Our regional advocacy for Medicare beneficiaries and caregivers support provides navigation through all the complexities of Non-Medical Home Care, Medicare benefits, Medicaid eligibility, elder law, family case management, Caregiver Support Team, and also a Long-Distance Caregivers Team. We are a Medicare provider for non-medical services in the home, health home care, community medical care homes, diabetes care management and hospice care with dignity and compassion working together to serve our seniors and caregivers.
We strive to keep patients the ability to live safely in the comfort of their homes – even with complex medical situations – we evaluate every aspect of our clients' health — medical, functional, psychosocial, emotional and spiritual. We focus to implement care consistently and professionally that works for the family as a whole. Our clients remain at home – and out of the hospital and emergency room – because we monitor relentlessly, provide consultation with their doctors, share information with the appropriate providers, and constantly tailor care to meet their changing needs and preferences for the client and caregiver.
