VNA Extended Care is designed for patients who need long-term care when a chronic condition, trauma, or illness limits their ability to carry out their own basic care needs. For many, care can be safely provided in the comfort of their homes and can include: Nursing services, Geriatric Care Management, Personal Care, Companionship,Nurse-aide, homemaker/housekeepers, meal preparation, In home respite care, medication reminders, errands/incidentals, infant and new mother care, transportation, laundry.

I am with VNA Palliative Care at this time. The nurses I have had the pleasure of working with are simply outstanding. They take into account your entire person as they work with you. They are the only ones that keep me goin'.So keep on doin' what you're doin', cuz it sure is workin' for me!
