Many staff were impolite to my family member; often said they would respond to a request and never returned to the room. Med was missed and incorrectly documented. Other OTC meds prescribed by outside dr were very inconsistently given, or not given at all. Dietary requests via menu were often wrong or missing. Gait belt rarely used, even after a fall. Kindest people were administrators. Direct care staff left much to be desired.

My family’s experience was terrible. For the amount of money you are charged the service is unacceptable. Not enough staff to respectfully care for every resident. Most time one nurse and one attendant handling an entire wing or two. Not acceptable for staff or residents.
Administration more concerned about $$ and trying to get as much as they can out of you. Will find anything to charge you for.
Amenities are old and outdated, equipment raggedy. Some linens stained and dingy. White clothes sent to the laundry came back pink.
If you do not have an advocate or someone able to check on your loved ones regularly , God only knows what could happen to them. My family member fell twice, thankfully unharmed. Had an injury once on arm, took 3 days to get the X-ray right and still found nothing. Arm swollen, hand swollen and warm. To no avail they found nothing and had no idea.
There are nurses and CNA’s that care about the residents but unfortunately there are some that are rude and disrespectful as well.
We were able to move our beloved family member out of there to a much better facility that takes pride, care and shows them respect and dignity.

Wanted to share that my grandma had a wonderful experience while staying at Lewis Memorial. We felt that the staff always treated her with dignity and respect. Her room was spacious and clean. Our entire family appreciated the care that was provided at Lewis Memorial and would recommend this facility to others.

My grandmother was at Lewis Memorial and I couldn’t be happier with the experience. She had zero complaints during her stay. The room size was great and everything always looked very clean and well kept. The staff was very helpful and always had a smile on their face which goes a very long way. I would highly recommend Lewis Memorial.

I am very disappointed with the cleanliness of Grace Point and the lack of cohesive nursing services. They are understaffed and need to employ more CNA's. Should have at least 2 for each hall and a floater to help where needed. The kitchen runs out of food/drinks and serves the food cold. The menu is repetitive and tasteless. The consistency between the nurses is very poor, especially when it comes to distribution of prescribed medication. They have good physical and occupational therapists and there is a handful of nurses and CNA's who are dedicated to the patients.

Looks are deceiving……This is a beautifully decorated facility that is way understaffed for the care of a loved one.
On August 6, 2018, my Grandmother was admitted to this facility for rehabilitation after she was in the hospital for 5 days after experiencing a fall that fractured her pelvis & tailbone. While in the hospital it was discovered that she had a very bad UTI & was put on antibiotics. She took her last dose of the antibiotic at Lewis Memorial’s rehabilitation facility. We requested that they retest her since we were concerned that the UTI may not be gone. They told us that they had to wait three days after the antibiotic was completed before they could take a sample. Our family was told that they took a urine sample on August 10, 2018, and results should be back by Sunday, a little longer than made sense to me. After inquiring still no results on Sunday. On Monday, August 13, 2018, we were fortunate enough to be able to remove my Grandmother & return her home with private care. We found out yesterday that a urine sample had never been taken at Lewis Memorial – I hate to say we were lied to but – we were. A visiting nurse took a urine sample yesterday & we received the results this morning that she does in fact have a bad UTI again. I feel it would be unfair to the caregivers employed at Lewis Memorial to go on about the lack of care at this facility. They are trying to take care of more patients than they can handle, doing the best they can. Shame on whoever is running this facility.
POA for Mom

Lewis' staff was quite pleasant, but their care was poor. Mom was in fairly good shape when she got there for rehab after an injury, and she was in good shape after 2 weeks (walking 400 yards). She got pneumonia there at Lewis and got severe diarrhea, neither of which problem she had prior to going there, and had to be on antibiotics several times. Lewis staff took days to respond to doctors' orders and seldom knew what had been done, or when. She was barely able to get out of bed after 2 months at Lewis (a huge regression). Property kept disappearing as well. The small rooms and the disappearance of valuable items do not justify the price.

He was at the rehab center at Lewis Memorial Christian Home. It's very nice, clean, and well kept. They might have been understaffed, but they took really good care of my father-in-law there. The staff are very friendly; anytime we needed something they are always there to help. They are very polite and kind. We absolutely have no complaints. He was in a private room and it's large enough. It had its own bath and shower, a television, a wardrobe for your clothes and all of them had windows.
