At Collaborative Home Care, our number one goal is to help our clients age gracefully and maintain their independence in the comfort and safety of their home, whether they are living with a chronic illness, recovering from an injury, or needing companionship or help with personal care tasks around the home.
We structure our approach to care based on personalized wellness plans. We call it Proactive Wellness, and it focuses on three important areas - Safety, Nutrition, and Meaningful Activities. Instead of a task-oriented approach that traditional agencies follow, we take time to get to know our clients – their background, personality, and interests – to create a deeper, personalized connection. Proactive Wellness allows us to address the concerns you have for a loved one, synergizing the mind, body and the surrounding environment to create a comprehensive plan for reaching optimal wellness.
We proactively review our clients' care plans to ensure we are delivering the right care, and our caregivers are professionally trained to monitor for changes in our clients' health. With regular communications and check-in visits, our management team will be with you every step of the way as an added layer of support. Our home care services include:

They helped with cooking and cleaning.
I called a dozen different home care agencies when I was first looking. Of all the places I spoke with, this was the only place I spoke with the owner and he was very flexible with scheduling and I talked to him directly. He took the time to find a good fit. He did a phenomenal job.

My siblings and I used Collaborative Home Care to help our father out after he got out of the hospital. Overall we had a very positive experience using them. Our contact at Collaborative Home Care was professional, responsible and very responsive to our needs and questions. The aide always showed up on time and was good to our father. I would definitely recommend this agency to anyone in need of a home aide service.

Bryan, the leader of this company, is one of the most diligent people I have ever had the fortune to work with. As the owner of this company, he takes his clients’ needs very seriously and does his utmost to match his staff with client needs. He is a fabulous communicator and always gives prompt responses to queries and any problems that might arise. He first provided home care for my mom, it when my mom went into assisted living we terminated his services. Just a few weeks ago, my mom went from her assisted living facility into the hospital. After a week in rehab, I had two days to arrange supplemental care for my Mom. I called Bryan and he put the whole thing together with time to spare. In my opinion this was an amazing feat and I will always appreciate the effort Bryan put into this. If my Mom ever needs more help, you can be sure I’ll call Collaborative Home Care first!

I had a surgery in Greenwich recently. I did not have any friends or family in the area to help after the surgery with transportation. The owner Bryan was very responsive and able to help me safely get home from the surgery center and get situated once there. He also was able to help last minute when my recovery was not going as quickly as expected and I needed to move to another Airbnb to continue my recovery journey before traveling. My experience with them was very professional and caring and felt very safe. I appreciated their help even though I may not have been a typical client. They were timely, responsive, and primarily provided transportation for me.

They do whatever needs to be done and they always come.
I would recommend them because the owner is super efficient. He's very good to his employees and has very good retention of them. If something goes wrong and a caregiver can't come, he comes himself. I can't speak highly enough about him!

We are so incredibly pleased with the service we received from Collaborative Home Care. Bryan was very caring and asked us all the right questions to match our Parents up with the perfect caregiver. She was very attentive, caring and kind. She treated both our Parents really well, and understood how to care for each of them. Both of our Parents have dimentia. Our Mom is in a wheelchair and is incontinent. The caregiver that Bryan provided was wonderful in caring for both of them at our Sisters wedding. We will definitely use this wonderful company again anytime our Parents need to be transported and cared for!

The service and attention to detail we have received from Brian has been exceptional. Starting with the process of interviewing candidates for the home aide to continued monitoring of our satisfaction, we are pleased with the results.
Paulette , our caregiver, is knowledgeable and attentive. My mother broke several ribs and Paulette seamlessly helped her with her personal needs while she recovered. We highly recommend her.
Paulette worked around the clock yet was always attentive. When she did have to step out for a few hours, the agency provided fill-ins to cover her. Overall we have been very happy and would not hesitate to use the agency in the future if needed

Right now my mother-in-law is not very receptive to the caregiver. She is not getting the things she needs. The caregiver is unable to get her to do the things we need her to do.
I like that I can have personal communication with the owner. He has great communication.

Collaborative Home Care has been providing in-home care for my mother, who was recently diagnosed with Alzheimer's. I can't say enough good things about the care they've given her and the way they've interacted with my family. They've been super pro-active in many ways, checking in routinely with me and providing updates. This is a really stressful time for all of us, but their concern, responsiveness, punctuality, professionalism and kindness have made things so much easier. I'm incredibly impressed and grateful, and would not hesitate to recommend them to anyone looking for compassionate, professional in-home care.
