Our geriatric care managers can consult with you in your home to help evaluate your situation, listen to your goals and needs, and develop a specific plan of care for you or your loved one.

Senior Care Managers also:

• Monitor the level of care given
• Coordinate household chores, meals, maintenance, bill paying and banking
• Assist with medication management
• Coordinate and/or accompany the client to doctors' appointments
• Assist in obtaining and coordinating home care, nursing home care, and care in retirement or extended care facilities
• Offer guidance to out-of-town family members and act as a liaison between the family, older adult, physician, attorney and others
• Support caregiving family
• Advocate and coordinate care while in hospital, nursing home, and/or rehab
• Advocate for clients' and families' needs
• Reassess and reevaluate your long-term health plan
