was nice. It was functional for what it was needed for. The dining room seemed quite nice, and they have quite a variety of meals, that seemed to be well-prepared and tasty.
