At Legend at Rivendell, we recognize that each person has individual needs and preferences. With this in mind, we offer each resident a flexible service combination we call Tailored Services. The combination of the Base Monthly Service and the Tailored Services you choose results in your monthly Tailored Pricing. This unique plan provides you with the most individual personal service in the most cost effective manner.

Base Monthly Service Includes:

Enriching Activities

Life enriching activities come alive at Legend at Rivendell. Residents participate in hobbies, cultural events, educational opportunities, religious activities, and interesting trips, along with the opportunity for new experiences. New friends are made and relationships strengthened by spending enjoyable time together. Special cultural and community events are a highlight on the Legend Life Enrichment Calendar.

The Life Enrichment Coordinator and the nurse create The Legend Wellness Program which may include fun exercises, activities, and health education. The Legend Wellness Program is designed to enhance the overall health and well-being of the residents.

Companion Services

You may choose additional personalized support or services through our specialized program call Legend Companions. Legend Companions spend time one-on-one with the resident providing special attention or care such as reading, shopping, letter writing, supervision, and support.

Reflections Alzheimer's and Memory Care

Dedication to Service

Caring for a loved one with Alzheimer's or other memory impairments is a very personal, loving dedication to service. It requires service for specific and individual needs.

More often, the resident with memory loss needs assistance with life's daily activities including personal care but may not have an acute medical condition requiring around the clock skilled nursing or instruction. Finding that unique combination of physical setting and specialized care is a journey traveled by many families.

The Reflections Program

At Reflections, our trained staff works closely with family members and friends to assemble a "Life History" identifying the personal experiences of each resident's life.

Through the "Life History," we get to know the personality behind the person, fostering a one-to-one association more like "friend to friend" rather than "staff to patient." Details and events from your loved one's past become "shared" memories and experiences serving as a foundation for activities, communication, and the development of friendships built on support, understanding and mutual respect.

The Legend at Rivendell was really clean. The staff couldn't have been nicer and kept checking up on us. They have a really structured activity program and graduated care that we really like. The fact that my mom could be at one level and then at another level as the disease progresses would be great. It is expensive. The room is a 350 square foot studio apartment, but with just a bathroom. They have a community kitchen. One thing we really liked was that they had an enclosed outdoor area where people could go when they wanted to. They go to art museums. A doctor comes in once a week, which is great. The dining facilities are really nice with linen tablecloths and napkins. They have nice fireplaces throughout the building, which we love. The staff was perfect, patient, answered all the questions, and knowledgeable. I have a friend whose mom is in there, and she was saying the meals were fantastic. I went out there and watched what they were serving for dinner the other night, and it was great.

My mom has moved into Legend at Rivendell. We chose it because it's clean, and it has a home environment. It doesn't look like a nursing home, and it doesn't smell like one. The Monday through Friday staff are very knowledgeable and very capable, but on weekends, I don't know. There's a fireplace, and there are the poster chairs. It looks like a home. It's an environment that you want to be in. The dining area is not like a cafeteria, it has a restaurant feel to it. They have tables there, and there's a dining bar; you can choose to eat with people or not. They don't feed everybody the same thing. They've had some challenges with mom, and they've handled them very well. The food is as good as it's going to get. For whatever reason, nursing homes in the state give their clients a high glycemic menu; you can't tell me that everybody in there is not diabetic. But I rate the food a 5 because no one else complains about it.

We chose Legend at Rivendell for my parents. The employees all made eye contact and talked to us. They had a very nice menu. They have regular full, nice meals and lots of options. I feel like there's more staff watching and taking care of them. It was clean and didn't smell. Everything was very up to date. They have different activities, like putting flower arrangements together. They do that weekly and have the residents put all the bouquets together. They were the only ones that had a nice room for a married couple. They had a little divider, so you have a living room and a bedroom. They had vanities, which was nice. They have a bingo, exercise, and movie night forum on Fridays with popcorn. They start the mornings off with cookies because that helps with their smell and puts them in a good mood.

We loved Legend at Rivendell and we liked how the memory care can walk the hallways. It was like a square so when they turn around they'll eventually find their room. In the middle, there's a really pretty courtyard with features. And all the staff talked to us as we were walking around, and they interacted with each other. So we like that. The staff who gave the tour was very nice, very friendly and very informative. There was even a client there that they were helping at the time, and called them by name. We talked to her twice since we left, and she was trying to get it more feasible for us. The building was very nice, well kept, had pretty landscaping, a nice parking lot, and easy access. We had to wait for somebody to unlock the door, which is what we would have expected with the memory care.

