Immediately following the assessment, our Clinical Director will make recommendation for care and seek your input. We encourage families to participation in this process of discovery and planning. Together, we will develop a proper ‘CarePlan’ based on your loved one’s unique physical, emotional and social framework. In this way, we can meet the needs of your loved one and provide you with full control of the CarePlan. Home healthcare of an individual is a fluid situation. In that, it requires constant monitoring and adjusting of the ‘CarePlan’ to meet the ever changing physical, emotional and social framework of the individual being cared for.
