Sustained Hope, LLC, is a dependable provider of exceptional and private personal home care services to the residents of Georgia. Personal passion, firsthand experience with stroke of a parent and several years of experience in neurodegenerative diseases and medical trauma are the motivations for founding Sustained Hope. Our care continuum covers a spectrum of family-centric services from discharge to recharge.
Our neuro- and trauma-focus, multipoint client-caregiver matchmaking, and family-centric services distinguish us from many others offering homecare services.
We have a uniquely designed hospital-to-home transition program that offers education, counseling, consultancy and resource planning services to the patient and/or their family members. The goal of this FREE program is to make the transition smooth and effective.