Legend at Rivendell was impressive. Out of four places that we went to, this one was the one that we thought was the best for our mother. We're specifically looking for a place that can specialize in treating Alzheimer's, and we just felt like they did the best job with that. We like the way that they have it broken up into what they specialized in, rather than just breaking it up into memory care versus assisted living. They also broke it up into 3 different phases depending on severity. My mom would be in a community with other people that were on the same stage of the journey as her. That was one of the things that we really liked. It's also a very secure facility so the patients are not in danger of wandering off. The staff does monthly training on dementia. There was just a lot that they focused on, specifically dementia; that's what we were looking for. We didn't really witness it, but they showed us some of the activities that they had done and talked about some of them. More than the specific activities though, what we were really looking for is a place where they would keep our mother active because she may not necessarily be able to participate in organized activities (like doing crafts and things) because she's a little bit farther along. The thing that impressed us the most is that they said that from the time she gets up until the time she goes to bed, they would have her doing activities and being in the community. Just the interaction with people and the physical movement is what we were most pleased about. One of the things that we were most impressed with this facility is they have a doctor on staff. Whenever she needs medical care, they will call the doctor to her instead of us having to take her out to a doctor's appointment. That was a huge advantage, especially because we live two hours away from where she's going to be living.

I visited Legend at Rivendell. The nice thing about them is that they had separate buildings. They had 3 buildings for memory care, which were separated for what level of care they require. So, there is a 1, and a 2, and 3; if the people are just barely into it, so to speak they could be in a one. They do outings all the time with them. They get them in a bus, and they go shopping, or they go sightseeing or whatever. They had activity bars. I thought the residents were vivacious and happy, and having a good time. You would think that they were a part of the assisted living, not part of a memory care area. Unfortunately, my husband, at the time I was looking for a new place, he was already a 2, and he's more like a 3 now. So, he was not going to be going out on outings anymore, and their price was more than I can afford. It's over budget, but it was a nice place. I like their activities for their residents. They have single bedrooms. They have an exercise and activity room, they had other small rooms there for puzzles and drawings and things. Few of the residents could sit around the table and do things. They have broken them down to small groups, to do things, which is a good idea. They would feel more comfortable that way. It is so nice there and it is well kept, and they don't have stains on the rug.

Both my husband and my mom are at Rivendell in Oklahoma City. I like everything about it, it's superb. I didn't really want to move them, but we're running out of money. It's just beautiful. They take really good care of my family. My sister seems to like it too. We hate to have to move my mom and my husband, but we have to. They make these places so expensive. The staff takes extra care of my husband and my mom as well. The last time I was there the food was pretty good, but it has been a while since this COVID thing. I haven't been up there to eat with them but it's generally good. They have activities, they try to keep them busy, and they play games. The service is good up there.

Legend at Rivendell was very nice. I’m a simple person and kind of a country kid, so I felt very out of place. It felt ritzy to me. It wasn’t for me. The place was very nice and very clean. The shared room was small. There was just a cabinet to divide the room. The staff member who showed me around was very accommodating. She answered all our questions and showed us pretty much everything that there was to see. The food was very good too.

Legend at Rivendell is quiet and very well kept. It's a pretty place. Everybody is as friendly as they could be and takes really good care of my wife. I ate with her once and they have good food. They give small portions, and everybody there is slim and trim. They have somebody who does their nails. The day that she started, they took them all up for ice cream to celebrate a new resident coming. They go on scenic drives, they have a piano player that comes in, they do arts and crafts with them, and they have exercises. They keep them busy all day long. We're satisfied with them. The staff is really good. It's well kept, well lighted, up-to-date, and very clean.

My brother-in-law hs been in Legend Memory Care at Rivendell since April. It is very nice. The staff is very nice, very caring, and take really good care of him. They seem to serve very good food. They have shared apartments and some private rooms and have activities all of the time. They have a TV area where they can sit and watch movies. The dining room is set up like a nice restaurant with tablecloths and China. They have several sitting areas and an outside courtroom where the residents can go sit outside if they want and still be secured.

I can't say enough good things about the group of people that for our elders. My mom has dementia and the really understand the disease and don't make quick judgments about her because they know it's the disease. They have educated us as a family and helped us help out mom. I would have given 1000 stars if they were available!
