During April and May 2022, I spent 20 days in the Palms rehab unit after being discharged from the hospital with Parkinsons Disease and chronic back problems. 5 stars for the physical therapy and occupational therapy workers. I came in unable to walk. On day 20 I was able to stand and walk a short distance. But only 1 star for the Palms as a whole. Several nurses, especially on the late afternoon/early evening shift were outstanding, delivering medications promptly. Other nurses and staff were rude and even abusive and slow to respond to requests. Often it was more than 30 minutes at night before a nurse would respond to the buzzer. After my first night, I reported roaches coming out from behind my dresser and even pointed out the break in the air conditioner bottom where I saw some coming in and leaving. When I left on day 20, the opening had not been sealed and the roaches were still there.

I am Power of attorney over my mother. Nobody from the business office or a an employee named Rachel has contacted me about my mother. When I call the facility many times the phone rings until it hangs up. Poor communication between staff.

This place is clean but the the food they give these patients is unrecognizable and awful. You can never get a nurse on the phone when you want to talk to someone about your loved ones conditions. Some of the staff are nice some are downright rude. I don’t recommend this facility there are much better places.

Not sure if I would call this place a "rehabilitation" facility. Where do I start? First thing first: The Nursing Assistants I found for the most part to be nice, good hearted individuals who either lack skill or direction. Their answer to everything is more drugs. The rooms were fairly clean. The facility was fairly clean. The PT and OT were pretty good. I felt they can take charge of the program and not allow the patient to dictate what they're going to do. The biggest issue I had was with front office. Cannot get a straight answer from anyone. The Business Manager and the Case Manager seemed to be the worst! Very vague. We were lead to believe my father was worst off than he was. He was bind to a wheelchair for at least 21hrs a day. I really thought he couldn't walk. He was discharged on a Monday at 11am and by 3pm (I tell you no lie) my dad was walking and talking like the man I've always known. Overall, I think the Nursing Assistants mean well and have a tough job but I wouldn't recommend this place to anyone due to the lack of structure. It's feels like one big daycare and I'm ONLY referring to the short term residents.

Under no circumstances would I ever recommend this facility to anyone. The list of issues I have range from no response to call buttons for 45 minutes, inadequate staffing levels, very poor quality food, constantly being late in delivering pain medications, actually running out of medications, disinterested nurses, one evening they even ran out of clean towels and washcloths, assistants who do not do their jobs to constant apologies for the poor treatment but no real changes being made. Also found one unidentified person going through my personal papers while I was sleeping and then claiming she was doing an "inventory" of my things, 3 days after I had gotten here. Please, do not send anyone you care about to this facility. I just found out that I am allowed 2 showers per week at very specific times, and if I miss my time I will have to wait 3-4 days for my next one. Conditions are deplorable in my opinion. Please avoid this place for any reason. Only positive...rehab personnel are great at their jobs.

My impression of The Palms Rehabilitation and Healthcare Center was positive. The activities were quite good. It depends on what your needs are, but we thought they were pretty good for what we would be looking for. The food was quite good. They have physical therapy. It was a favorable facility.

We were able to put my mother in The Palms Rehabilitation and Healthcare Center. It’s not the best, but it’s all my parents can afford until Medicaid kicks in. My mom fell and was bruised; there was an investigation. I understand it happens especially with someone like my mother who takes a lot of medication thus she could become dizzy and lose her footing and balance, but I still wanted to know how it could be prevented from happening again. They do treat her very well. So far we’ve been satisfied.

Dad loved it there. Before he went in The Palms, every day, he would say he just wanted to die. He wouldn't do anything, I couldn't get him to go for a walk, all he did was watch TV. And now, he's in there and he's out and about the halls, he's doing activities, socializing, and he likes it really well. And everybody there seems to like him. He has Alzheimer's and he's 83 years old. It's a small facility and everybody knows everyone. They have a real nice courtyard that you can go out and sit out, it was nice and shaded with trees and stuff. He likes it and in his own mind he can get the bus right out front and he's been going back to Chicago where he's brought up. I felt very fortunate that I found a place so close to me.

The Palms was a very nice assisted living facility. When I first arrived I noticed that there were a lot of people working there. There are two sides of the facility. One side is dedicated to rehabilitation and the other side is dedicated to long term stay. The Long term stay side had lots of activites that the residents could participate in. There were elcetions for council, games, day trips and many other fun things to do. The staff consisted of mostly haitain women. They were not extremely friendly but they did a great job attending to the residents. I never realized how easy it was to become a CNA. Only a couple weeks classes and BAM! You're a CNA. It explained why so many women who seemed to be very uneducated could hold such a virtuous positions as a "nurse". The palms over all was pretty clean but had it's downfalls. The shower area was one place of great concern. One man was noted to have defacated in the shower. The only means of cleaning that were performed was to just wash it away with the shower head. No sanitizer or anything!
