About This Community

Louisiana-owned and operated, Sage Oak Assisted Living & Memory Care of Lake Charles was founded to fill a need in personal home care for seniors. Our boutique, luxury care homes are located on one campus at the corner of Ham Reid Rd and Weaver Rd, giving residents a home in a neighborhood, rather than in a facility disconnected from the outside world. With only 16 residents per care home, Sage Oak of Lake Charles feels like a home, not a hospital or an institution. This encourages better physical and mental health for our seniors.

A philosophy of caring guides us to provide outstanding service based on the belief that it is a community responsibility to preserve the health, safety, comfort, and general welfare of our seniors who may be suffering from loneliness and anxiety, and require special care. We are dedicated to meeting the daily needs of the seniors who need help with everyday activities that exceed what their loved ones can provide.

With a higher caregiver-to-resident ratio than a typical nursing home, we ensure attentive and specialized care to meet each resident’s specific needs.

Seniors at Sage Oak of Lake Charles live in a clean and safe environment, and enjoy a familiar and comfortable homelike setting. Sage Oak of Lake Charles is a campus of five care homes located within a residential neighborhood. Our homes have been purpose-built to include numerous safety features and modern amenities that provide our residents with security and comfort.

Sage Oak of Lake Charles Assisted Living homes will be able to provide loving care to anyone who needs a little extra attention in their day-to-day life, including chef-inspired meals. Our memory care homes will be able to provide loving care to folks suffering from dementia, Parkinson’s, Alzheimer’s and many other forms of cognitive impairment.

I toured Sage Oak Assisted Living and Memory Care of Lake Charles. We did look at the memory care, it's good, and it's like a group home. The rooms are very, very small like I probably would have to borrow a twin bed. The only complaint I had was the room size. There was no place for privacy, if your family wants to come to visit, the open areas are completely open. She wouldn't have room to accommodate chairs for visitors and things like that. It's a nice facility, the ratio of caregivers to patients is very good. They have a chef preparing meals, and the cooking is interactive. I thought that it had a lot of good things about it. The common area is one big room. On one side, it's a kitchen with a very long counter, and on the other side, there's one TV. Everybody has to watch the same thing or whatever or go sit in that room, which is extremely tiny. She would have a twin-sized bed and her chair, and that would have been that. The staff was great. They were nice. The facility is good for some people, but my mother wasn't ready for that yet. There was no one in the building that we went in for memory care. It was a building they had just finished. They have 16 rooms in each building, so it's kind of like a group home. It was a fast and furious go-through, because we got a call at the same time, and we had to meet with the doctor at the hospital.

I went to Sage Oak Assisted Living and Memory Care of Lake Charles. I visited the memory care. It appeared to be brand new, so everything was like new. They had a chef, they had nice rooms. It is very clean and it appears to be a very nice place. The lady that showed me around was very knowledgeable. I saw them preparing food. I looked and talked to the chef. I thought the set-up was very pleasant. I saw people sitting there communicating with the counselors. They're just talking, they're watching television, and they're getting ready for dinner. The facility is brand new and very clean. All the rooms were clean, and the floors looked clean. The appearance was very nice. The kitchen was clean, the chef appeared to be clean, and they were sanitized.
