Galaxy Home Care develops a written Plan of Care for each client based on an assessment of their needs. The Plan of Care is developed before providing service in cooperation and agreement with the client and family’s representative and will be used to direct the caregiver in the delivery of in-home care services. Plan of Care is reviewed with the client and family periodically to ensure that the client is receiving the appropriate level of care and is pleased with the caregiver providing the care.
