HomeWell of FortBend is owned and operated by an occupational therapist and strives to
provide holistic, client focused home care aimed at meeting stated goals. All levels of care
include our comprehensive care management approach to home care. We strive to be the
liaison between clients, families and other service providers. We serve the Fort Bend County
which includes the cities of Sugar Land, Stafford, Missouri City, Meadows Place, Fulshear,
Richmond, Rosenberg and Houston. Our main office is located in Stafford.
Life Enrichment & Activities Program:
HomeWell offers the Life Enrichment & Activities Program to all of our clients, regardless of
limitations. As people age, they find one of their greatest struggles is not being too tired or
incapable of doing new things—it is never being given the opportunity! At HomeWell, we
understand that one of life's greatest privileges is the ability to try new things, to
experience adventure! Our home care agency also understands an important principle: you
are never too old to try something new.! Our thorough assessment of each client’s capabilities
allows is to create a detailed plan of suitable activities you or your loved one can enjoy. In
addition to the company that our caregivers provide, new experiences keep spirits vital and help
the healing process. When the mind lacks engagement, life loses its excitement. LEAP
eliminates the frustration of boredom through life- enriching activities.
“Go HomeWell” Program:
Studies prove post-medical discharge home care services decrease client readmissions to the
hospital.
At HomeWell, our goal is to address the drivers that contribute to those readmissions and put a
plan in place to work with the client, their family and our healthcare partners to prevent
unnecessary hospital readmissions. This is where our “Go HomeWell Post-Medical Care
Program” works very well.
Our Go HomeWell Program was designed to front load care allowing the client to taper down
services as they become more independent. Each client need is unique, so not all clients “taper
down” or graduate from HomeWell services. Some may need more up-front care initially and but
others but may need or require a high threshold of service hours throughout the duration of their
time with HomeWell.
All our caregivers are fully screened, pass a stringent national identity and background check,
have excellent references, undergo competency testing and are employed by HomeWell. We
do not sub-contract help. Our hiring standards meet or exceed the most stringent state
requirements. All levels of care include comprehensive care management which include
announced and unannounced visits to the home by our care manager to ensure continued
learning on part of our caregivers. We also provide our caregivers education opportunities.
Ongoing education and training is required for continued employment.

Activities of daily living help (bathing, grooming, dressing, medication assistances,
transfers.

Support via Go HomeWell program after discharge from hospitals or rehab units

Comprehensive case management for a continuous holistic approach

Respite care
Our Hourly Care program allows you or your loved one to manage a personal schedule,
enabling us to be available at any moment to meet your needs. We create custom plans for our
clients around the 24-hour day, adjusted according to our free assessment. Our solutions are
not only cost-effective, but provide the highest value to our clients because they provide
assistance when it is needed.
When it comes to in-home elderly care, each schedule is as unique as each client we
serve. There is no “one schedule fits all.” We provide care whenever you request it, be it
early in the morning, late at night, or 24 hours a day.

They do not do to much in the home.
They do not have consistency. We have a lot of change in the caregiver with in the week.

As she loses the ability to do things for herself and needs more help, they do everything they can to help her.
They have been very responsive when our needs have changed, and they covered shifts quickly.

As soon as she comes in, she gets to work without being told what to do, and she works up until the time that she has to leave. She is outgoing.
I like that they follow up. The worker is an excellent worker, and I think I am happier with them more than the others I have been with.
